Patient Experiences Regarding Feasibility of Implementing Real-World EQ-5D Collection at an Oncology Centre in Ontario, Canada

Cancer treatments impact health-related quality of life (HRQoL). EQ-5D is a standardized generic measure of HRQoL. The objective of this project was to assess, from the patient’s perspective, the feasibility of implementing real-world EQ-5D-3L measurement at a pilot site, as a first step to large-scale collection of EQ-5D from patients with cancer across Ontario. This was a prospective longitudinal study at a single oncology centre to understand the feasibility of routinely collecting EQ-5D-3L while patients receive chemotherapy (N = 170). Consenting patients completed an additional questionnaire on feasibility, and a subset of participants were directly interviewed to provide further feedback and suggest improvements to questionnaire collection. Themes emerging from the interviews were analyzed using content analysis. Of 170 eligible and consenting patients who completed an initial EQ-5D-3L questionnaire, 103 (60.6%) completed at least one follow-up questionnaire. When asked about willingness to answer future questionnaires at subsequent visits, 115 (67.3%) answered definitely; 35 (20.5%) very likely. Patients provided feedback on their overall experience of completing EQ-5D-3L, the questionnaire presentation, frequency of completion, and analytic plans. Patients expressed that routinely collecting EQ-5D-3L is feasible. Incorporating patient feedback regarding EQ-5D collection will facilitate implementation of systematic collection at cancer centres across Ontario.

Comparison of self-report data validity in undergraduate samples using remote versus in-person administration methods.

In the internet age, recruitment, participation, and compensation for survey research can occur remotely, away from a laboratory setting. Although this method of data collection offers notable benefits such as access to more diverse samples and lower study costs, it is possible that rates of inattentive or otherwise invalid response patterns are more common when survey completion occurs without any oversight. To answer this question, undergraduate student participants (final n = 678; 65% women, 76% White/European American) were randomly assigned to complete a battery of self-report surveys either in a typical laboratory administration setting (in person) or remotely from the location of their choosing. Following a preregistered analytic plan, data from both conditions were screened using multiple methods for self-report data validation, including the use of embedded and stand-alone validity scales, response timing, and identification of multivariate outliers. Results showed null-to-small differences between survey administration conditions. However, differences between screening methods in the proportion of data flagged as "invalid" emerged. Pragmatic clustered randomised control trial to evaluate a self-regulated learning intervention to be implemented in South Australian primary schools—study protocol

BackgroundSelf-regulated learning (SRL) is described as a process whereby learners actively take control of their learning by setting goals, planning, monitoring, evaluating, and adjusting their learning strategies to improve performance and achieve desired outcomes Panadero (Front Psychol 8:422, 2017). SRL proficiency has been shown to predict educational success and lifelong outcomes, such as income and health. While SRL is recognised as a key lifelong competency, there remain questions regarding how educators can best develop and promote SRL in school settings. A scalable, low-cost intervention targeted at grade 1 students (6–7 years old) in Germany was found to have substantial effects on impulse control and self-regulated learning, with sustained impacts on long-term academic success Schunk (Nat Hum Behav 6(12):1680-90, 2022). This study protocol seeks to adapt the Schunk et al. (2022) randomised trial to the Australian content and extend it to grades levels 2, 4 and 6.MethodsWe will use a standard pragmatic clustered (by school) randomised controlled superiority trial with an additional population-wide matched parallel control arm. Effectively, we will conduct three trials—one for each age/grade level. Each trial will be powered to assess the impact of the intervention on the age/grade groups independently: grade 2 (early primary, 7–8 years), grade 4 (mid primary, 9–10 years), and grade 6 (late primary, 11–12 years). Schools assigned to the treatment group will have all three grade levels (grades 2, 4, and 6) receiving the treatment (at least one class per grade); no classes in the schools assigned to the control group will receive the intervention. A minimum of 56 schools with an average cluster size of 19 children/class will be required to detect a minimum impact of 0.25 SD effect size at 80% power taking into account the clustered design with an intraclass correlation coefficient (ICC) of 0.05. This results in a total sample of 1064 per grade and thus 3192 students in total (56 schools per arm × 19 students in an average-sized class × three grade levels). Randomisation will occur on a 1:1 ratio, such that half of the schools (n = 28), and effectively about half of the students (n = 1596) will receive the intervention. The primary outcome will be improved self-regulation assessed at 6 weeks, 6 months and 12 months post the intervention. Longer-term secondary outcomes will include academic and wellbeing measures obtained through administrative data linkage to the National Assessment Program in Literacy and Numeracy (NAPLAN) outcomes and the Wellbeing and Engagement Collection (WEC) outcomes measures in the year following implementation (grades 3, 5, and 7). Follow-up via the South Australia Data Linkage Systems will allow for longer-term academic outcomes, mental health, school completion, criminal justice, and tax data.DiscussionThis protocol paper provides a detailed record of the trial design. We also discuss our analytical plan, especially highlighting the opportunities associated with the linkage of the trial participants to South Australia’s population-wide administrative data linkage systems.Trial registrationACTRN12623001331628. 595 Is PHASTR faster? A target trial emulation case study in the N3C

Objectives/Goals: Our study team won a Public Health Answers to Speed Tractable Results (PHASTR) contract to conduct a target trial emulation to answer “Does metformin show a reduction of severe outcomes of COVID-19 or of Long COVID in the N3C Data Enclave?” We quickly delivered an answer due to productive technical and collaboration support in the N3C. Methods/Study Population: Our analytic plan was updated based on helpful feedback from the PHASTR program. We performed a trial emulation analysis using the N3C data, comparing adult new users of metformin to controls prescribed fluvoxamine, fluticasone, ivermectin, or montelukast. The composite outcome was Long COVID or Death (LC/D) within 180 days of COVID infection. We used entropy balancing to estimate the average treatment effect with a weighted log-linear model. Productivity was enhanced by reusing code workbooks and validated codesets from related N3C projects. The team of 4 (physician, informaticist, data programmer, and statistician) and key unpaid advisors spent 10 weeks developing and analyzing the data. Results/Anticipated Results: Totally, 9,660 patients were identified for analysis. After weighting, there were 248 in the metformin and control groups. In the metformin group, 4.0% developed LC/D vs. 8.5% in the control group, with an adjusted risk ratio (aRR) of 0.47 (95% CI 0.25 to 0.89). Results were consistent across subgroups and sensitivity analyses. The PHASTR contract structure helped produce high-quality results quickly by not only providing funding but also requiring a compressed timeline for a small team to focus on the study. The most time was spent on contract execution, enclave provisioning, and too many last-minute download requests. A project final report was submitted in March and a full manuscript was submitted in September. Discussion/Significance of Impact: The analysis was productive because the environment made reuse easy and supported rich collaborations among clinicians, informaticists, epidemiologists, statisticians, and data developers. Advice from PHASTR advisors (Axel) and N3C diabetes domain team members was also key to a faster completion.

Abstract P3103: Circulating and tissue levels of omega-3 fatty acids and incident peripheral artery disease: an individual participant-level pooled analysis of prospective studies

Background: Evidence from observational studies and randomized trials suggest a favorable role of omega-3 fatty acids on cardiovascular outcomes. However, whether omega-3 fatty acids may reduce the incidence of peripheral artery disease (PAD) is not known. Aims: We prospectively evaluated blood and adipose tissue levels of alpha-linolenic acid (ALA), eicosapentaenoic acid (EPA), docosapentaenoic acid (DPA), docosahexaenoic acid (DHA), and the sum of EPA and DHA, with respect to incident PAD. Methods: We included 11 prospective studies from a global consortium up to May 2023 with measurements of ALA, EPA, DPA, or DHA (as a % of total fatty acids) in blood or adipose tissue among adults (age≥18), who were free of coronary artery disease, stroke, and PAD at baseline, and assessed incident PAD events. Each cohort conducted de novo individual-level analyses with a prespecified analytical plan and harmonized definitions for exposures, outcomes, covariates (including demographics, cardiovascular risk factors, medication use, and omega-6 fatty acid levels), and subgroups. Pooled hazard ratios (HRs) were calculated using inverse-variance weighted meta-analysis. Results: Among 142,316 participants from North America, Europe, and Australia followed for a weighted median of 13.7 years (range of median follow-up: 9.8 to 26.2 years), 1,842 incident cases of PAD were ascertained. In multivariable-adjusted pooled analysis, very long-chain EPA, DPA, DHA, and EPA+DHA each associated with lower PAD incidence, with HRs (95% CI) per interquintile range of 0.79 (0.69, 0.91), 0.80 (0.68, 0.95), 0.74 (0.64, 0.85), and 0.79 (0.69, 0.90), respectively ( P <0.01 for each) ( Figure 1 ). Long-chain ALA was not associated with PAD, 1.14 (0.98, 1.33). Heterogeneity between studies was low to moderate ( I 2\ ranging from 0% to 57%). Results were broadly consistent in prespecified subgroups by age, sex, smoking status, prevalent diabetes, and biomarker lipid fraction. Conclusion: In this large international consortium, objective levels of very long-chain, but not long-chain, omega-3 fatty acids inversely associated with incident PAD. Our data suggest a protective role for very long-chain omega-3 fatty acids in the development of PAD, supporting the need for further mechanistic studies and appropriately powered randomized controlled trials.

The Combined Effect of Multisensory Stimulation and Therapist Support on Physical and Mental Health of Older Adults Living in Nursing Homes: Pilot Randomized Controlled Trial.

Increasing life expectancy has led to a rise in nursing home admissions, a context in which older adults often experience chronic physical and mental health conditions, chronic pain, and reduced well-being. Nonpharmacological approaches are especially important for managing older adults' chronic pain, mental health conditions (such as anxiety and depression), and overall well-being, including sensory stimulation (SS) and therapist support (TS). However, the combined effects of SS and TS have not been investigated. This randomized controlled trial examines the specific and combined effects of brief SS and TS interventions on older adults' physical and mental health and pain intensity levels, among individuals living in nursing homes. A total of 96 patients aged 65-99 years from a nursing home were randomly assigned to 3 groups: SS, TS, and combined SS+TS interventions, each delivered as four 20-minute sessions. SS was implemented using a multisensory Snoezelen room. Pain intensity levels (per a Visual Analog Scale), blood pressure, heart rate, blood oxygen saturation, and hand grip strength (using a Jamar hand dynamometer) were measured before and after each of the 4 weekly therapeutic sessions. In addition, life satisfaction (per the Satisfaction with Life Scale) and anxiety (per the 7-item General Anxiety Disorder Scale) were evaluated before and after the whole intervention. Mixed model analyses tested the relative efficacy of the 3 interventions, applying simple slope analysis with Tukey correction. Study rationale and analytical plans were preregistered. The combined intervention of SS and TS (SS+TS) resulted in reduced pain levels compared with SS (B=0.209, P=.006) and TS alone (B=0.23, P=.002) over 4 sessions (F6,266=2.62; P=.017; R2=0.23). Further, the combined SS+TS intervention resulted in reduced systolic blood pressure versus SS (B=0.09, P=.01) and TS alone (B=0.016, P<.001) groups (F6,272=5.42; P<.001; R2=0.29). In addition, the combined SS+TS intervention resulted in an increased grip strength versus SS (B=-0.35, P=.003) and TS alone (B=-0.032, P=.008) groups (F6,273=2.25; P=.04; R2=0.19). Moreover, combined SS+TS resulted in an improvement in life satisfaction (B=-4.29, P<.0001) compared with SS (B=-2.38, P=.0042) and TS alone (B=-1.20, P=.13) groups (F2,39=3.47; P=.04). Finally, SS+TS demonstrated greater improvement in symptoms of general anxiety disorder (B=10.64, P<.0001) compared with SS (B=3.30 P=.01) and TS alone (B=1.13, P=.37) (F2,38=13.5; P<.001) groups. No differences between the interventions were shown for blood oxygen saturation (F6,273=2.06; P=.06), diastolic blood pressure (F6,272=1.12; P=.35), and heart rate (F6,273=1.33; P=.23). The combined intervention of SS and TS showed therapeutic benefits for pain management and physical and mental health of older adults living in nursing homes, relative to each therapeutic component in isolation. This brief intervention can be readily implemented to improve well-being and optimize therapeutic resources in nursing home settings. Racial and Ethnic Differences in Relations Between Childhood Adversity and Psychological Adjustment Among Child Welfare-Involved Youth: A Systematic Review

This systematic review examined the extent of evidence that meaningfully examines racial/ethnic differences in the association between childhood adversity and psychological adjustment among child welfare-involved youth and synthesized findings. We pre-registered our study rationale and methodological and analytic plan in PROSPERO (#CRD42022350707). We conducted a search for articles on May 11, 2022, in APA PsycInfo, Academic Search Complete, CENTRAL, Child Development and Adolescent Studies, CINAHL, Family Studies Abstracts, Psychology and Behavioral Sciences Collection, PubMed, Social Services Abstracts, Social Sciences Abstracts, Social Work Abstracts, SocINDEX, Web of Science, and Violence &amp; Abuse Abstracts. Inclusion criteria included: written in English, peer-reviewed, quantitative analysis, examined the association between childhood adversity and psychological adjustment and examined racial/ethnic differences using moderation or multiple group modeling, and included a sample of child welfare-involved youth (i.e., 0–18 years) in the United States. Two reviewers independently screened articles for inclusion, extracted data, and assessed quality. The final sample included 13 articles. There was evidence of racial/ethnic differences in the association between childhood adversity and psychological adjustment; however, some studies found that there were no significant differences across racial/ethnic groups. Variability in findings may be due to inconsistent assessment of childhood adversity and psychological adjustment, differences in sample age ranges and in how race/ethnicity was examined, and/or lack of power. Racial/ethnic differences were primarily between White, Black, and Hispanic racial/ethnic groups, and most studies were cross-sectional. It is important for future research to examine whether racial/ethnic differences exist in the association between childhood adversity and psychological adjustment over time.

Does Context Matter? Daily Diary Study Exploring the Effects of Stressor Intensity and Perceived Controllability on the Relationship Between Emotion Regulation and Affect in Adolescents.

Theoretical accounts emphasize the importance of context sensitivity in emotion regulation, yet research often neglects this. This study explored how adolescents' use of three emotion regulation strategies-cognitive reappraisal, distraction, and rumination-interacted with stressor intensity and perceived controllability to influence daily emotions. A total of 249 adolescents participated in a daily diary study (Mage = 12.73, SDage = 0.78; 63.1% males). Adolescents reported their daily use of emotion regulation strategies, emotional states, and contextual factors. In high-intensity stress situations, reappraisal and distraction each showed associations with reductions in negative affect, whereas rumination was related to increases in negative affect. In low-intensity stress contexts, reappraisal was associated with decreases in positive affect, and no significant effects emerged for distraction or rumination. For perceived controllability, no impact on negative affect was found; however, positive affect increased in uncontrollable situations when levels of reappraisal and rumination were low. Overall, the findings underscore the importance of stressor intensity and perceived controllability, suggesting that distraction's effectiveness is less context-dependent than reappraisal and rumination. Abstract BackgroundWhile a great deal of recent effort has focused on addressing a perceived reproducibility crisis within brain structural magnetic resonance imaging (MRI) and functional MRI research communities, less attention has been paid to reproducibility of brain positron emission tomography (PET) research.MethodsWe examined the current landscape of factors that contribute to reproducible neuroimaging data analysis, comparing brain MRI to brain PET. The factors included scientific standards, analytic plan pre‐registration, data and code sharing, containerized workflows, and standardized processing pipelines. To demonstrate the positive impact that further developing such reproducibility factors would have on brain PET research, we conducted one case study in which the brain PET processing pipeline developed for one flagship study (Alzheimer’s Disease Neuroimaging Initiative, ADNI) for brain 18F‐florbetapir and 18F‐fluorodeoxyglucose (FDG) analysis was reproduced in our laboratory.ResultsCompared to fMRI, PET research encounters substantial challenges in adopting essential reproducibility practices. While 85% of fMRI studies adhere to the standardized reporting practices, PET is in the early stages of establishing reporting practices, lacking a standardized checklist. Pre‐registration is prevalent for fMRI (57.6%) but notably absent in PET, with no pre‐registered PET studies found on platforms such as the Center for Open Science. The number of shared datasets is higher for fMRI compared to PET, with 92 fMRI datasets and only 9 PET datasets on repositories like OpenNeuro. Moreover, there are very few data repositories for newer PET radiotracers. Containerized software availability also favor fMRI, with platforms like (Brain Imaging Data Structure, BIDS) apps hosting 66% dedicated fMRI tools, while no PET‐specific software is available. Standardized processing pipelines follow the same trend, with fMRI boasting optimized workflows for specific protocols, while PET has few dedicated pipelines and lacks validation studies for existing options. Furthermore, while our case study demonstrated excellent correlation results for both the 18F‐FDG and 18F‐florbetapir methods (Figure 2), the replication of the ADNI PET pipeline posed substantial challenges and implementation delays. These obstacles and delays in execution were due to incomplete documentation and ambiguities in application details.ConclusionPET neuroimaging lags behind its MRI‐related counterparts in achieving robust reproducibility.

Relationship Between Food Service Satisfaction and Macronutrient Intake With Nutritional Status of Patients at DKT TK IV 02.07.04 Hospital Bandar Lampung

Malnutrition affects 13-69% of people receiving care in hospitals worldwide. Anthropometric examinations reveal that 50% of patients in hospitals suffer from malnutrition. Patient psychology during the healing process is affected by a number of factors, including the patient's level of satisfaction with the hospital food service. The purpose of this study was to determine how the nutritional health of patients at DKT Hospital Bandar Lampung is related to satisfaction with food service and macronutrient consumption. This study uses a cross-sectional strategy as a design or analytical observational research plan. The sample used in this study was 47 respondents selected using the simple random sampling method. The bivariate test used in this study is the Chi-Square test. The results showed that there was a relationship between food service satisfaction and patient nutritional status (p-value = 0.013) and there was a relationship between macronutrient intake and patient nutritional status (energy = 0.023, protein = 0.035, fat = 0.021, carbohydrates = 0.045). Based on the results of the study, it can be concluded that there is a significant relationship between food service satisfaction and macronutrient intake with the nutritional status of patients at DKT TK IV hospital 02.07.04 Bandar Lampung.

A Reanalysis: SRSS-IE Internalizing Cut Scores to Support Data-Informed Decision-Making Efforts in Elementary Schools

We report findings of this replication study, examining the internalizing subscale (SRSS-I4) of the revised version of the Student Risk Screening Scale for Internalizing and Externalizing behavior (SRSS-IE 9) and the internalizing subscale of the Teacher Report Form (TRF). Using the sample from 13 elementary schools across three U.S. states with 195 K–5 students, we replicated the initial data analytic plan used to determine initial cutting scores. Results from logistic regression and receiver operator characteristic curves suggested the following preliminary SRSS-I4 risk categories: 0–1 low, 2 moderate, and 3–12 high risk for internalizing behavior patterns relative to the TRF internalizing subscale score. Results yielded adequate levels of sensitivity and specificity for the first cut, and questionable sensitivity for the upper cut. We planned to conduct subsequent analysis to address the nested nature of the data. Yet, most teachers completed the TRF for one student making this not possible. We discuss limitations and directions for future research, calling for replication with larger samples to facilitate nesting given schoolwide screening practices involve teachers screening multiple students. We also encourage consideration of two risk categories: 0–1 for low and 2+ for a risk category, not distinguishing between moderate- and high-risk categories.

NCMP-16. CNS METASTATIC PATTERNS IN BREAST CANCER TREATED WITH CDK4/6 INHIBITORS: PRELIMINARY RESULTS OF A RETROSPECTIVE ANALYSIS

Abstract BACKGROUND CDK4/6 inhibitors combined with hormonal therapy or as monotherapy (abemaciclib) play an important role in the management of hormone receptor positive, HER-2 negative breast cancer (BC). To date, these drugs were shown to improve progression-free survival, whereas their comparative effect modulating breast cancer central nervous system metastases (CNSm) has not yet been extensively studied. Here, we present the early findings and analytic plan of our retrospective study to evaluate CNSm patterns and differences among CDK4/6-inhibitors in BC. METHODS In this retrospective chart review study, we queried the Johns Hopkins Electronic Medical Record from 1/1/2014 to 4/25/2024 using the following prescreening criteria: 1) Age ≥18 years; 2) Diagnosis and/or problem list including BC; 3) Palbociclib, abemaciclib, or ribociclib included in the medication list. The identified patients were dichotomized based on the presence/absence of ICD10 codes for CNSm and are currently being chart-reviewed. Patients without longitudinal data or confirmed CNS or with CNSm diagnosed before/within 1 month of CDK-inhibitor initiation were excluded. Population: Prescreening identified 1,419 patients (1,055 palbociclib, 301 abemaciclib, 145 ribociclib) of whom 239, 36, and 19 patients had ICD10 codes for brain metastases, respectively. After exclusion of false positives from this cohort, we confirmed 53 patients with CNSm in the palbociclib, 3 in the ribociclib, and 1 in the abemaciclib group. RESULTS We present the current data from the first-line palbociclib cohort (N=53) where, median time to CNSm on was 24 months (interquartile range 13-46). Fifty-two (98.1%) patients had documented bone, 31 (58.5%) dural, 7 (13.2%) leptomeningeal, and 33 (62.3%) calvarial involvement. Dural and calvarial metastases were strongly associated with each other (pearson-chi2=10.7; p=0.001). CONCLUSIONS This is a work-in-progress, with early preliminary findings showing a high rate of dural and calvarial metastases in this cohort. A more detailed review of the dataset that will also include the CNS-metastasis-free cohort is underway.
