Bilateral coronoid hyperplasia is a rare condition in paediatric patients easily confused with other temporomandibular dysfunctions on presentation.

Contribution of anatomical criteria to surgical approach for treatment of bilateral coronoid hyperplasia

IntroductionBilateral coronoid hyperplasia is a rare condition characterized by a progressive and painless limitation of mouth opening. The treatment consists of coronoidectomy by intraoral or coronal approach. There is no recommendation in the literature on the choice of the surgical approach according to the importance of the hypertrophy. The objective of our study is to search for predictive anatomical criteria of each approach. Material and methodsThese anatomical criteria were evaluated from 3D CT facial reconstructions of 4 male patients aged 4 to 30 years retrospectively after their surgery. A single intraoral approach was used for cases 1 and 2 (group A), a double approach for cases 3 and 4 (group B). Same measurements were performed on 10 male case controls (group C). We performed a descriptive analysis of our results due to an insufficient number of patients. ResultsThe average width of the coronal processes was much greater in group B, when a double approach was necessary, than in groups A and C. In groups A and B, the average height of the coronoid notch is low compared to group C when there is a need for a coronal approach. The width/TZS ratio is increased in group B compared to group C. DiscussionIn our study, 3 criteria could be informative for the choice of the surgical approach: the width at the superior end of the coronoid process, the width/TZS ratio and the height of the coronoid notch but additional data are needed to confirm our therapeutic options.

Difficulty of Arch Reduction with Gillies Temporal Approach in a Coronoid Hyperplasia Case.

Bilateral coronoid hyperplasia is an abnormal elongation of the mandibular coronoid process which is rare in occurrence and causes progressive but slow reduction in mouth opening. In case of reduction in zygomatic arch fracture by Gillies temporal approach, the presence of bilateral coronoid hyperplasia can be a possible factor for hindrance and improper reduction. We propose a technical note to overcome this hindrance caused by the coronoid hyperplasia and propose tips and tricks to successful reduction in zygomatic arch. Adequate reduction in the zygomatic arch and pretrauma mouth opening was achieved. Manual repositioning of the mandible during Gillies temporal approach along with ultrasound guidance leads to a satisfactory outcome.

Development of temporomandibular ankylosis after diagnosis of Jacob’s disease: clinical and tomographic assessment

Jacob's disease is a rare condition that consists of the formation of a pseudo joint structure between the mandibular coronoid process and the zygomatic bone, resulting in limited mouth opening. The disease is difficult to diagnose and etiology is uncertain. This article describes a clinical case of a 6-year-old child with limited mouth opening, which resulted in the formation of a temporomandibular ankylosis leading to complete immobility due to delayed diagnosis and treatment of the disease. A review of this pathology and the most precise imaging exams are discussed for the early and differential diagnosis of the disease.

Bilateral coronoid hyperplasia causing limited mouth opening: A report of two cases

Bilateral hyperplasia of the coronoid process of the mandible is an unusual entity which often presents with limited mouth opening. Thorough clinical and radiographic examination can help in the diagnosis and the treatment planning of this condition. This article consists of a case report of two patients. The patient in each case, presented with a reduced mouth opening, and when subjected to further investigations, was diagnosed with bilateral coronoid hyperplasia.

Outcomes of coronoidectomy as treatment of coronoid pseudoankylosis and hyperplasia: case series

Background: The coronoid process may play an important role as an underlying cause of trismus due to its anatomical position in relation to the zygomatic arch, however patients rarely present with coronoid related underlying causes. We are presenting a case series of seven patients with mandibular hypomobility due to coronoid hyperplasia and traumatic coronoid pseudoankylosis. Objectives: To evaluate the effectiveness of coronoidectomy as a treatment for severe mandibular hypomobility in cases of developmental coronoid hyperplasia and traumatic coronoid pseudoankylosis. Methods: Seven patients clinically diagnosed with severe mandibular hypomobility (10–20 mm) were treated in our hospital from 2008 till 2015. Four patients had pseudoankylosis of the coronoid process with the untreated-fractured zygoma. Three patients had bilateral coronoid hyperplasia, with one case forming a bilateral joint articulation with the zygoma (Jacob's disease). All patients underwent coronoidectomy. Surgery was followed by vigorous jaw physiotherapy. Patients were evaluated using interincisal distance at maximum mouth opening, preoperatively and postoperatively for a period ranged between 6 months and 2 years. Findings: Mandibular mobility improved significantly in six patients postoperatively and was maintained throughout the follow up period with satisfactory range of motion (30–42 mm). Postoperative recovery was uneventful, however two patients had significant iatrogenic postoperative deranged occlusion, and one had re-pseudoankylosis six months postoperatively despite initial improvement. Conclusion: Coronoidectomy is an effective measure for treatment of coronoid hyperplasia and coronoid pseudoankylosis, with minimum complication.

The presence of situs inversus totalis (full reversal of internal organs) in twins is briefly reviewed. Information gathered from 35-year-old monozygotic (MZ) female twin pair discordant for this condition is presented. This is followed by summaries of research on the frequency of trisomy 21 (Down syndrome) in twins, the first case of MZ twin concordance for bilateral coronoid hyperplasia, prenatal hormonal effects in mixed-sex non-human primate litters, and links between insurance mandates and twinning following in vitro fertilization. The final section of this article describes twin-related events reported in the news, namely, the first recorded birth of identical twin puppies; the 100th birthday celebration of a pair of fraternal female twins, the passing of an award-winning identical twin production designer, and the first running of the New York City Marathon by a set of quadruplets.

Mandibular hypomobility secondary to bilateral coronoid hyperplasia

Coronoid process hyperplasia is one of a large number of disorders affecting the stomatognathic system and causing mandibular hypomobility. It is characterized by an excessive coronoid process growing to such an extent that it impinges on the posterior portion of the zygomatic bones. Despite the low prevalence of this condition, it should be considered as a possible diagnosis in patients with painless, progressive, and chronic restriction of mouth opening. The diagnosis is based on clinical signs in association with radiographic examinations. Coronoidectomy is the only treatment option for this condition along with early postoperative physical therapy. This article presents the case of a patient with bilateral coronoid process hyperplasia, who was initially diagnosed as having bilateral temporomandibular joint disorders. The patient underwent months of physiotherapy and bite-appliance therapy without any improvement in mouth opening. FinalIy, he was referred to La Timone Hospital for a proper diagnosis and, consequently, effective treatment.

MANAGEMENT OF ATYPICAL MANDIBULAR CORONOID PROCESS ELONGATION WITH BILATERAL INTRAORAL CORONOIDOTOMY

Bilateral coronoid hyperplasia is characterized by a progressive limitation in mandibular movement secondary to mechanical impingement of the elongated coronoid processes on the posterior surface of the zygomatic bones. The etiology of coronoid hyperplasia remains uncertain, with various proposed theories. As it is an unusual and infrequent clinical entity, hyperplasia of the coronoid process is often overlooked or diagnosed too late. A 27-year old, ASA I, male patient referred to our clinic because of his restricted mouth opening. Bilateral coronoid hyperplasia was detected after detailed temporomandibular joint examination. Restricted mouth opening was successfully treated by bilateral intraoral coronoidotomy with additional physiotherapy. The aim of this case report were emphasizing the mandibular coronoid process elongations as an unignorable differential diagnosis of the asymtomatic mandibular hypomobility cases additionally to the temporomandibular joint and masticator muscles dysfunctions and were describing its successful treatment by intraoral surgery and physiotherapy.

Coronoid-condylar index in assessing of mandibular coronoid hyperplasia. Preliminary results

The purpose of this study was to define the coronoid-condylar index (CCI). The material for studies comprised 26 patients. Thirteen patients were treated for bilateral coronoid hyperplasia. The remaining 13 patients were included in the control group. Based on 3D CT measurements, the coronoidcondylar index (CCI) was calculated and assessed statistically. In patients with coronoid hyperplasia mean CCI was 1.25, while in the control group it was 1.00. The proposed CCI can be used for the evaluation of anomalies of the mandibular coronoid processes.

A case is reported of bilateral coronoid hyperplasia. The literature is reviewed concerning this condition's etiology, pathogenesis, clinical characteristics, diagnosis, and treatment. Jacob disease and coronoid elongation are both clinical features of coronoid hyperplasia. It is usually accompanied by restricted opening. The etiology and pathogenesis of coronoid hyperplasia are unclear. The condition can be diagnosed by panoramic radiographs and with 3-dimensional reconstructions from computerized tomography image data sets. Hyperplasia of the coronoid processes can be treated using an intraoral approach for coronoidectomy and dynamic laser physiotherapy after surgery. Although hyperplasia of the coronoid processes is uncommon in clinic, it can be found through careful examination and proper radiographic study. A 39-year-old female patient was referred for coronoid hyperplasia (Jacob disease on right and elongation on left). The histologic diagnosis for the right condylar condition was osteochondroma.

Mandibular Coronoid Hyperplasia in Pediatric Patients

Bilateral coronoid hyperplasia is a relatively rare condition in the pediatric population and yet may be an unrecognized cause of limited mouth opening in children. There are multiple theories as to the causes of the hyperplasia, which include temporalis hyperactivity, hormonal stimulus, and genetic inheritance. The resulting excess growth of the coronoids results in impingement on the zygomatic processes leading to mandibular hypomobility. The diagnosis is confirmed with plain films and computed tomography scans. Treatment involves bilateral coronoidectomies to relieve impingement on the zygoma. Postoperative physical therapy is crucial for success; the therapy focuses on maintaining the mouth opening achieved at the time of surgery. Outcome reports have been variable despite good physical therapy, suggesting that the exact pathology of the condition is not well understood.

Use of a Therabite appliance in the management of bilateral mandibular coronoid hyperplasia

Persistent restriction of mouth opening after coronoidectomy to treat bilateral coronoid hyperplasia, may be the result of soft tissue fibrosis. We present the use of a mouth-opening appliance that helps to overcome this problem and improves long-term results.

Hyperplasia of the Coronoid Process

Objective: To review the morphology, aetiology, clinical symptoms, diagnosis, and therapy of true coronoid hyperplasia.Patients and Methods: The record of all patients with a confirmed diagnosis of mandibular coronoid hyperplasia presenting to the Department of Oral and Maxillofacial Surgery, University of Bonn, Bonn, Germany, between 1995 and 2004 were reviewed and details of age, sex, clinical symptoms, radiography, and treatment were noted. The results of this series were compared with those previously reported in the literature by meta-analysis.Results: Fourteen new patients from the Department of Oral and Maxillofacial Surgery and 101 patients with coronoid hyperplasia reported in the literature were included. There were 96 patients with bilateral coronoid hyperplasia and 19 patients with unilateral coronoid hyperplasia. The average age at diagnosis was 23.7 years. The main presenting symptom was progressive limitation of mouth opening. All 14 patients from the Department of Oral and Maxillofacial Surgery underwent coronoidectomy and postoperative physiotherapy, with an improvement in mouth opening to more than 30 mm for 86% of patients. In the previously published series, 95% of patients underwent coronoidectomy and 5% had coronoidotomy; 65% of patients were reported to have undergone postoperative physiotherapy with 73% improved mouth opening.Conclusion: Compared with the relatively disappointing results for mouth opening in previous studies, the treatment approach at the the Department of Oral and Maxillofacial Surgery underlines the superiority of coronoidectomy over coronoidotomy and the importance of dynamic physiotherapy with a jaw rehabilitation system to reduce post-surgical fibrosis and to maintain stable long-term results.

Bilateral Coronoid Hyperplasia in an Adolescent: Report of a Case and Review of the Literature

Bilateral coronoid process hyperplasia is a rare condition characterized by an enlarged mandibular coronoid process. Painless progressive reduction of the mouth opening is caused by the coronoid process impingement on the posterior aspect of the zygomatic bone. Isberg et al1 examined 163 consecutive patients with limited mouth openings and found that 5% of the cases were due to elongation of the coronoid process. The present case of bilateral coronoid process hyperplasia in a 13-year-old Japanese boy was treated by intraoral coronoidectomy and physiotherapy.

The value of three-dimensional computed tomography in diagnosis and management of Jacob's disease

This study reports a case of Jacob's disease characterized with limited mouth opening due to bilateral coronoid hyperplasia forming pseudojoints with zygomatic arches. Magnetic resonance (MR) examination of temporomandibular joint (TMJ) is usually the imaging method chosen in patients with such symptoms. However, the coronoid processes can not be displayed because they are not included in field of view in MR imaging of TMJ. For that reason, these patients may be treated for a misdiagnosis of TMJ disorders. In this study, the aetiology and diagnostic methods of Jacob's disease, the pre-operative/post-operative role of three-dimensional computed tomography and some measurements used in diagnosis were evaluated.

At this juncture, surgical intervention is required. However, complications such as inhibition of jaw growth and postoperative relapse occur in patients treated at an early age. In addition, the timing of surgery is controversial. The current report describes a patient presenting with hyperplasia of the bilateral coronoid process who underwent surgery at an early age.
