Determinants of neonatal hypoglycemia among neonates admitted to neonatal intensive care units of public hospitals in Wolaita Zone, Southern Ethiopia, 2023: An unmatched case-control study.

Globally, neonatal hypoglycemia is a common child health problem and significantly contributes to morbidity and mortality, with its impact being particularly detrimental in developing countries. Although being a prevalent metabolic condition, it is frequently overlooked. Furthermore, the problem is not adequately studied in Ethiopia, as seen by a few published studies on the topic, highlighting the lack of knowledge about its determinants. This study aims to assess the determinants of neonatal hypoglycemia among neonates admitted to neonatal intensive care units of public hospitals in Wolaita Zone, Southern Ethiopia, 2023. An institution-based unmatched case-control study design was conducted among 249 (83 cases and 166 controls) participants. Data were collected from 29 March to 23 May 2023 using a pretested chart review extraction tool/checklist. A consecutive sampling method was used for participant selection. Data were analyzed using Statistical Package for Social Science version 25. Binary logistic regression analysis was used to identify the determinants of neonatal hypoglycemia, and statistical significance was declared at P < 0.05. A total of 83 cases and 166 controls were included in the study. The following factors were significantly associated with neonatal hypoglycemia: preterm neonates [adjusted odds ratio (AOR) 4.888 (95% confidence interval, CI, 1.113-21.478)], age of the neonate at admission (3-5 h) [AOR 4.205 (95% CI 1.852-9.547)], hypothermia [AOR 5.485 (95% CI 2.360-12.748)], late initiation of breastfeeding [AOR 6.229 (95% CI 2.665-14.599)], mode of delivery [AOR 5.034 (95% CI 1.688-15.011)], and small for gestational age [AOR 3.645 (95% CI 1.286-10.330)]. In the current study, numerous determinants of neonatal hypoglycemia have been identified. Therefore, it is crucial to give due emphasis to providing comprehensive health education to mothers regarding effective breastfeeding methods, strategies to avoid neonatal hypoglycemia, and the various factors that heighten its likelihood. In addition, healthcare professionals should emphasize implementing preventive measures to prevent this grave condition.

Challenges Faced by Midwives who Care for Neonates in Neonatal Intensive Care Units of Public Hospitals in Limpopo Province, South Africa

Background Providing quality neonatal care is one of the most challenging healthcare service issues globally. Ensuring quality care during pregnancy and delivery is paramount to improving health outcomes and reducing mortality among neonates. About 2.9 million neonatal deaths occur worldwide, accounting for 40% under 5 years’ deaths. Midwives in the neonatal intensive care units of public hospitals in Limpopo province encounter challenges such as shortage of staff, inadequate and malfunctioning equipment, etc. when rendering care to small and sick neonates. Objective The purpose of this study was to explore and describe the challenges encountered by midwives who provide care to neonates at neonatal intensive care units of Public hospitals in Limpopo province, South Africa. Methods A qualitative, explorative, descriptive and contextual research designs were used. A non-probability purposive sampling was employed to select twelve (12) midwives who constituted the sample. The sample size was not pre-determined, however, it was determined by data saturation. Data were collected through one-on-one semi-structured interviews using an interview guide. Tesch’s eight steps were utilised to analyse data wherein themes and sub-themes emerged. Results The results revealed that midwives encounter several challenges such as shortage of staff, inadequate infrastructure or structure of NICU, work overload, lack of equipment and supplies, etc., when providing care to neonates in NICUs of the public hospitals. Conclusion The study recommended the implementation of hiring additional staff, staff training in neonatal care specialty and short courses, renovations of NICUs, consistent provision of equipment and material resources to improve the quality of neonatal care and reduce neonatal mortality.

Time to recovery and predictors among admitted preterm neonates in the neonatal intensive care units of public hospitals of Addis Ababa, Ethiopia, 2021

IntroductionEthiopia implemented measures to reduce preterm mortality, and much is currently being done to avoid preterm death, yet preterm death remains the top cause of infant death. As a result, evaluating median time of recovery and determinants will provide information to planners and policymakers to design strategies to improve preterm survival.MethodsHospital-based retrospective follow-up study was conducted in four selected public hospitals of Addis Ababa from September 2018 to August 2021. Data were collected using a pretested structured questionnaire. Epi-data 4.6 and STATA Version 16 were used for data entry and analysis. Kaplan-Meier survival curve, log-rank test, and median time were computed. To find predictors of time to recovery, a multivariable Cox proportional hazards regression model was fitted, and variables with a p-value less than 0.05 were considered statistically significant.ResultsA total of 466 preterm babies were included in the study of which 261 (56.1%) preterm neonates survived and were discharged from NICUs. The median time to recovery was 10 days (95% CI: 9–12). Low birth weight (Adjusted hazard-ratio [AHR]: 1.91, 95% CI: 1.2–3.06), normal birth weight (AHR: 2.09, 95% CI: 1.16–3.76), late preterm (AHR: 1.91, 95% CI: 1.02–3.55), no hospital-acquired infection (AHR: 2.19, 95% CI: 1.36–3.5), no thrombocytopenia (AHR: 1.96, 95% CI: 1.27–3.02), continuous positive airway pressure (AHR: 0.66, 95% CI: 0.48–0.91), and kangaroo mother care (AHR: 2.04, 95% CI: 1.48–2.81) were found to be independent predictors of time to recovery of preterm babies.Discussion/ConclusionThe recovery rate was found relatively low. Several predictors of preterm recovery time were discovered in the study. The majority of predictors were preventable or treatable. Therefore, emphasis should be given towards prevention and early anticipation, and management of these predictors. Studies to assess the quality of care and cause of low survival rate of preterm infants are recommended.

Knowledge, Practice, and Associated Factors Towards Supplemental Oxygen Therapy Among Nurses Working in Neonatal Intensive Care Units in Public Hospitals of Addis Ababa, Ethiopia, 2022: A Cross-Sectional Study.

Proper administration of oxygen therapy for hospitalized newborns is undoubtedly essential. World Health Organization estimates that approximately 1.4 million deaths worldwide each year are due to inappropriate administration of oxygen, but its administration in clinical practice is usually inappropriate due to lack of knowledge and practice. Therefore, the aim of this study was to determine the level of knowledge, practice and associated factors related to supplemental oxygen therapy among nurses working in neonatal intensive care units. An institutional cross-sectional study was conducted with 166 randomly selected respondents using a self-administered questionnaire for data collection. Bivariate and multivariable analyses were performed using SPSS version 26. Variables with a p-value of <0.25 in the bivariate analysis were included in the multivariable logistic regression, and statistical significance was ensured by a p-value <0.05 with 95% CI. One hundred and sixty-six nurses took part in this study, with a response rate of 96%. Of these, 60.8% had good knowledge and 54.2% had good practice in supplemental oxygen therapy. In the multivariable logistic regression analysis, work experience was significantly associated with knowledge (AOR: 5.40; 95% CI: 2.12-13.73)) and practice (AOR = 3.69, 95% CI: 1.30-10.44)) of supplemental oxygen therapy. Level of knowledge (AOR=2.92, 95% CI: (1.24-6.86)) and educational status (AOR=6.51, 95% CI (1.34-31.55)) were also significantly associated with the practice of supplemental oxygen therapy with a p < 0.05. Approximately 40% and 46% of nurses in this study had poor knowledge and practice of supplemental oxygen therapy. This figure is shocking and shows a large gap in the knowledge and practice of supplemental oxygen therapy that requires urgent action. Educational status, professional experience and level of knowledge were found to be associated with good knowledge and professional experience with good practice in relation to supplemental oxygen therapy.

Knowledge and Practice of Neonatal Pain Management and Associated Factors among Health Care Providers in Neonatal Intensive Care Units of Public Hospitals in North Shoa Zone, Amhara Regional State, Ethiopia, 2023.

Neonatal pain has been underrecognized and undertreated in many settings with the youngest children and neonates suffering the greatest consequences. Despite recent advancements in the assessment and therapy of children's pain, a knowledge-to-practice gap still exists especially in developing nations including our country Ethiopia. To assess knowledge, practice, and associated factors towards neonatal pain management among health care professionals working at neonatal intensive care units of public hospitals in North Shoa Zone, Amhara regional state, Ethiopia. Facility-based cross-sectional study design was conducted among health care professionals working at NICU in all public hospitals in North Shoa Zone, from May 9, 2023, to May 23, 2023. Data were collected using a self-administered questionnaire from 123 health care professionals working at NICUs in eleven public hospitals. The collected data were checked and entered into EPI data version 3.1 and then exported to SPSS version 25 for further descriptive and logistic regression analysis. Data are summarized using descriptive statistics and presented using narrations, tables, and graphs. The study reveals that 48% of health care professionals possessed adequate knowledge, while only 5.7% demonstrated good practices in neonatal pain management. Factors significantly associated with better neonatal pain management practices included having good knowledge of neonatal pain management (AOR = 3.36, 95% CI [1.19-9.49]), a higher educational level (AOR = 4.48, 95% CI [1.62-13.88]), and the availability of a pain scale assessment tool in the neonatal unit (AOR = 3.91, 95% CI [1.50-10.20]). Moreover, the type of profession, specifically being a nurse (AOR = 0.23, 95% CI [0.08-0.66]), was significantly associated with knowledge of neonatal pain management. Health care professionals' knowledge and practice regarding neonatal pain management are insufficient. Multisectoral collaboration is essential to enhance their knowledge and skills and improve the availability of pain scale assessment tools and supportive materials.

Early Neonatal Mortality and Predictors in Newborns Admitted to the Neonatal Intensive Care Unit at Public Hospitals in Hadiya Zone, Central Ethiopia: A Retrospective Cohort Study.

Background. Despite numerous life-saving measures, neonatal mortality remains high. This research aims to investigate the incidence and predictors of early neonatal mortality among newborns admitted to intensive care units in public hospitals in Hadiya Zone, Ethiopia. Methods. A retrospective cohort study was conducted on 689 neonates admitted to the neonatal intensive care unit. Cox proportional hazard regression by STATA was used. Results. This study followed for 3439 person-days found an incidence rate of 16.9 deaths per 1000 person-days. Birth weight [AHR = 4.4, 95% CI; 1.29, 14.94], APGAR score at the fifth minute 4 to 6 [AHR = 0.42, 95% CI; 0.2, 0.87], hypoglycemia [AHR = 8.1, 95% CI; 2.17, 30.43], no treated with oxygen [AHR = 2.6, 95% CI; 1.1, 5.9], and obstetric complications [AHR = 0.41, 95% CI; 0.18, 0.93] predicted early neonatal mortality. Conclusion. The study revealed a high neonatal mortality rate, necessitating increased focus on oxygen treatment for newborns and improved early diagnosis and treatment of obstetric complications.

Sepsis Risk Factors in Neonatal Intensive Care Units of Public Hospitals in Southeast Ethiopia, 2020: A Retrospective Unmatched Case-Control Study.

Neonatal sepsis is a significant contributor to neonatal morbidity and mortality worldwide. It is more prevalent in developing countries. Thus, understanding the risk factors for neonatal sepsis is critical to minimizing the incidence of infection, particularly in Ethiopia. The purpose of this study was to identify the risk factors for neonatal sepsis in neonates admitted to neonatal intensive care units of public hospitals in Southeast Ethiopia in 2020. An institution-based, retrospective unmatched case-control study was conducted on 97 cases and 194 controls in neonatal intensive care units of public hospitals in Southeast Ethiopia. A pretested, structured questionnaire was used to collect the data. Data was entered using EpiData 3.1 and analyzed using SPSS version 23. Bivariable and multivariable logistic regression analyses were performed to identify associated factors. An adjusted odds ratio with a 95% confidence interval was used to determine the degree of association, and statistical significance was declared at a p value of < 0.05. In this study, 97 cases and 194 controls were included. About two-thirds (63.9%) of cases were with early onset neonatal sepsis (<7 days). Mode of delivery with spontaneous vaginal delivery (AOR:5.032; 95% CI (1.887-13.418)), type of birth attendant (traditional birth attendant) (AOR: 4.407 95% CI (1.213,16.004)), history of STI/UTI (AOR:2.543; 95% CI (1.313,4.925)), intrapartum fever (AOR:4.379; 95% CI (2.170,8.835)), APGAR score at the 5thminute < 7 (AOR:4.832; 95% CI (1.862,12.537)), neonate received resuscitation (AOR:3.830; 95% CI (1.753,8.369)), low birth weight (AOR:6.101; 95% CI (2.124,17.525)) were the identified risk factors for neonatal sepsis. Both maternal and neonatal factors contribute to the risk of neonatal sepsis. Spontaneous vaginal delivery, birth attended by the traditional birth attendant, history of STI/UTI, presence of intrapartum fever, low APGAR score at the 5th minute, neonate receiving resuscitation, and low birth weight were identified as independent risk factors for neonatal sepsis. Prompt identification of the aforementioned factors and management should be sought for all newborns.

Mothers’ perceptions and experiences of caring for sick newborns in Newborn Care Units in public hospitals in Eastern Uganda: a qualitative study

IntroductionMothers’ participation in the care of their sick newborns in Newborn Care Units (NCUs) has been linked to several advantages including earlier discharge, fewer complications, better mother–baby bonding, and an easier transition to home after discharge. This study aimed to understand mothers’ perceptions and experiences while participating in the care of their sick newborns in the NCUs to inform interventions promoting mothers’ participation in public health facilities in Uganda.MethodsWe conducted an exploratory qualitative study comprised of 18 in-depth interviews with mothers caring for their newborns in two NCUs at a Regional Referral and General hospital in Eastern Uganda between April and May 2022. The interviews were audio-recorded and then transcribed. For analysis, we used a thematic analysis approach.ResultsThe fear of losing their baby was an overarching theme that underlay mothers’ perceptions, actions, and experiences in the NCU. Mothers’ confidence in the care provided to their babies was based on their baby’s outcomes. For example, when mothers saw almost immediate improvement after treatment, they felt more confident in the care than when this was not the case. Furthermore, mothers considered it essential that health care providers responded quickly in an emergency. Moreover, they expressed concerns about a lack of control over their personal space in the crowded NCU. Additionally, caring for babies in these settings is physically and financially taxing, with mothers requiring the combined efforts of family members to help them cope.ConclusionThis study shows that for mothers of sick newborns in the NCU, the baby’s survival is the first concern and the basis of mothers’ confidence in the quality of care provided. Efforts to improve parental participation in NCUs must focus on lowering the costs incurred by families in caring for a baby in the NCU, addressing privacy and space concerns, leveraging the family’s role, and avoiding compromising the quality of care in the process of participation.

Birth asphyxia, determinants, and its management among neonates admitted to NICU in Harari and Dire Dawa Public Hospitals, eastern Ethiopia.

Despite a declining neonatal mortality rate globally, Ethiopia has scored 29-30 deaths per 1,000 live births. Birth asphyxia is a major contributor to neonatal mortality, where 4-9 million newborns develop birth asphyxia each year. This study aimed to assess the prevalence of birth asphyxia, its determinants, and its management among neonates admitted to the NICU in Harari and Dire Dawa public hospitals. A facility-based cross-sectional study was conducted among 409 randomly selected neonates and their index mothers admitted to neonatal intensive care units of public hospitals in Harari and Dire Dawa from June 20 to August 20, 2021. Data were collected through card review and interviewer-administered questionnaires. The collected data were entered into Epi data version 3.1 and exported to SPSS version 20 for analysis. Logistic regression models were fitted to identify factors associated with birth asphyxia. Adjusted odds ratios along with 95% CIs were estimated to measure the strength of the association, and statistical significance was declared at p-value <0.05. One-fifth of neonates [20.8% (95% CI: 16.4, 24.6%)] had birth asphyxia. Neonates born by instrumental delivery (AOR = 2.29, 95% CI: 1.10, 4.76) and neonates born to mother with PIH (AOR = 3.49, 95% CI: 1.47, 8.27), PROM (AOR = 2.23, 95% CI: 1.17, 4.26), and chorioamnionitis (AOR = 3.26, 95% CI: 1.10, 9.61) were more likely to have birth asphyxia compared to their counterpart. Ventilation with a bag and mask 50(58.8), putting on free oxygen 19(22.4), and endotracheal intubation 15(17.6) were taken as management methods. One out of five neonates had birth asphyxia. This urges care providers to adhere to national guidelines of obstetrics and neonatal continuum care. They also need to decrease instrumental delivery and treat PIH, PROM, and chorioamnionitis.

Determinants of neonatal jaundice among neonates admitted to neonatal intensive care unit in public hospitals of Sidama Region, Sidama, Ethiopia, 2022: an unmatched case-control study.

neonatal jaundice appears in most neonates as "physiological jaundice" in the first few weeks of life; however, pathological jaundice is associated with an increased risk of long-term complications and mortality only a few studies have been conducted on the determinants of neonatal jaundice in Ethiopia. The aim of this study was to identify the determinants of neonatal jaundice (pathological) among neonates admitted to neonatal intensive care units in Sidama Region general and referral public hospitals. a hospital-based unmatched case-control study was conducted among 270 neonates in public hospitals of Sidama Region from June 23 to August 8, 2022. We used a pre-tested interviewer-administered questionnaire and collected by open data kit (ODK) then the data was downloaded and exported to Microsoft Excel worksheets (XLS) and imported to SPSS version 26 for further analysis. Bi-variable logistic regression analysis was performed. Variables with a P-Value of less than 0.25 were included in multivariable logistic regression. Multi-variable logistic regression was performed and Adjusted Odds ratio (AOR) with a 95% confidence interval was computed and statistical significance was declared at a p-value <0.05. a total of 270 neonates with mothers (90 cases and 180 controls) with a response rate of 100% were included in this study. Factors significantly increased the odds of developing neonatal jaundice were multiparty (AOR=2.869(95%CI 1.426-5.769)), prolonged duration of labor (AOR=4.618(95%CI 1.689 - 12.625)). ABO incompatibility (AOR=3.362(95%CI, 1.185 - 9.537)). Preterm (AOR=2.936(95%CI, 1.2456.923)), birthasphyxia (AOR=2.278(95%CI,1.1454.531)) and polycythemia (AOR=3.397(95%CI, 1.147-10.061)). in this study multiparty, prolonged duration labor, ABO incompatibility, preterm gestational age, birth asphyxia, and polycythemia were factors that significantly increased the odds of developing neonatal jaundice.

Occupational Exposure to Blood and Body Fluids Among Nurses in the Emergency Department and Intensive Care Units of Public Hospitals in Addis Ababa City: Cross-sectional Study.

This study investigated occupational exposure to blood and body fluids among nurses at the emergency department and intensive care units of public hospitals in Addis Ababa city. A cross-sectional study was conducted from June 18, 2021 to September 20, 2021. A simple random sampling method and semi-structured, self-administered questionnaires were used to collect the data, which were analyzed using SPSS version 25. A binary logistic regression model was used to identify factors associated with exposure to blood and body fluids on the AOR with a 95% confidence interval at a P-value of <.05. No patients were involved in this study. Of the total study participants (260), 198 (76%) nurses had been exposed to blood and body fluids in their professional live, and 167 (64%) were exposed to blood and body fluids in the last 12 months. Being male (AOR = 2.88, 95% CI: (1.35, 6.12), lacking access to handwashing facilities (AOR = 5.02, 95% CI: (3.73, 14.51)), not consistently wearing all the required types of personal protective equipment (AOR = 6.21, 95% CI: (2.39, 9.55)), and the lack of the required personal protective equipment (AOR = 5.53, 95% CI: (1.87, 10.38)) were all significant factors that were positively associated with exposure to blood and body fluids. This study showed that a higher proportion of nurses in the emergency department and intensive care unit were exposed to blood and body fluids in the study setting. Most nurses do not consistently wear all the required types of personal protective equipment, putting them at a higher risk of acquiring blood-borne pathogens. The authors recommended that there is a need to implement and strengthen appropriate and consistent use of all required personal protective equipment during any procedure, and patient care to reduce exposure to blood and body fluids. This study provides baseline information for other action-based studies to assess exposure to blood and body fluids among nurses in emergency and intensive care units. This manuscript was prepared based on the strobe checklist guidelines. The study's findings and recommendations might be used for healthcare providers, non-governmental organizations, and policymakers for appropriate planning and interventions to minimize or safeguard nurses' exposure to blood and body fluids.

Knowledge, Attitudes, Practices, and Associated Factors Towards Care of Elderly Patients among Nurses.

Ethiopia's elderly care service is relatively young and little is known about the practices or readiness of nurses to provide care. Nurses need to have good knowledge, a positive attitude, and experience when it comes to caring for the elderly or patients who are chronically providing quality care. This study aimed to assess the knowledge, attitudes, and practices towards care for elderly patients and associated factors among nurses working in adult care units of public hospitals in Harar, 2021. An institutional-based descriptive cross-sectional study was conducted from February 12 to July 10, 2021. A simple random sampling technique was used to select 478 study participants. Data were collected by trained data collectors using a pretested self-administered questionnaire. From the pretest, Cronbach's alpha for all items was above 0.7 for all items. Data were entered into Epi Data v.4.6 and exported to the Statistical Package for Social Science Version 26 for binary logistic regression analysis. A P-value of 0.05 was used to declare a significant association between variables. The study revealed that 311 (69%) had inadequate knowledge. Having a first degree and an unfavorable attitude towards nurses showed a statistically significant association with nurses' inadequate knowledge. A total of 275 (61.0%) nurses were found to have an unfavorable attitude and were significantly associated with having a diploma and first degree, learning in a private organization, 6 to 10 years of experience, lack of training, and inadequate knowledge of nurses. A total of 297 (65.9%) study units had inadequate practice towards the care of elderly patients. Nurses' practices showed a significant association with the type of hospital, work experience, and adherence to guidelines with a response rate of 94.4%. The majority of nurses had inadequate knowledge, an unfavorable attitude, and inadequate practice towards the care of elderly patients. Having a first-degree and an unfavorable attitude with inadequate knowledge, lack of training and inadequate knowledge, unfavorable attitudes and less than 11 years of experience, working in non-academic hospitals, and unavailability of guidelines with inadequate practices were significantly associated.

Para além do imaginável: experiências vividas por profissionais de saúde em UTI durante a pandemia da Covid-19

Abstract The study aimed to analyze the experiences lived by professionals related to changes in health work in the Intensive Care Unit, during the critical period of the first wave of the Covid-19 pandemic in Maranhão. This is a descriptive-exploratory study with a qualitative approach, carried out in the Intensive Care Units of public hospitals in Maranhão, from November 2020 to January 2021. Fifteen professionals took part in the study. Data collection was done through semi-structured interviews, with the sample defined by the criterion of saturation of meanings. Content analysis and NVIVO® 12 software were used. It emerged that work overload, shortage of professionals and fear of contagion affected the quality of care and generated new ways of producing care. In that context, providing "high standard" care was a challenge cutting across the social and technical spaces of intensive care. Experiencing deaths on a larger scale affected intersubjective relationships in personal and professional spheres. Changes in the organization of space, care and interprofessional relationships may indicate ways to rethink the effects of this phenomenon on agents, users and services, as well as providing greater skills to deal with future emergency scenarios.

Parental Satisfaction with Care and Support in Neonatal Care Units of Public Hospitals of Nepal

Introduction: Parental satisfaction is one of the indicators of quality care. An understanding of the satisfying areas would help to identify and prioritize care and support areas for infants and parents. Therefore, this study was conducted to find out the parental satisfaction with care and support in neonatal care units (NCUs). Methods: Cross-sectional descriptive study was conducted in NCUs of five selected public hospitals. The study was conducted among 305 parents of preterm infants (PTIs). After obtaining ethical approval, in-person interviews were conducted with parents using a satisfaction questionnaire. Both descriptive and inferential statistics were used for data analysis. Results: Parents were moderately satisfied with care and support with a median score and interquartile range of 3.4 (3.1- 3.8). They were highly satisfied with the infant care and least satisfied with the attachment and care guidance support with median scores of 4.0 (3.5 - 4.0) and 3.1 (2.6 - 3.8) respectively. Parental satisfaction was significantly associated with parental age, gestational age at birth, and birth weight (P value &lt; 0.05). Conclusions: Parents are moderately satisfied with the care and support received in the NCUs. Their satisfaction is lower with the support for attachment and care guidance. In addition to competent and affectionate care to PTIs, parental support for attachment, guidance, and involvement in care need to be considered by NCU personnel.

Admission Hypothermia and Factors Associated with Mortality among Admitted Hypothermic Preterm Neonates in Neonatal Intensive Care Units of Public Hospitals of Addis Ababa, Ethiopia.

Background Limited knowledge on the proportion of admission hypothermia and factors of death in hypothermic preterm neonates is hindering early and appropriate interventions in Ethiopia. Thus, studies on the proportion of admission hypothermia and factors of death in hypothermic preterm neonates are critical to enhancing preterm infants' survival. Methods Hospital-based cross-sectional study was conducted on 398 participants using the systematic sampling method from October 10, 2021, to December 15, 2022. A pretested data extraction tool was used to collect data. EpiData version 4.6 and STATA version 16 were used for data entry and analysis. Multivariable logistic regression analysis evaluated the association between independent and outcome variables with a 95% confidence interval (CI). Hosmer and Lemeshow test and variance inflation factor were assessed to check model fitness and collinearity, respectively. Pvalue < 0.05 was considered statistically significant. Result Of the 398 admitted preterm neonates, 331(83.2%) had hypothermia at admission. Factors that were significantly associated with mortality included outborn babies [Adjusted hazard ratio (AOR) = 2.18 : 95% CI (1.03-4.62)], GA less than 32 weeks [AOR = 6.64 : 95% CI (1.87-13.58)], weight less than 1500 gram [AOR = 7.91 : 95% CI (1.21-15.38)], thrombocytopenia [AOR = 3.36 : 95% CI (1.49-7.58)], and kangaroo mother care [AOR = 0.38 : 95% CI (0.16-0.88)]. Conclusion The proportion of admission hypothermia was high. Outborn babies, birth weight less than 1500 gram, gestational age < 32 weeks, being thrombocytopenic, and lack of kangaroo mother care were identified as risk factors for hypothermic preterm neonatal mortality. Preterm labor prevention, improved inborn delivery, and kangaroo mother care may alleviate the high proportion of admission hypothermia and related mortalities in preterm neonates.

Determinants of time to full enteral feeding achievement among infants with birth weight 1000-2000g admitted to the neonatal intensive care unit of public hospitals in Hawassa city, Sidama region Ethiopian, 2019: A retrospective cohort study

BackgroundNutritional care during the neonatal period is a cornerstone towards achieving optimal care. However, very limited data is available on optimizing parenteral and enteral nutrition that directly affects infant survival among Ethiopian neonates. Therefore, the objective of this study is to identify determinants of time to full enteral feeding achievement among low-birth-weight neonates admitted to neonatal intensive care units of public hospitals in Hawassa city.MethodsA facility-based retrospective cohort study was conducted in Adare general hospital and Hawassa University’s comprehensive specialized hospital from August 2018 to 2019. Charts of infants with a birth weight of 1000-2000g (n = 273) neonates who were admitted to the neonatal intensive care unit (ICU) were reviewed. The sample size for each hospital was allocated proportionally and subjects were selected by using a simple random sampling technique. Data were entered using Epi. data version 3.1, and analysis was performed using SPSS version 20. Kaplan-Meier estimator and a Cox proportional hazard model were used.ResultThe mean (SD) age when an enteral feed (trophic feeding) was first commenced was 2.13(1.373) days. The median time to achieve full enteral feeding was 8 days with IQR (7–10 days). Gestational age reduces the time to full enteral feeding by 18.8% for each additional week of gestation (AHR = 0.812, p-value = 0.003). The time to achieve full enteral feeding was shorter by 70.4% among neonates who were small for gestational age, as compared with that appropriate for gestational age (AHR = 0.296, p-value<0.001).ConclusionAccording to this study, the time that the neonate takes to achieve full enteral feeding was relatively short. Gestational age and weight for gestational were the determinants for time to full enteral feeding achievement. Further research needs to be conducted to explore further, in addition to current findings.

Pain assessment practice and associated factors among nurses working at adult care units in public hospitals in Wolaita Zone, Southern Ethiopia, 2021

BackgroundUsing standard pain assessment techniques is a cornerstone for effective pain management. Pain is not assessed in a standardized manner in numerous practice settings. The problem of applying pain assessment principles was found to be higher in low-income countries. Very limited evidence indicates the standard of pain assessment practice among nurses in Ethiopia. Therefore, the purpose of this study was to identify pain assessment practice and associated factors among nurses working at adult care units in public hospitals in the Wolaita Zone, Southern Ethiopia.MethodsA cross-sectional study was conducted among 290 nurses working at adult care units in public hospitals in Wolaita zone. Data were collected from February to March 2021. A structured self-administered questionnaire was used for data collection. Epi Data 4.6 was used to enter the data, and SPSS version 26 was used to analyze the data. A logistic regression model was used, and statistical significance was declared at P ≤ 0.05. An adjusted odds ratio with 95%CI was used to measure the degree of association.ResultsA total of 267 nurses participated in the study, giving a response rate of 97.8%. Almost three-fourths (73.8%) of the study nurses reported that they assessed pain for their patients. Only 23.6% of the study nurses discussed pain assessment scores during a nurse-to-nurse report. Below, half (47.2%) of the study participants documented pain assessment scores. The proportion of nurses with good pain assessment practice was found to be 38.2%.The odds of having good pain assessment practice among nurses who received training on pain management was two times higher than its counterpart. The nurses who perceived organizational support were twice more likely to have good pain assessment practice than their counterparts.ConclusionNurses’ pain assessment practice was found to be low. Moreover, a substantial proportion of the study nurses did not discuss pain assessment scores during a nurse-to-nurse report with low documentation practice. Continuous professional development through in-service training and education is crucial to the improvement of nurses’ pain assessment practice. Furthermore, ameliorating organizational support by means of a supportive working environment is suggested to the betterment of nurses’ assessment practice.

Survival and predictors of asphyxia among neonates admitted in neonatal intensive care units of public hospitals of Addis Ababa, Ethiopia, 2021: a retrospective follow-up study

BackgroundGlobally, perinatal asphyxia (PNA) is a significant cause of most neonatal deaths. Similarly, the burden of birth asphyxia in Ethiopia remains high (22.52%) and has been noted the second leading cause of neonatal mortality. Thus, researches on survival status and predictors of perinatal asphyxia are critical to tackle it. Therefore, the current study intended to determine the survival status and predictors of asphyxia among neonates admitted in Neonatal Intensive Care Units of public hospitals, Addis Ababa, Ethiopia.MethodsHospital-based retrospective follow-up study was conducted in four selected public hospitals of Addis Ababa from January 2016 to December 2020. Data were collected using a pretested structured questionnaire. Epi-data 4.6 and STATA Version 16 was used for data entry and analysis, respectively. Kaplan–Meier survival curve, log-rank test and Median time were computed. To find the predictors of time to recovery, a multivariable Cox proportional hazards regression model was fitted, and variables with a P-value less than 0.05 were considered statistically significant. Finally, the Schoenfeld residual test was used to check overall model fitness.ResultFour hundred eleven admitted asphyxiated babies were followed a total of 3062 neonate-days with a minimum of 1 h to a maximum of 28 days. The Overall incidence density rate of survival was 10 (95% CI: 0.08–0.11) per 100 neonate-days of observation with a median recovery time of 8 days (95% CI: 7.527–8.473). Low birth weight (Adjusted hazard ratio [AHR]: 0.67, 95% CI: 0.47–0.96), stage II hypoxic ischemic encephalopathy (HIE) (AHR: 0.70, 95% CI: 0.51–0.97), stage III HIE (AHR: 0.44, 95% CI: 0.27–0.71), seizure (AHR: 0.61, 95% CI: 0.38—0.97), thrombocytopenia (AHR: 0.44, 95% CI: 0.24–0.80) and calcium gluconate (AHR: 0.75, 95% CI: 0.58–0.99) were found to be independent predictors of time to recovery of asphyxiated neonates.ConclusionIn the current findings, the recovery time was prolonged compared to others finding. This implies early prevention, strict monitoring and taking appropriate measures timely is mandatory before babies transferred into highest stage of HIE and managing complications are recommended to hasten recovery time and increase survival of neonates.

Professional quality of life of nurses in critical care units: Influence of demographic characteristics

Background Professional quality of life, measured as compassion satisfaction, is a prerequisite for nurses working in intensive care units where patients rely on their care. Nurses who experience compassion satisfaction, or good professional quality of life, engage enthusiastically with all work activities and render quality patient care. In contrast, compassion fatigue eventually leads to disengagement from work activities and unsatisfactory patient outcomes. In this study, we described the demographic factors influencing professional quality of life of intensive care nurses working in public hospitals in Gauteng, South Africa (SA), during the first wave of the COVID-19 pandemic. Objectives To describe the demographic factors associated with professional quality of life of critical care nurses working in Gauteng, SA Methods In this cross-sectional study, we used total population sampling and invited all nurses who had worked for at least 1 year in one of the critical care units of three selected public hospitals in Gauteng to participate. One-hundred and fifty-four nurses responded and completed the ProQol-5 tool during the first wave of the COVID-19 pandemic. Data were analysed using descriptive and inferential statistics. Results The nurses’ average age was 45 years, and 59.1% (n=91) had an additional qualification in critical care nursing. Most of the nurses had a diploma (51.3%; n=79), with a mean work experience of 12.56 years. The main demographic variables that influenced professional quality of life were years of work experience (p=0.047), nurses’ education with specific reference to a bachelor’s degree (p=0.006) and nurse-patient ratio (p<0.001). Conclusion Nurses working in critical care units in public hospitals in Gauteng experienced low to moderate compassion satisfaction, moderate to high burnout and secondary traumatic stress, suggesting compassion fatigue. The high workload, which may have been associated with the COVID-19 pandemic, influenced nurses’ professional quality of life. Contributions of the study This study reports on the important problem of compassion fatigue and burnout amongst South African ICU nurses working in the public sector. Associated factors were identified, which should be addressed to improve nurses’ wellbeing.
