Simultaneous Bilateral Quadriceps Tendon Rupture in a Patient Undergoing Long-Term Hemodialysis: A Case Report

Bilateral quadriceps tendon ruptures are rare lesions. At.raumatic forms, or those following low-energy trauma, are much rarer and occur on pre-existing lesions. We report a case of bilateral and simultaneous rupture of the quadricipital tendon in a 45-year-old hemodialysis patient with secondary hyperparathyroidism (PTH: 1642 pg /ml). He underwent surgical repair with good evolution after rehabilitation. Spontaneous and simultaneous bilateral ruptures of the quadricipital tendon are exceptional. A pathology that weakens the tendons, such as secondary hyperparathyroidism, which is common in dialysis patients, remains the most common cause.

Introduction: Bilateral quadriceps tendon rupture is an infrequent injury caused by an eccentric contraction of the quadriceps when the knee is in semiflexed position under load. Different chronic pathologies and some drugs associated with this injury have been described. Case report: We present the clinical case of a 63-year-old man with acute pain, functional impotence for active extension of both knees and the presence of a bilateral suprapatellar gap, which was confirmed by ultrasound. Results: The patient underwent surgical treatment by transosseous suture. Conclusion: Early surgical treatment of bilateral quadriceps tendon rupture allows an optimal recovery with good functional results.

Simultaneous bilateral quadriceps tendon rupture is an uncommon but disabling clinical entity that usually has an underlying cause of tendon weakening. We present a case of a 36-year-old male who presented with full-thickness tears of both quadriceps tendons following a fall on the knees and was found to have underlying calcific tendinopathy secondary to chronic kidney disease and hemodialysis. Imaging with radiographs and ultrasound scans usually suffices for the evaluation of tendon pathology in cases of clinical suspicion of quadriceps tendon rupture.

Simultaneous bilateral quadriceps tendon rupture in a healthy young male: a case report

BackgroundSimultaneous bilateral quadriceps tendon rupture is rare, particularly in young individuals with no prior medical history. We present the case of a young man who presented with bilateral quadriceps tendon rupture.Case presentationA 27-year-old Japanese man missed a step while descending a flight of stairs, stumbled, and became aware of severe pain in both knees. He had no past medical history, but was severely obese, with a body mass index of 43.7 kg/m2 (height 177 cm, weight 137 kg). Five days after injury, he was referred to our hospital for examination and treatment. Bilateral quadriceps tendon rupture was diagnosed based on magnetic resonance imaging, and quadriceps tendon repair with suture anchor was performed on both knees 14 days after injury. The postoperative rehabilitation protocol was to immobilize both knees in extension for 2 weeks, then to gradually proceed with weight-bearing and gait training using hinged knee braces. Both knees obtained a range of motion from 0° to 130° without any extension lag by 3 months postoperatively. One year postoperatively, tenderness was evident at the suture anchor in the right knee. That suture anchor was therefore removed in a second operation, and histological evaluation of the tendon of the right knee revealed no pathological changes. As of 19 months after the primary surgery, the patient showed a range of motion from 0° to 140° in both knees, did not complain of any disability, and had fully returned to normal daily activities.ConclusionsWe experienced simultaneous bilateral quadriceps tendon rupture in a 27-year-old man with no past medical history other than obesity. Suture anchor repair was performed for both quadriceps tendon ruptures and achieved a favorable postoperative outcome.

Despite initial misdiagnosis, a thorough evaluation, including physical examination and imaging, revealed bilateral quadriceps tendon rupture. Surgical repair was performed, followed by a comprehensive rehabilitation program. At the four-month follow-up, the patient showed significant improvement in pain and function. This article provides a comprehensive review of the existing literature on bilateral quadriceps tendon rupture, emphasizing the challenges in the diagnosis and management of this rare condition. Early diagnosis, prompt surgical intervention, and a tailored rehabilitation program are crucial for successful outcomes.

Bilateral Chronic Quadriceps Tendon Ruptures Treated with Hamstring Autograft Reconstruction: A Case Report.

A 30-year-old semiprofessional football player presented with chronic bilateral quadriceps tendon ruptures. Both quadriceps tendon ruptures were unsuitable for isolated primary repair because of tendon retraction and immobility. A novel reconstruction technique using semitendinosus and gracilis tendon autografts was performed to restore both lower extremities' disrupted extensor mechanisms. At the final follow-up, the patient regained excellent motion of both knees and returned to high-intensity activity. Chronic quadriceps tendon ruptures present challenges related to tendon quality and mobilization. Reconstruction with hamstring autograft using a Pulvertaft weave through the retracted quadriceps tendon in a high-demand athletic patient represents a novel approach for treating this injury.

A case of delayed diagnosed spontaneous bilateral quadriceps tendon rupture in a female patient with gout Spontaneous bilateral quadriceps tendon ruptures are an extremely rare finding which are generally associated with comorbidities such as chronic renal failure, diabetes mellitus, obesity and systemic diseases (systemic lupus, gout, pseudogout and hyperparathyroidism). Other related risk factors include the use of anabolic steroids and fluoroquinolones. Due to the rare occurrence, the diagnosis, which is mainly clinical, is often delayed. An MRI remains the gold standard. However, an ultrasonography often leads to a quicker diagnosis and treatment. A prompt diagnosis is crucial as the functional outcomes diminish with delayed surgical repair and increasing age. An intensive, tailored postoperative rehabilitation program is imperative in regaining a proper knee function and optimizing the functional outcome. In this article, a rare case of a delayed diagnosed spontaneous bilateral quadriceps tendon rupture in an elderly patient with gout, initially misdiagnosed as spinal stenosis with bilateral L2-S1 radiculopathy, is reported.

Spontaneous bilateral quadriceps tendon rupture treated with polyethylene terephthalate tape augmentation: Report of two consecutive cases.

Spontaneous rupture of quadriceps tendon (QT) is a rare condition and it is commonly associated with systemic comorbidities and medical products assumption, which may lead to tendon degeneration. While unilateral rupture is a quite common injury, spontaneous bilateral ruptures are very rare. Herein, we report two consecutive cases of spontaneous bilateral QT rupture in two patients with a positive history of chronic statin use, successfully treated with bilateral single-stage reconstruction with polyethylene-terephthalate tape augmentation. At 12 months of follow-up, both patients recovered the full extension, a 120° pain-free flexion and knee scores improvement. Magnetic resonance imaging at one year showed a complete, bilateral, bio-integration of the augmentation for each knee. In conclusion, polyethylene terephthalate tape augmentation provides a good structural support with a good grade of bio-integration, allowing a fast recovery.

The patient underwent successful operative repair the following day.DiscussionBilateral quadriceps tendon rupture is rare and can be difficult to diagnose due to the impossibility of comparing the affected to the unaffected limb. Plain radiographs are usually not helpful, but ultrasound, CT, and magnetic resonance imaging may be used to confirm the clinical diagnosis.

Concomitant Use of Single Suture Anchor and Transosseus Repair Provide Optimal Results in Simultaneous Bilateral Quadriceps Tendon Rupture: A Case Report

Introduction
 Bilateral quadriceps tendon rupture is a rare condition and often associated with underlying medical comorbidities such as chronic renal insufficiency. This case report is aimed to evaluate the surgical outcome using the combination of suture anchor and transosseus repair technique.
 
 Presentation of Case
 Presenting a 54-year-old male who sustained simultaneous bilateral quadriceps tendon rupture with history of right nephrectomy and chronic kidney disease requiring intermittent hemodialysis. He came with sudden bilateral knee pain, history of falling, and inability to walk since three weeks before admission to the hospital. Tenderness above the superior pole on both patella, accompanied with slight swelling and no active range of motion (ROM) of either knee joint was found on physical examination. Complete rupture of the quadriceps tendon of both knees at their osseus-tendon junction were revealed on magnetic resonance imaging (MRI).
 
 Discussion
 Recent literatures suggested that transosseus technique offers the safest and more stable tendon reattachment, while suture anchor provides biomechanical superiority. We tried to combine these advantages by incorporating both methods. Progressive active knee motion exercise was performed shortly after using the Kocher slab for 2 weeks.Subsequently, the patient can achieve painless, active extension and active 90°-120° knee flexion 1 month post-operatively.
 
 Conclusion
 The application of a single suture anchor with transosseus repair in quadriceps tendon rupture led to an optimal outcome, improved stability, and earlier rehabilitation.

Introduction: Simultaneous bilateral quadriceps tendon rupture is a very rare injury that may be associated with systemic illnesses or drugs. We present a case of bilateral quadriceps tendon rupture in a patient with no identifiable risk factors. 
 Case: Sixty seven year old male patient presented to emergency room with bilateral knee pain after trip and fall from 2 steps on his knees. His physical exam revealed bilateral suprapatellar swelling and tenderness with palpation on both knees and he could not fully actively extend knees. Magnetic resonance imaging revealed bilateral quadriceps tendon rupture. Both knees were immobilized with long leg cast in full extension. Early operative treatment was offered. 
 Conclusion: Diagnosis may be delayed in bilateral quadriceps tendon rupture. Since it is a rare injury emergency medicine physicians should be familiar with symptoms and signs of it to diagnose it early and prevent disability.

Isokinetic Evaluation of Knee Strength and Endurance in Hemodialysis Patients Treated with the Transpatellar Tunnel Technique for Bilateral Spontaneous Quadriceps Tendon Ruptures.

There are numerous surgical techniques described for treating acute quadriceps tendon rupture. We aim to evaluate the strength of the quadriceps tendon using isokinetic tests in hemodialysis patients treated with the transpatellar tunnel technique for bilateral spontaneous quadriceps tendon ruptures. Additionally, functional and clinical results were investigated and compared with hemodialysis patients without a history of quadriceps tendon rupture. We retrospectively reviewed hemodialysis patients diagnosed with quadriceps tendon rupture who were operated between 2005 and 2015 at our institution. Eight patients with simultaneous bilateral quadriceps tendon rupture treated with the transpatellar tunnel technique (group 1; the operated group) and 10 patients with chronic renal failure without a history of quadriceps tendon rupture who underwent hemodialysis (group 2; the control group) were included in this study. The active range of motion (ROM), Lysholm and Tegner scores, knee flexor and extensor muscle strengths, and endurances were measured using an isokinetic dynamometer for each group and compared between the groups. The mean age was 44.6 and 40.2 years in groups 1 and 2, respectively. The mean follow-up was 58.6 (range, 24-140) months in group 1. The active ROM was 113.7 ± 9.7 degrees in group 1 and 130 ± 4.8 degrees in group 2 (p < 0.001). The mean Tegner score was not significantly different between the groups (1.87 ± 1.1 in group 1 vs. 2.6 ± 0.9 in group 2; p = 0.212). However, the Lysholm score was significantly higher in group 2 (93.2 ± 7.3 vs. 76.7 ± 13.8; p = 0.009). The mean extensor strength and endurance were significantly higher in group 2 than in group 1. The mean flexor strength and endurance were higher in group 1 than in group 2, but no significant difference was found. Although using the transpatellar tunnel technique for bilateral spontaneous quadriceps tendon ruptures in hemodialysis patients can provide acceptable functional results, the ROM and strength particularly in extensor muscle might be significantly lower than control group. Level of evidence is Level III therapeutic case-control study.

Bilateral Quadriceps Tendon Repair with Suture Anchors: Case Series.

Bilateral and simultaneous quadriceps tendon rupture is rarely observed. This case series evaluates 3 patients with bilateral ruptures of the quadriceps tendon. The purpose of this case series is to display the effectiveness of the suture anchor surgical repair technique in these patients. Three obese male patients experienced some type of traumatic injury to the knee extensor mechanism, resulting in bilateral quadriceps tendon ruptures. Each of the patients was evaluated and given operative and nonoperative treatment options, and they all elected to undergo surgical tendon repair. Two double-loaded anchors were placed into the superior pole of the patella, allowing the tendons to be reduced back to their original positioning. Postoperatively, all patients were given the same guidelines for physical therapy, and their progress was monitored periodically. Each patient returned to his normal level of physical activity. Range of motion at the knee and weight bearing were reestablished in all 3 patients. These quadriceps tendon ruptures occurred in patients with a predisposition to injury as a result of excessive weight bearing. When addressing bilateral quadriceps tendon ruptures in this patient population, we found that a suture anchor-based construct allows for a secure repair, early initiation of physical therapy, and a noted improvement in pain scores on the Visual Analog Scale.

BackgroundThe incidence of rupture of the quadriceps or patellar tendon s is low, especially that of bilateral quadriceps tendon rupture, and it is generally considered a complication secondary to chronic systemic disorders. We report two rare cases of simultaneous bilateral tendon rupture affecting the extensor function of the knee in patients with chronic kidney disease who have been treated with long-term haemodialysis.Case presentationTwo young males with a history of chronic kidney disease who were being treated with long-term haemodialysis presented to our hospital with clinical signs of disruption of the extensor mechanism of the knee. One patient was diagnosed with bilateral quadriceps tendon rupture, and the other patient had bilateral patellar tendon rupture. They underwent surgical repair of the tendons, and their knees were actively mobilized during physiotherapy.ConclusionBilateral quadriceps or patellar tendons rupture is a rare occurrence in patients with chronic kidney disease who are being treated with long-term haemodialysis. Timely surgical treatment and scientific physiotherapy can lead to good recovery of knee joint function.
