Early childhood caries epidemiology, aetiology, risk assessment, societal burden, management, education, and policy: Global perspective

This paper is a summary of the proceedings of the International Association of Paediatric Dentistry Bangkok Conference on early childhood caries (ECC) held in 3-4 November 2018. The paper aims to convey a global perspective of ECC definitions, aetiology, risk factors, societal costs, management, educational curriculum, and policy. This global perspective on ECC is the compilation of the state of science, current concepts, and literature regarding ECC from worldwide experts on ECC. Early childhood caries is related to frequent sugar consumption in an environment of enamel adherent, acid-producing bacteria in a complex biofilm, as well as developmental defects of enamel. The seriousness, societal costs, and impact on quality of life of dental caries in pre-school children are enormous. Worldwide data show that ECC continues to be highly prevalent, yet infrequently treated. Approaches to reduce the prevalence include interventions that start in the first year of a child's life, evidence-based and risk-based management, and reimbursement systems that foster preventive care. This global perspective on ECC epidemiology, aetiology, risk assessment, global impact, and management is aimed to foster improved worldwide understanding and management of ECC.

Access for all – an analysis of children-related abstracts at the International AIDS Conference, Bangkok

HIV issues in children are often less well highlighted than in adults. International conferences form a focal point for dissemination and research and this study provides an analysis of children related abstracts from the Bangkok AIDS conference (2004). A total of 8057 abstracts were printed onto a conference-specific CD, which forms the basis of this analysis. Submitting authors had a 6.08% chance of an oral abstract, a 62.9% chance of a poster presentation and a 31% chance of their abstract being rejected. A keyword search compared the topic of children with 18 other HIV-related topics. An in-depth analysis and coding of the child-related abstracts (n = 337 abstracts) examined patterns of acceptance, methodologies used (empirical or not) nature of data (qualitative vs. quantitative), subject matter, nature of population studied and age of children. Of 490 oral and 5068 poster abstracts accepted in Bangkok, 19 (3.9%) and 116 (2.3%) were child-related. Child-related abstracts formed 3.6% of accepted abstracts, appearing in all tracks as both oral and poster presentations, but less likely to feature than the average acceptance rate. A broad keyword search shows that medical topics [highly active anti-retroviral therapy (HAART), side effects, treatment] are more likely to be accepted than are child-related topics. Rejection rates for child-related abstracts were higher than for medical topics. Oral abstract acceptance was significantly more likely in the medical tracks (A, B and C) than the more social science tracks (D and E). There were more psychosocial than medical abstracts, yet medical abstracts were nearly four times as likely to be accepted for oral presentation (χ2 = 12.96, p > 0.005). Poster acceptance was roughly equal. Studies that utilized quantitative methodology had higher acceptance (χ2 = 9.6, p = 0.008). Medical abstracts were more likely to be empirical and quantitative. There was a threefold higher acceptance rate for empirical abstracts. More than 90% of qualitative abstracts were psychosocial. Psychosocial abstracts cross into the clinical fields, but with no reciprocity. Work on HIV-positive children was more likely to be accepted than on affected children. Work concentrating on younger children was less frequent and less likely to be accepted and more likely to appear in the medically related tracks. There is an under-representation of child-related abstracts at the Bangkok conference. A picture of prioritization of medical, empirical quantitative data emerged. This should inform research and abstract submission for the future.

The Bangkok Conference: steering countries to build national capacity for health promotion

Millions of young people in the developing world never achieve two decades of life let alone seven and so it is with mixed feelings that Health Promotion International celebrates its 20th birthday this issue. Much has been written and said about the antecedents and milestones of the health promotion phenomenon but what is clear from history is that any rapidly growing movement or organization needs to re-invigorate its purpose for existence as well as build its capacity for success. This is vital if health promotion is to be truly a response to both national and global challenges. The forthcoming Bangkok Conference and foreshadowed Bangkok Conference will seek to fill this gap. The 2004 International AIDS Conference and How to Globally Counter HIV/AIDS

This article reports noteworthy HIV/AIDS clinical trials presented at the XVth International AIDS Conference, Bangkok, July 2004, and also outlines goals of comprehensive prevention, care, treatment, and monitoring plans. The Bangkok conference theme was "Access for All." Outlined are goals of comprehensive prevention, care, and treatment programs: increased education and prevention efforts, greater involvement of national health authorities, reduction of new HIV infections, increased use of voluntary counseling and testing, increased acceptance and use of condoms, acceptance of an individual's right to be protected against HIV infection during sexual activity, increased support of NGOs, reduction of sexual partners, increased sexual fidelity, availability of antiretroviral medication, prevention of mother-to-child transmission, reduction of AIDS deaths, improved surveillance of sexually transmitted infections, improved blood supply security, increased coordination with tuberculosis and malaria treatment, equity for urban and rural persons, increased orphan services, reduction of orphan rate, greater involvement of local leaders, increased media involvement, reducing HIV/AIDS discussion taboo, reduced injecting drug user needle sharing, and continuing education for health care professionals. Monitoring parameters include incidence and prevalence of HIV infections, use of voluntary counseling and testing, condom use and attitudes to right of protection, AIDS deaths, orphan rate, public advertisements, leadership participation, antiretroviral use and availability, public awareness of services, blood supply security, and professional education.

Changing The Debate About Health Research For Development

We represent ten groups of citizens and researchers from low- and middle-income countries who were recipients of International Health Research Awards (IHRA) presented at the Bangkok Conference on International Health Research for Development, 2000. The IHRA, funded by the Rockefeller Foundation and administered by the World Health Organization, recognized the need for us to find effective ways of solving our countries' health problems and acknowledged the special environmental constraints we face in conducting research and exploiting the results. We used this funding to explore alternative ways for harnessing largely untapped resources in our countries to generate and apply knowledge to improve people's health and address inequalities. In the Bangkok Declaration, we joined many others to express our commitment to research as a vital force for health of all of our people.

L'auteur explique le role qu'elle a joue dans les changements du Bureau de l'IFLA, et plus particulierement dans la recherche du nouveau Secretaire General. Elle presente egalement les activites qu'elle a accomplies dans le cadre de l'IFLA l'annee derniere, dont le programme mondial de visites et la definition des priorites avec des organisations en relation. INTERNATIONAL RELATIONSHIPS IN MISSION

Introduction(1) A. Summary of historical developments in international relationships in mission Four different periods can be distinguished in the contemporary conciliar or ecumenical missionary movement: The first main period starts in the middle of last century and lasts until 1961. It finds its clearest expression in the aims of the Edinburgh mission conference of 1910 and later in the formation in 1921 of the International Missionary Council (IMC). During that period, Protestant mission agencies, movements or councils, tried to find institutionalized ways to coordinate their work or at least to share information, first among themselves as northern mission bodies, and later on also between them and emerging church structures in other continents. The second main period can be understood as starting in 1938 and lasting until 1972. During these 35 years, the mission reflection was specifically church-centred, an emphasis that proved to have institutional consequences. It started with the Tambaram conference of the IMC, held in a period marked by a new emphasis laid on the role of the (confessing) church in mission work, in a context of dangerous political movements drawing on 'militant paganism.' The unfinished task of evangelisation was understood as being the responsibility of the whole church, the whole world being the field of action. The accomplishment of the task was to be done in partnership between 'older' and 'younger' churches in a pool of resources. The expression of 'partnership' was repeated in the IMC meeting at Whitby in 1947 and very often used since then. The formation in 1948 of the WCC itself can be understood as an expression of the church-centred approach. In the fifties, the same emphasis pushed mission bodies towards facilitating the creation of national councils of churches, instruments of local ecumenical church cooperation, and towards envisaging the integration of mission structures with church structures. That was accomplished on the world level in 1961 with the merger of the IMC and the WCC. The same dynamics were followed in the WCC programs to foster coordination of the exchange of persons and resources, locally and internationally (Joint Action for Mission and Ecumenical Sharing of Personnel). The same ecclesio-centric missiology eventually pushed mission societies to create new international church structures for mission as was the case within the Paris Mission, where the reflection and debate lasted from around 1963 to 71. That led to the third main period which can be characterized as the period of new intercontinental structural models of relationships in mission. It started in 1971/72 and still lasts. At the Bangkok mission conference organised by the Commission for World Mission and Evangelism (CWME) of the WCC (carrying on the IMC tradition), two 'models' for addressing unjust and alienating international church and mission relations were proposed: A) the moratorium understood as a temporary halt of sending personnel and other resources from the North to other continents. First proposed (already) in 1928 by P. Ooman Philip from India, the moratorium idea came up again in the beginning of the seventies within the then East Asia Christian Conference and the All Africa Conference of Churches. Despite its reception at the CWME Bangkok conference, it was not put into practise in many places. In countries like Burma or China, political decisions put churches in a moratorium situation. The Philippines as well as the Malagasy Republic, e.g., would be examples where the churches themselves followed a policy inspired by the moratorium idea. It must be added that what could be called a 'moratorium mood' indeed influenced many church or mission bodies although they did not put the proposal fully into practice. B) In a certain way as an alternative to moratorium, the model of the Communaute Evangelique d'Action Apostolique (CEVAA) was also presented at Bangkok. …

The Right to Development as a Human Right?

This article examines efforts to strengthen human rights, especially at the Bangkok Conference of 1993, and discusses the conflict between traditional Western views of human rights as individual rights, as opposed to the idea of collective rights. In this context, the author analyzes the debate over the right to development vigorously espoused at Bangkok and the concern of some Western governments that this might undermine the concept of individual human rights. He argues that an interpretation of the right to development as a collective right is compatible with the individualist tradition of human rights and urges further dialogue so as to recognize different views on what constitutes a dignified life.

Differential and More Favorable Treatment of Developing Countries and the International Trading System

This article argues that and more favourable of developing countries in the General Agreement on Tariffs and Trade (GATT) has been a logical consequence of their own inward-looking policies and the GATT's implicit mercantilism, the latter implying that liberalization, being costly, should not be demanded of relatively poor countries. Time has, however, reduced both the appeal of the protectionist model of development and the willingness of developed countries to accord differential treatment. The upshot has been pressure on more advanced developing countries to graduate and a growing literature recommending fuller and more equal participation of developing countries in the GATT. The case for fuller and more equal participation is not self-evident. It needs to be assessed on its merits in terms of the prospects for improved market access abroad and more efficient policy at home. The analysis indicates that the potential benefits should not be oversold. On balance, however, the most advanced developing countries would probably gain from active and more equal participation in both GATT and the multilateral trade negotiations while the remaining developing countries would benefit from graduation by the more advanced. (iv) Contracting parties agree that the principle of differential and more favourable treatment embodied in Part IV and other relevant provisions of the General Agreement and in the decision of contracting parties of November 28, 1979 on differential and more favourable treatment, reciprocity and fuller participation of developing countries applies to the negotiations. In the implementation of standstill and rollback, particular care should be given to avoiding disruptive effects to the trade of less-developed contracting parties. (v) The developed countries do not expect reciprocity for commitments made by them in trade negotiations to reduce or remove tariffs and other Martin Wolf is director of studies at the Trade Policy Research Centre, London. There is a wide-spread feeling that the recent conference on ‘Salvation Today’ which took place in Bangkok from December 28th, 1972 to January 8th, 1973, under the auspices of the World Council of Churches’ Commission on World Mission and Evangelism, may herald the beginning of a new period in the missionary endeavour of the Christian churches. A significant feature of the conference was that delegates from churches in the Third World outnumbered those from the West. They attacked the sending of missionaries and funds by powerful Western churches to the poor churches in the Third World, urging that the missionary enterprise should become ‘completely mutual and international’, and even proposed a moratorium on the sending of funds and personnel to Third World churches, to allow them to discover their own identity and resources for missionary expansion.Only a few months after the Bangkok conference had discussed the meaning of ‘Salvation Today’, 720 delegates attended a ‘Congress on Mission and Evangelism’ in Durban, South Africa. As in Bangkok, the greater part of those who took part in the South African congress were ‘Non-Whites’. There was a further similarity of great significance: The aim of the congress was ‘. . . to discover together the relevance of the Gospel and the meaning of mission and evangelism in present-day Southern Africa ... to assess the resources and potential of all Christian churches and agencies for mission and evangelism . . . and to give stimulus, inspiration and encouragement to Christians as they face this task’.
