FOUR ADULT CASES OF INTUSSUSCEPTION

We report 4 adult cases of intussusception. The primary lesions were comprised of one benign tumor and 3 malignant tumors, that is, a lipoma of the cecum, malignant lymphoma of the ileum, transverse colon carcinoma, and sigmoid colon carcinoma. The male-to-female ratio was one to 3. Three malignant patiens were in their seventies. Preoperative freeing of intussusception was succeeded in only one case, and the remained 3 underwent emergency operation. It is said that adult intussusception is found in 5-10% of all cases of intussusception including infant cases, and 80-85% of adult cases commonly have an occult organic disease of the intestine. When classified by the regions, ileocecal intussusception is most frequently found, and colic intussusception has a low rate of incidence.Intussusception due to a lipoma of the caecum is rare.

Isolation and partial characterization of two glycoproteins from human liver metastases of sigmoid colon carcinoma

1. 1. Perchloric acid-soluble glycoprotein fraction (PASF) extracted from human liver metastases (LM) of sigmoid colon carcinoma was chromatographed on a DEAE-cellulose column. The main fraction (DEAE-nonadsorbed fraction) passed through the column was then subjected to Sephacryl S-200 superfine gel filtration and separated into 12 fractions. 2. 2. Among 12 fractions, only both Fractions 3 and 4 were demonstrated to be chemically and immunologically homogeneous glycoproteins, respectively, by a combination of chemical composition analysis, SDS-PAGE and EITB assay using antisera against the DEAE-nonadsorbed fractions of PASFs from human LMs, normal liver (NL) and normal sigmoid colon (NSC). Each of Fractions 3 and 4 reacted with anti-LM serum to give one immuno complex on a nitrocellulose sheet in EITB assay, but did not react with anti-NL and -NSC sera. 3. 3. Apparent molecular weights of 80,900 and 62,100, respectively, were found for Fractions 3 and 4. Both the fractions, respectively, had abnormal sugar compositions. Fraction 3 contained sialic acid, fucose, galactose, N-acetylglucosamine and N-acetylgalactosamine, but lacked glucose and mannose, and Fraction 4 contained sialic acid, fucose, galactose and N-acetylglucosaminne, but lacked glucose, mannose and N-acetylgalactosamine, as sugar components.

The impact of radioimmunoguided surgery (RIGS) on surgical decision-making in colorectal cancer.

Radioimmunoguided surgery (RIGS system) was performed in ten patients with rectal or low sigmoid colon carcinoma with the use of a hand-held gamma detector (Neoprobe 1000) intraoperatively and externally after injection of radiolabeled (125I) monoclonal antibody to detect pelvic and metastatic tumor. Fifteen procedures, including six exploratory laparotomies, four transperineal explorations, two transsacral explorations, one transvaginal biopsy, one brachytherapy, and one transanal polypectomy, were performed. Two patients had previous low anterior resection, seven abdominoperineal resection, and one a rectal polypectomy. Five patients had previous pelvic radiation therapy. Reoperation was indicated by elevated CEA levels in seven patients (70 percent), persistent pelvic pain in six (60 percent), and a suspicious radiologic study in seven (70 percent). RIGS system localized tumors verified by histopatholoy in all ten patients (100 percent); one patient with a positive CT scan and probe findings lacked histopathologic confirmation on frozen section, but had a tumor confirmed on permanent histology. Five major abdominal operations were avoided; in five patients major modifications were made in the surgical procedure based on probe findings. Six received chemotherapy or radiation therapy based on findings of the RIGS system. In six patients with negative or equivocal CT scans, the RIGS system localized histopathologically confirmed tumor. Major abdominal procedures can be avoided, the surgical approach modified, and other modes of therapy instituted earlier with the use of the RIGS system.

COLORECTAL CARCINOMA AND COLORECTAL INTUSSUSCEPTION

Eighty-one patients with colorectal carcinoma were treated at the Department of Surgery, National Tohsei Hospital, from January 1985 to December 1986. Of the 81 patients, three (3.7%) had intestinal intussusception (one with cecal carcinoma, two with sigmoid colon carcinoma).Intestinal intussusception was diagnosed by contrast radiography, ultrasonography and CT of the large intestine as well as by clinical symptoms. Colonofiberscopy proved to be most useful in confirming the coexisting tumor.Intestinal intussusception is a comparatively rare disease, and is seldom caused by colorectal carcinoma. It is found in the large intestine more frequently in adults than in children, and so often caused colorectal tumor, which is not infrequent among adults. Considering the above facts, preoperative diagnosis with regard to the presence of a tumor and its characteristics may be most important in treating intestinal intussusception in adults, and it may facilitate appropriate surgical treatment, including lymph node dissection.

A CASE OF ADENOSQUAMOUS CARCINOMA OF THE SIGMOID COLON

A 62-year-old man was admitted to the hospital with left lower abdominal discomfort which was diagnosed as sigmoid colon carcinoma by X-ray examination and fiberoptic study.Sigmoidectomy and regional lymphadenectomy were carried out. Postoperative microscopic examination revealed adenosquamous carcinoma of the colon.Primary adenosquamous or squamous carcinoma of the colon is very uncommon. The patient is still being followed, without evidence of recurrence.

To establish a method of laserthermia without control system, we performed Nd:YAG laserthermia to the tumor bearing BDF1 mice in various conditions. Finally we found that a continuous irradiation for 90-120 seconds followed by repeated pulse irradiations for about. 10 minutes with low power Nd:YAG laser light (2.0 W of the fiber tip) can cause the same effect on the tumor as the conventional laserthermia using control system. We tried this method on two cases (one common bile duct carcinoma and one sigmoid colon carcinoma) under the single channel endoscope in succeed. We conclude that laserthermia without control system can be performed effectively and safely in our method.

Bone imaging—Tracer uptake in non-neoplastic osseous and nonosseous disease processes

A 60-YR-OLD black male had resection of carcinoma of sigmoid colon. A bone scan was performed which showed no evidence of skeletal disease except increased 99mTc-phosphate uptake (Fig. 1) in the abdominal scar (Fig. 2). Due to postoperative complications, the patient died. At autopsy, gross calcification was noted in the site of abdominal scar. This case represents an example of nonosseous tracer uptake on a bone scan. The following is the gamut of uptake of 99mWc phosphate in a number of non-neoplastic osseous as well as nonosseous disorders. !. Osseous Causes A. A CASE REPORT OF HEPATIC METASTASES TREATED BY TRANSCATHETER EMBOLIZATION THERAPY

This is a case report of 50 year old male patient, post-anterior resection of carcinoma of sigmoid colon with known hepatic metastases. CEA level was 20.1 ng/ml.On infusion hepatic angiogram, multiple hepatic hypervascular areas are presented in right hepatic lobe. Successful embolization of proper hepatic artery with 2 pieces of MMC soaked Gelfoam was done. On 16 days post-embolization, CEA level decreased to 5.2 ng/ml. GOT & GPT raised 110IU/dl, 86 IU/dl immediately after the procedure, but returned to normal in 6 weeks. The patient was discharged 45 days post-embolization.

Abdominal scintigraphy for diagnosis of intestinal bleeding

AbstractAbdominal scintigraphy with99mTc‐pertechnetate, using a gamma‐camera linked to a data processor, was done in 8 patients with acute massive intestinal hemorrhage and in 34 patients with chronic recurrent intestinal hemorrhage. Endoscopy and x‐ray studies had failed in all patients to reveal a bleeding source. All but 3 patients underwent exploratory laparotomy, confirming the abnormal findings of scintigraphy. In all acutely bleeding patients, scintigraphy was positive. The bleeding was proven by laparotomy to be due to Meckel's diverticulum with gastric mucosa in 5, jejunal neurinoma in 1, and sigmoid colon carcinoma in 1 patient. Scintigraphy was positive in only 3 chronically bleeding patients, 1 of whom had a Meckel's diverticulum with gastric mucosa and 2 of whom had bleeding colon polyps. Scintigraphy was equivocal in 10 patients, 7 of whom were found to have chronic inflammation of the appendix, and it was negative in 21 patients, including 1 who had a Meckel's diverticulum without gastric mucosa. In chronic intestinal hemorrhage, abdominal scintigraphy was only occasionally helpful for locating a bleeding source. In acute intestinal bleeding, however, scintigraphy was a useful test, particularly in identifying a Meckel's diverticulum with gastric mucosa in children.
