Tissue factor activated thromboelastography correlates to clinical signs of bleeding in dogs

The ability of a laboratory assay to correlate to clinical phenotype is crucial for the accurate diagnosis and monitoring of haemostasis and is therefore challenging with currently used routine haemostasis assays. Thromboelastography (TEG) is increasingly used to evaluate haemostasis in humans and may well be of value in the workup of dogs suspected of having a haemostatic disorder. This study was undertaken to evaluate prospectively how tissue factor (TF) activated TEG correlated to clinical signs of bleeding in dogs, compared to a routine coagulation profile. A prospective case-control study was performed over a 2 year period from 2004–2006. Eligible dogs were those where the primary clinician requested a coagulation profile to evaluate haemostasis. The dogs were simultaneously evaluated with a TF-activated TEG assay. Twenty-seven dogs, characterised as hypo-coagulable based on the TEG parameter G (<3.2 K dyn/cm 2), were included in the study as cases. Size matched control groups of TEG normo- ( G = 3.2 K–7.2 K dyn/cm 2) and hyper-coagulable ( G > 7.2 K dyn/cm 2) dogs were selected retrospectively from the eligible dogs. For all dogs, clinical signs of bleeding were noted at time of analysis. There were statistically significant differences between all TEG values of hypo- and normo- and hyper-coagulable dogs. Thromboelastography correctly identified dogs with clinical signs of bleeding with a positive predictive value (PPV) of 89% and a negative predictive value (NPV) of 98% based on G alone. In comparison, the coagulation profile had a PPV between 50–81% and a NPV between 92–93% for detection of bleeding, depending on the observer. In conclusion, a TF-activated TEG G value < 3.2 K dyn/cm 2 correctly identified dogs with clinical signs of bleeding with very high PPV and NPV, irrespective of observer. The findings strongly suggest that TF- activated TEG may be of value in the workup of dogs suspected of having a haemostatic disorder.

Sharp Versus Blunt Needle: A Comparative Study of Penetration of Internal Structures and Bleeding in Dogs

Complications associated with interventional pain procedures have raised questions regarding the relative safety of sharp vs. blunt needles. It has been speculated that the incidence of hemorrhage, intraneural and/or intravascular injections may be reduced by the use of blunt needles. In this study we compared penetration and bleeding associated with sharp vs. blunt needle punctures. Attempts were made to insert blunt and sharp needles (18-, 20-, 22-, and 25-gauge) directly or percutaneously into kidney, liver, renal artery, intestine or spinal nerve/nerve root of anesthetized dogs. Penetration and bleeding were ascertained by direct vision through a surgical wound. All attempts to directly puncture the kidney and liver with sharp needles were successful. All but one attempt to puncture a spinal nerve/nerve root with 20-, 22-, and 25-gauge sharp needles were successful but half or less attempts to puncture the intestines were successful. All attempts to puncture the renal artery with sharp needles were successful. Blunt needles never punctured the renal artery, spinal nerve/nerve root and intestines and rarely penetrated the kidney (22- and 25-gauge one time each). All attempts to puncture the liver with blunt needles were successful. Bleeding scores for kidney punctures were generally higher for larger sharp needles than for smaller ones. Bleeding scores for blunt needle punctures of the liver were generally smaller than for sharp needle puncture. Blunt needles are less likely than sharp ones to enter vital structures and/or produce hemorrhage. Thus, blunt needles may be preferable to sharp ones for performing interventional pain procedures.

Clinical evaluation of cimetidine, sucralfate, and misoprostol for prevention of gastrointestinal tract bleeding in dogs undergoing spinal surgery

SUMMARY Objective To determine the incidence of gastrointestinal (GI) tract bleeding in dogs undergoing spinal surgery with adjunct corticosteroid treatment, and to determine the protective efficacy of cimetidine, sucralfate, and misoprostol against such bleeding in these dogs. Animals 40 dogs that underwent spinal surgery. Procedures Myelography and surgery were performed on the first or second day of hospitalization. Methylprednisolone sodium succinate was given at a dosage of 30 mg/kg of body weight prior to myelography, followed by a second full or half dose 2 to 4 hours later at clinician discretion. Spinal surgery was performed in conventional manner, postoperative administration of analgesics was done, and dogs were fed a diet lacking red meat. Dogs were assigned at random to 1 of the 3 treatment groups or to the control group. Dogs of the treatment groups received cimetidine, sucralfate, or misoprostol. Physical examination and determination of PCV and serum total protein values were performed daily. A fecal sample was examined daily for gross and occult blood. Results 36 of 40 dogs had GI tract bleeding during a hospitalization period of 3 to 6 days. There was no significant difference in development of bleeding between the control group and any of the treatment groups. Conclusions Gastrointestinal tract bleeding occurred in 90% of dogs undergoing spinal surgery combined with administration of methylprednisilone sodium succinate, a higher rate than that found in previous studies. This bleeding was not life-threatening. Prophylactic benefit from any of the GI protectants tested was not found. (Am J Vet Res 1997;58:1320–1323)

Hemostatic defects associated with two infusion rates of dextran 70 in dogs

Summary We investigated changes in hemostatic function after infusion of 6% dextran 70 (high molecular weight dextran) at 2 rates. Six healthy dogs underwent 3 regimens: 20 ml of dextran/kg of body weight administered in 1 hour (trial A), 20 ml of dextran/kg administered in 30 minutes (trial B) and 0.9% sodium chloride solution as a control administered over 1 hour to achieve hemodilution equivalent to that for 20 ml of dextran/kg (trial C). Before and at 2, 4, 8, and 24 hours after the start of trials A and B, we measured pcv, total solids (ts) concentration, amount of von Willebrand factor antigen (vWf:Ag), factor VIII coagulant activity (VIII:C), prothrombin time, activated partial thromboplastin time (aptt), platelet retention in a glass bead column, and buccal mucosa bleeding time (bmbt). Values were not obtained at 8 and 24 hours for trial C. Saline-induced changes in hemostasis were significant (P &lt; 0.05) from baseline throughout the sample collection period. Significant differences (P &lt; 0.05) between trial A and control were observed for vWf:Ag, VIII:C, bmbt, aptt, ts, and pcv values at 2 hours, and for VIII:C at 4 hours. Significant differences (P &lt; 0.05) between trial B and control were observed for aptt, ts, and pcv values at 2 hours, and for vWf:Ag, VIII:C, bmbt, aptt, ts, and pcv values at 4 hours. During trials A and B, mean values of analytes infrequently deviated from reference intervals, and clinical signs of bleeding were not observed in any dog. Data for the dextran infusions paralleled each other and had a tendency to normalize, infrequently reaching baseline by 24 hours. Differences in overall hemostatic function were not detected between dextran infusions. Dextran 70 at a dosage of 20 ml/kg induces minimal hemostatic abnormalities when infused over 30 or 60 minutes to clinically normal dogs, but may precipitate bleeding in dogs with marginal hemostatic function.

Spatial and temporal differing control of sympathetic activities during hemorrhage.

We performed simultaneous multifiber recordings of cardiac, renal (RNA), hepatic (HNA), adrenal, and splenic sympathetic nerve activities during 10 min of sustained hemorrhagic hypotension to a mean blood pressure of 50 mmHg. One minute after bleeding in dogs with intact baroreceptors, all measured nerve activities increased significantly, and a gradual decline toward prebleeding level followed. Only RNA showed a great inhibition below prebleeding level. In cervically vagotomized dogs, all nerve activities showed significantly higher levels than prebleeding control throughout the experiment. The initial increase in HNA observed at 1 min postbleeding in animals with intact baroreceptors was attenuated. HNA in the vagotomized group showed a gradual increase so that 5 min postbleeding, HNA reached a peak level that was maintained until the end of the experiment. Sympathoinhibition in RNA and decrease in heart rate during hemorrhagic hypotension were reversed to significant increases after vagotomy. In contrast, animals with denervation of the carotid sinus and aortic nerves showed initial increases in nerve activities followed by recovery to prebleeding control level within 2 min postbleeding. After complete denervation of systemic baroreceptors, rapid hemorrhage did not cause any significant change in sympathetic nerve activity in any nerve. These results indicate that early reflex response to hemorrhage in regional sympathetic nerves is unidirectional sympathoexcitation mediated by summative unloading of both arterial baroreceptors and cardiopulmonary receptors. When greater than 1 min passed after bleeding and sustained hypotension of 50 mmHg was established, a reflex mechanism through the vagal afferents participated to inhibit the unidirectional sympathoexcitation mediated by unloading of arterial baroreceptors, with different magnitude in different innervated organs.(ABSTRACT TRUNCATED AT 250 WORDS)

Experimental studies of injection therapy for severe nonvariceal bleeding in dogs

Efficacy and tissue effects of injection therapy for nonvariceal upper gastrointestinal bleeding were studied in 16 mongrel dogs. The results were compared with those obtained by neodymium-yttrium-aluminum-garnet laser, bipolar electrocoagulation, and heater probe. Epinephrine (1:10,000), absolute ethanol, and 1% polidocanol were used as injection solutions. In acute, severely bleeding experimental ulcers as well as in transected submucosal arteries, injection methods were not as effective as thermal methods in achieving complete hemostasis, although injection therapy, especially with large volumes of epinephrine (1:10,000), very quickly decreased the rate of bleeding. Chronic experiments showed that epinephrine (1:10,000) caused almost no tissue injury, but also did not induce vessel thrombosis. In contrast, absolute ethanol and 1% polidocanol caused tissue necrosis, ulceration, and vessel thrombosis, the former by acute dehydration and fixation of the tissue, the latter by acute edema and subsequent inflammation and sclerosis. These data suggest that although epinephrine injection may slow or temporarily stop bleeding, this modality is not as efficacious as injection with 1% polidocanol or absolute ethanol in inducing definitive vessel thrombosis. In the present experimental conditions sclerotherapy was not as effective as thermal methods in achieving hemostasis. These data also show that injection therapy with 1% polidocanol and absolute ethanol is by no means safer than thermal methods.

Effects of Phenylbutazone and Indomethacin on the Post-operative Course following Experimental Orthopaedic Surgery in Dogs

Randomized placebo-controlled crossover studies were carried out in dogs to evaluate how two non-steroidal anti-inflammatory drugs (NSAID) might modulate an acute post-traumatic inflammatory reaction. Two "identical" surgical interventions were performed on the forelimbs of each animal with an interval of 28 days, to enable a paired comparison of the inflammatory signs and the wound/bone healing processes. At one operation 8 dogs received 300 mg phenylbutazone twice daily for 8 days starting on the day before surgery, and at the other operation matching placebo tablets were given. In a similar placebo-controlled trial another group of 8 dogs received 5 mg indomethacin twice daily. With phenylbutazone the post-operative swelling was not significantly reduced compared to placebo, but there was less pain and limping. With indomethacin the swelling was somewhat reduced, but there was no consistent difference to placebo in the pain and limping assessments. None of the drugs appeared to distinctly effect the wound or fracture healing, as evaluated by clinical inspection, comparison of radiographs and comparison of bone sections from the sites of surgery. It proved difficult to select an appropriate dosage of indomethacin due to its high potential to induce GI ulceration and bleeding in dogs. In this experimental surgical model with an acute inflammation, neither phenylbutazone nor indomethacin showed impressive anti-inflammatory or analgesic properties. In the same model paracetamol has proved to significantly and more efficiently, reduce both swelling and pain without any noticeable adverse effects, and appears to be a better alternative than the two presently tested NSAID.

Effects of aspirin-like drugs on canine gastric mucosal blood flow and acid secretion.

1. The effects of aspirin, paracetamol and benorylate were studied on gastric mucosal blood flow (MBF) and acid secretion in canine denervated gastric pouches. 2. Aspirin 20 mM in the unstimulated pouch had no effect; pentagastrin-stimulated acid output, but not MBF, was reduced. Aspirin buffered to pH 6 was ineffective. 3. Aspirin 3-50 mg/kg reaching the pentagastrin-stimulated pouch through the blood, increased acid secretion and MBF, but the MBF:secretion ratio was variably affected. 4. Paracetamol (10 or 20 mg/kg i.v., or 20 mM in the pouch) or benorylate (280 mg/kg orally) mainly had little effect. 5. Circular muscle strips from dog arteries were contracted by prostaglandins E2, F1alpha or F2alpha, and often slightly by indomethacine, but prostaglandin E1 produced variable effects. 6. These results do not favour the view that aspirin causes gastric bleeding in dogs by breakdown of blood vessels due to ischaemia following mucosal vasoconstriction.

MARROW HYPERPLASIA AND HEMOGLOBIN RESERVE IN EXPERIMENTAL ANEMIA DUE TO BLEEDING

In long continued severe anemia due to bleeding in dogs we may observe all degrees of red marrow spread. The maximal red marrow spread takes in the total marrow area in ribs, vertebrae and long bones. The minimal red marrow spread may involve but 10 to 20 per cent of this maximal area. All gradations between these extremes are observed in this series. The extent of the red marrow spread is not dependent upon the length of the anemia period nor is it related to the capacity of the dog to produce new hemoglobin and red cells on standard diets. Evidence points to the liver as concerned with this hemoglobin production and liver function may set the limits for hemoglobin production on these standard diets. This in turn may determine the needful red marrow spread. During favorable diet periods there is a storage of hemoglobin or hemoglobin precursors which come out later in the control periods as finished red cells. This reserve of hemoglobin producing material is not stored as mature red cells in the marrow or elsewhere but as intermediates stored probably in the liver and red marrow but also perhaps in the kidney and spleen. Red marrow very rarely gives evidence of exhaustion even after many years of continuous severe anemia (Dog 18-114). This would seem to consist of a shrinkage of the red marrow area rather than a degenerative change of the red marrow cells. The spleen shows some evidence of erythrogenesis in these dogs. Megakaryocytes in some instances are conspicuous (Fig. 6) and nests of nucleated red cells are found in the spleen pulp but it is unlikely that the spleen contributes any large amount of red cells to the anemic circulation in these experiments. There is no evidence that erythrogenesis occurs in the liver or lymphatic tissue in these dogs.

AA Ozherelyev and IB Kolodner experimentally studied the hemostatic effect of various heterogeneous parenchymal organs, such as lungs, muscles, liver, spleen, and kidneys in parenchymal bleeding in dogs, both under conditions of normal and experimental reduced blood coagulation.

BLOOD REGENERATION IN SEVERE ANEMIA

A liver extract No. 343, N.N.R. known to be fully potent in pernicious anemia shows but 10 to 20 per cent of the potency of whole liver feeding in severe continuous secondary anemia due to bleeding in dogs. There are wide individual variations which are not noted with whole liver feeding. Supplementing this liver extract with whole bile does not modify the reaction. Supplementing this liver extract with liver ash or apricot ash shows the sum of the two expected feeding reactions. When we supplement this liver extract with small amounts of whole liver (50 to 100 gm.) we may observe hemoglobin and red cell production in excess of the sum of the expected separate reactions. Whether the liver extract increases the potency of the whole liver feeding or vice versa, it suggests similar possibilities in various human secondary anemias. Liver and kidney fractions of varied types deserve much study to ascertain their effect upon widely divergent types of human anemias.
