This study confirms the null hypothesis, which indicates that 70.3% have high knowledge about vaccination, and it is associated with the age, education, and occupation of the mothers.

The ethics and practice of perinatal care at the limit of viability: FIGO recommendations.

An arbitrary gestational age limit of viability cannot be set, and in clinical practice the focus should be on a periviability interval-the so-called "gray zone" of prognostic uncertainty. For cases within this interval, the most appropriate decision-making process remains debatable and periviability has emerged as one of the greatest challenges in bioethics. Universally recognized ethical principles may be interpreted differently due to socioeconomic, cultural, and religious aspects. In the case of periviability, there is considerable uncertainty over whether interventions result in a greater balance of clinical good over harm. Furthermore, the fetus or neonate is unable to exercise autonomy and the physicians and parents will act as patient surrogates. When parents and physicians disagree about the infant's best interest, a dialogue without paternalistic attitudes is essential, whereby physicians should only offer, but not recommend, perinatal interventions. Parental choice, based on thorough information, should be respected within the limits of what is medically feasible and appropriate. When disagreements between parents and physicians occur, how is consensus to be achieved? Professional guidelines can be helpful as a framework and starting point for discussion. In reality, however, guidelines only rarely draw categorical lines and in many cases remain vague and ambiguously worded. Local ethics committees can provide counseling and function as moderators during discussions, but ethics committees do not have decision precedence. Counseling assumes the most significant role in periviability discussions, taking into consideration the particular fetal and maternal characteristics, as well as parental values. Several caveats should be observed relative to counseling: message fragmentation or inconsistence should be minimized, prognosis should preferably be presented in a positive framing, and overreliance on statistics should be avoided. It is recommended that decisions regarding neonatal resuscitation in the periviability interval be made before birth and not conditional on the newborn's appearance at birth. Regardless of decision, it is important to assure pre- and postnatal coherence. The present article describes how individual physicians, centers, and countries differ in the approach to the decision to initiate or forgo intensive care in the periviability interval. It is impossible to provide a global consensus view and there can be no unifying ethical, moral, or practical strategy. Nevertheless, ethically justified, quality care comprises early involvement of the obstetric and neonatal team to enable a coherent, comprehensible, nonpaternalistic, and balanced plan of care. Ultimately, physicians will need to adjust the expectations to the local standards, local outcome data, and local neonatal support availability.

Developmental delay can precede neurologic regression in early onset metachromatic leukodystrophy

ObjectiveMetachromatic leukodystrophy (MLD) is a rare neurodegenerative disorder. Emerging therapies are most effective in the presymptomatic phase, and thus defining this window is critical. We hypothesize that early development delay may precede developmental plateau. With the advent of presymptomatic screening platforms and transformative therapies, it is essential to define the onset of neurologic disease. MethodsThe specific ages of gain and loss of developmental milestones were captured from the medical records of individuals affected by MLD. Milestone acquisition was characterized as: on target (obtained before the age limit of 90th percentile plus 2 standard deviations compared to a normative dataset), delayed (obtained after 90th percentile plus 2 standard deviations), or plateau (skills never gained). Regression was defined as the age at which skills were lost. LI-MLD was defined by age at onset before 2.5 years. ResultsAcross an international cohort, 351 subjects were included (n = 194 LI-MLD subcohort). The median age at presentation of the LI-MLD cohort was 1.4 years (25th–75th %ile: 1.0–1.5). Within the LI-MLD cohort, 75/194 (39%) had developmental delay (or plateau) prior to MLD clinical presentation. Among the LI-MLD cohort with a minimum of 1.5 years of follow-up (n = 187), 73 (39.0%) subjects never attained independent ambulation. Within LI-MLD + delay subcohort, the median time between first missed milestone target to MLD decline was 0.60 years (maximum distance from delay to onset: 1.9 years). InterpretationEarly developmental delay precedes regression in a subset of children affected by LI-MLD, defining the onset of neurologic dysfunction earlier than previously appreciated. The use of realworld data prior to diagnosis revealed an early deviation from typical development. Close monitoring for early developmental delay in presymptomatic individuals may help in earlier diagnosis with important consequences for treatment decisions.

Early marriage is a significant social problem in Indonesia, including in Cilengkrang District, Bandung Regency. In an effort to address this problem, the Indonesian government issued Law Number 16 of 2019 which sets the minimum age of marriage to 19 years for both men and women. Community service in socializing this law in Cilengkrang Sub-district aims to increase community understanding and compliance with the regulation. The program involves various educational activities, such as counseling, group discussions, and training for community leaders and educators. The methods used include direct approaches through village meetings and indirect methods through social and print media. The results showed that the involvement of community leaders and religious leaders was very effective in conveying messages and reducing resistance to legal change. The main challenges faced were changing entrenched mindsets and problems accessing information in remote areas. Recommendations to improve the effectiveness of this program include increasing the involvement of community leaders, optimizing the use of media, continuous training, inter-agency cooperation, periodic evaluation, and a culturally sensitive approach. With the implementation of these recommendations, it is expected that the socialization of the marriage age limit law in Cilengkrang Sub-district can be more effective, reduce the number of early marriages, and create an environment that supports the optimal development of children and improves the overall welfare of the community.

Research in physiology of ruminant nutrition using the in sacco and mobile bag methods and developing and testing of a duodenal cannula of our own construction including our own surgical application in cattle, led to the intellectual property protection. The duodenal monocannula for determination of nutrient intestinal digestibility in cattle, designed by the National Agricultural and Food Centre (NPPC, Slovak Republic), has been registered by European Union intellectual property office, as a Registered Community Design. The innovativeness of the technical design of the "NPPC cannula" lies in the fact that the cannula is made of one piece of a straight tube with an inner diameter of 22 mm. The tube is smooth, with no additional extensions and glued parts. The operation does not require ligation of the bowel with an arterial prosthesis or a polyurethane felt. The dimensions of the cannula allow the use of the mobile bag method without any problems. The vinyl chloride emulsion was designed to polymerize at the lower temperature, than has been currently used in this procedure (130 °C), and the end used for fixation in the intestine could be shaped after immersion in warm water. During the growth of the animals, the tube length can be prolonged to adapt to the duodenal size. As a result, the life of cannulated animals can be extended without age limitation.

Hazard of Failed Nonoperative Management for Symptomatic Accessory Navicular in Children and Adolescents: A Population-Based Case-Cohort Study.

The accessory navicular (AN) is an idiopathic condition of the foot present in 4% to 21% of the population. Most ANs remain asymptomatic, but children and adolescents who develop symptoms can have remarkably reduced quality of life. Although many respond to conservative measures, surgery is occasionally needed. Our purpose was to determine factors associated with the failure of nonoperative management. This single-institution retrospective case-cohort study included patients up to age 19 years presenting between 2000 and 2021 with symptomatic AN and treated with standard-of-care. All 298 surgical cases, indicating failed nonoperative treatment, were included. For the subcohort, 299 patients were randomly sampled from all eligible patients, regardless of treatment. Baseline characteristics were summarized for the surgical cases and subcohort. Proportional hazards assumptions were checked and stratification implemented when necessary. Marginal structural proportional hazard modeling was used to estimate hazard ratios with 95% confidence intervals via inverse probability and LinYing weighting methods. The 298 surgical cases failed nonoperative management at a median of 5.2 months (IQR, 2.0-11.6 mo). In the subcohort, 86 failures of nonoperative management and 213 nonfailures constituted a 28.8% surgery rate. In both cohorts, nearly all patients played sports. Univariate proportional hazard modeling found older age ( P =0.02) and activity limitation ( P <0.001) at presentation, female sex ( P =0.002), higher BMI ( P =0.01), AN on the right ( P <0.001), and bone marrow edema of the AN ( P <0.001) and navicular body ( P <0.001) on MRI were associated with increased hazard of nonoperative failure. Nearly all of the surgical cohort reported improvement in pain (278/296, 94%) and returned to their primary sport (236/253, 93%) after surgery. Most also experienced full resolution of symptoms (187/281, 67%). Symptomatic AN predominantly affects female athletes, leading to surgery in 28.8% of our subcohort. Conservative treatment may be less successful-and therefore surgery could be more strongly considered-in older age, activity limitation at presentation, female sex, higher BMI, right-sided AN, and bone marrow edema on MRI. Surgery is effective for symptomatic and functional improvement. Case-cohort-Level III.

Screening for Breast Cancer: A Comparative Review of Guidelines.

Breast cancer is the most common malignancy diagnosed in the female population worldwide and the leading cause of death among perimenopausal women. Screening is essential, since earlier detection in combination with improvements in breast cancer treatment can reduce the associated mortality. The aim of this study was to review and compare the recommendations from published guidelines on breast cancer screening. A total of 14 guidelines on breast cancer screening issued between 2014 and 2022 were identified. A descriptive review of relevant guidelines by the World Health Organization (WHO), the U.S. Preventive Services Task Force (USPSTF), the American Cancer Society (ACS), the National Comprehensive Cancer Network (NCCN), the American College of Obstetricians and Gynecologists (ACOG), the American Society of Breast Surgeons (ASBrS), the American College of Radiology (ACR), the Task Force on Preventive Health Care (CTFPHC), the European Commission Initiative on Breast Cancer (ECIBC), the European Society for Medical Oncology (ESMO), the Royal Australian College of General Practitioners (RACGP) and the Japanese Journal of Clinical Oncology (JJCO) for women both at average and high-risk was carried out. There is a consensus among all the reviewed guidelines that mammography is the gold standard screening modality for average-risk women. For this risk group, most of the guidelines suggest annual or biennial mammographic screening at 40-74 years, while screening should particularly focus at 50-69 years. Most of the guidelines suggest that the age limit to stop screening should be determined based on the women's health status and life expectancy. For women at high-risk, most guidelines recommend the use of annual mammography or magnetic resonance imaging, while the starting age should be earlier than the average-risk group, depending on the risk factor. There is discrepancy among the recommendations regarding the age at onset of screening in the various high-risk categories. The development of consistent international practice protocols for the most appropriate breast cancer screening programs seems of major importance to reduce mortality rates and safely guide everyday clinical practice.

Outcomes of active surveillance for Japanese patients with prostate cancer (PRIAS-JAPAN).

To report the outcomes of repeat biopsies, metastasis and survival in the Prostate Cancer Research International: Active Surveillance (PRIAS)-JAPAN study, a prospective observational study for Japanese patients, initiated in 2010. At the beginning,inclusion criteria were initially low-risk patients, prostate-specific antigen (PSA) density (PSAD) <0.2, and ≤2 positive biopsy cores. As from 2014, GS3+4 has also been allowed for patients aged 70 years and over. Since January 2021, the age limit for Gleason score (GS) 3 + 4 cases was removed, and eligibility criteria were expanded to PSA ≤20 ng/mL, PSAD <0.25 nd/mL/cc, unlimited number of positive GS 3 + 3 cores, and positive results for fewer than half of the total number of cores for GS 3 + 4 cases if magnetic resonance imaging fusion biopsy was performed at study enrolment or subsequent follow-up. For patients eligible for active surveillance, PSA tests were performed every 3 months, rectal examination every 6 months, and biopsies at 1, 4, 7 and 10 years, followed by every 5 years thereafter. Patients with confirmed pathological reclassification were recommended for secondary treatments. As of February 2024, 1302 patients were enrolled in AS; 1274 (98%) met the eligibility criteria. The median (interquartile range) age, PSA level, PSAD, and number of positive cores were 69 (64-73) years, 5.3 (4.5-6.6) ng/mL, 0.15 (0.12-0.17) ng/mL, and 1 (1-2), respectively. The clinical stage was T1c in 1089 patients (86%) and T2 in 185 (15%). The rates of acceptance by patients for the first, second, third and fourth re-biopsies were 83%, 64%, 41% and 22%, respectively. The pathological reclassification rates for the first, second, third and fourth re-biopsies were 29%, 30%, 35% and 25%, respectively. The 1-, 5- and 10-year persistence rates were 77%, 45% and 23%, respectively. Six patients developed metastasis, and one patient died from prostate cancer. Pathological reclassification was observed in approximately 30% of the patients during biopsy; however, biopsy acceptance rates decreased over time. Although metastasis occurred in six patients, only one death from prostate cancer was recorded.

Analysis of the Success of Religious Courts in Reducing the Number of Marriage Dispensations as an Effort to Support the Government in Preventing Child Marriage

Dispensation for marriage is actually considered a simalakama fruit, because changes to the age limit for marriage seem futile if in the end minors can legally marry with dispensation from the Court. The facts on the ground show that the ambiguity in Article 7 of Law No. 16/2019 on Marriage leaves more serious problems. As a result, several court institutions have been flooded with applications for dispensation for various reasons submitted by parents of children who wish to marry. Malang District, which contributed the highest number of marriage dispensations in East Java in 2022, has conducted various policy programs that have succeeded in reducing the number of marriage dispensations. This article analyzes the success of the Religious Court in reducing the number of marriage dispensations as an effort to support the government in preventing child marriage and what the level of success is by using descriptive qualitative research methods with Allan McConnell's Policy Success theory. This article explores the success of preventing child marriage from the program dimension and its level of success. The results showed that in the indicator of implementation according to the objectives, the level of success was included in Success. Meanwhile, in the indicator of achieving the expected results, the level of success is included in Resilient Success because although there is a decrease in applications for marriage dispensation every year, the number is still high. And finally, the indicator provides benefits to the target group, the level of success is included in Success.

Out-of-Court Assistance Based on the Principle of the Best Interests of the Child: Study on Examination Process of Marriage Dispensation Cases

This research aimed to elaborate the legal phenomenon of marriage postponement proposed by the applicant in the examination process of marriage dispensation cases at the Religious Courts of Bukittinggi. The decision is taken after the applicant receives out-of-court assistance in marriage dispensation cases by other institutions that has been explicitly regulated in the Supreme Court (PERMA) Number 5 of 2019. The purpose of the assistance is to ensure the children’s readiness in entering marriage and the follow up to limited amendment of Article 7 Marriage Law on age limit. This phenomenon is intriguing due to a sizeable finding on previous marriage dispensation cases where most applicants insisted on getting married soon. This field study is descriptive qualitative research applying the descriptive-analysis method. The empirical-juridical approach was employed, analyzing primary and secondary data sourced from interviews and documentations. The collected data were subjected to Creswell’s technique starting from data processing to data interpretation. The results showed that the assistance practice has successfully encourage many child applicants to make decisions based on their best interests, namely postponing the marriage that is facilitated through an integrated system that includes psychological examination, legal approach, and religious approach. Indeed, this system is non-existent in the previous examination process of marriage dispensation cases. Interpretation and realization of Article 15 of PERMA are perceived to have contributed to the decreasing number of marriage dispensation cases in the Religious Courts of Bukittinggi, and establishing PERMA Number 5 of 2019 as the law that guarantees the prevention of child marriage.

Effect of Ni content on 475°C embrittlement of directed energy deposited duplex stainless steel using a laser beam and wire feedstock

Duplex stainless steel (DSS), specifically the 2209 grade, is increasingly employed in additive manufacturing, particularly in processes like directed energy deposition using a laser beam with wire (DED-LB/w). However, a significant challenge arises when DSS faces brittleness within the temperature range of 250–500 °C. This study employs advanced characterization techniques, including atom probe tomography (APT) and transmission electron microscopy (TEM), to investigate DSS embrittlement after aging at 400 °C for up to 1000 h. The hardness analysis revealed that the higher Ni content in DED-LB/w-fabricated DSS cylinder promotes the age hardening compared to 2205 wrought DSS plate. Furthermore, APT and TEM demonstrated that, alongside the decomposition of ferrite into Fe-rich (α) and Cr-rich (αʹ) phases, clustering of Ni, Mn, and Si atoms contributes to the embrittlement. Although the Ni-Mn-Si-rich clusters could suggest nucleation of G-phase, the G-phase crystal structure was not observed by TEM. This might be attributed to the short aging time or limitations in the characterization technique. This work underscores the impact of characterization techniques on the measurement of spinodal decomposition, with APT providing capability of detecting nanometer sized clusters. By elucidating the complexities of 475 °C-embrittlement in DED-LB/w DSS, this study offers valuable insights for industrial applications and a deeper understanding of age hardening in duplex DSSs under specific manufacturing conditions.

Molecular deuteration is commonly seen in starless cores and is expected to occur on a timescale comparable to that of the core contraction. Thus, the deuteration serves as a chemical clock, allowing us to investigate dynamical theories of core formation. We aim to provide a 3D cloud description for the starless core L\,1498 located in the nearby low-mass star-forming region Taurus and explore its possible core formation mechanism. We carried out nonlocal thermal equilibrium radiative transfer with multi-transition observations of the high-density tracer to derive the density and temperature profiles of the L\,1498 core. By combining these observations with the spectral observations of the deuterated species, ortho- and we derived the abundance profiles for the observed species and performed chemical modeling of the deuteration profiles across L\,1498 to constrain the contraction timescale. We present the first ortho- detection toward L\,1498. We find a peak molecular hydrogen density of $ cm$^ $, a temperature of 7.5$_ $ K, and a deuteration of $ in the center. We derived a lower limit of the core age for L\,1498 of 0.16 Ma, which is compatible with the typical free-fall time, indicating that L\,1498 likely formed rapidly.

The debate on the age limit for presidential and vice-presidential candidates is an important issue in the context of Indonesian democracy. Democracy emphasizes the principles of equality and participation, while meritocracy emphasizes competence and experience. Currently, the minimum age limit for presidential candidates in Indonesia is 35 years old. Debates have arisen over the relevance of this age in the current political context, with arguments in favor of the role of younger generations bringing innovation versus the need for experience and wisdom. This study aims to re-evaluate the age limit for presidential candidates in Indonesia from historical, constitutional and international comparative perspectives, and examine its impact on political dynamics and leadership quality. The research methodology involves literature analysis and recent case studies. The results are expected to contribute significantly to the discussion on the age limit for presidential candidates and its implications for democracy and meritocracy in Indonesia.

Analysis of the constitutional court's decision on the age limit of presidential and vice-presidential candidates in the perspective of Rechtstaat

Purpose: This study aims to determine the application of the concept of obedience and application of the concept of the Rule of Law (Rechtstaat) and the concept of Trias Politica to the Constitutional Court Decision Number 90/PUU-XXI/2023 concerning the age limit for presidential and vice-presidential candidates and how the implementation and principles of individual freedom in the rule of law are implemented in Indonesia. Method: The research was conducted using normative research methods (doctrinal research) and a conceptual approach. Results: The research results show that the Constitutional Court's decision requires an approach to the concept of the Rule of Law (Rechstaat) and the Trias Politica concept so that there is no deviation from power and it is able to face the dynamics of change that occur in society, as well as making laws that are more autonomous from intervention by other authorities, such as political interests. Thus, the implementation of the rule of law in the context of a rule of law can be interpreted and followed by all citizens. Limitations: The main aim of applying the concept of the rule of law (Rechtstaat) and the concept of Trias politica is to limit the power of authority so as to produce legal science teaching centered on rights.

Efficient bridge steel bearing health monitoring using laser displacement sensors and wireless accelerometers

Steel bearings have been commonly used to counteract induced loading from thermal and traffic conditions in numerous bridges. However, their effectiveness has been compromised due to aging and maintenance limitations, potentially impacting the overall bridge system performance. Existing monitoring techniques for detecting malfunctioning steel bearings lack automation and precision, making them inadequate for long-term and real-time bridge dynamics assessment. This study proposes a response-based approach to identify bearing malfunction by analyzing the traffic-induced response in the bearing vicinity. To implement this approach, laser displacement sensors and wireless acceleration sensors were employed to monitor both malfunctioning and well-functioning steel bridge bearings. Significant differences in bearing performance were observed through response analysis and comparison. Laser sensor measurements revealed larger vertical deflections in the girder at malfunctioned bearing under traffic loading. Moreover, the investigation of the acceleration response in the bearing locality indicated that bearing malfunction could alter the vibrational characteristics of the vicinity, significantly affecting Cross Power Spectral Density (CPSD) and cross-correlation. To quantitatively evaluate the performance of steel bearings, a Condition Score (CS) was introduced. The CS exhibited a strong correlation with bearing damage, providing valuable insights for maintenance and decision-making processes in bridge asset management. This study offers a comprehensive and automated method for identifying steel bridge bearing malfunction by utilizing advanced monitoring techniques and introducing the CS for assessment. The results obtained from this approach can enhance bridge maintenance strategies and contribute to effective bridge asset management.
