Parental Awareness regarding Aerodigestive Pediatric Foreign Bodies, Eastern Province, Saudi Arabia

Background: Aerodigestive foreign bodies are common clinical problem that may lead to life threatening complications or sudden death. It’s commonly seen in children since they are curious to explore environmental objects with their mouth, while the development of airway protection reflexes and chewing capacity are immature. Objectives: This study aims to assess the extent of parents' awareness about the danger of foreign objects that can be swallowed or inhaled by their children and how to manage such cases. Materials and Methods: A cross sectional study was used to assess the level of parents' awareness about the danger of swallowing or inhaling foreign objects by their children and to assess their knowledge about dealings with such cases. The data were collected from 459 by a distributed questionnaire among parents in Al-sharqiya. Results: 60.3% of participants were aware of aerodigestive foreign bodies.Regarding the management of child with aerodigestive foreign bodies in different age groups. For child with age group 1 year and less, about (34.9%) of participants were aware of the managment. However, there is misunderstanding of managing child with age 5 years and less where only 5.6% of participants were aware of the management in this age group. As regards the children more than 5 years old, only 36.2% of parents were aware of the management. Conclusion: The current study showed that most parents are lacking knowledge about the managment of this problem which reflect the importance of education and increasing the awareness among them to reduce the morbidly and mortality rate among children.

This case reports a rare case of accidental metal spoon swallowed by a normal adult which was successfully removed without complication under local anesthesia using a McGill forceps via laryngoscope. Aerodigestive foreign bodies are almost always managed by the otolaryngologist.

The challenges of cardiothoracic surgery practice in Nigeria: a 12 years institutional experience.

Although the specialty of cardiothoracic surgery has been practiced in Nigeria for many years, open heart surgery (OHS) has only in the last decade become relatively more frequent, mainly through visiting foreign cardiac surgical teams. At this early phase of development it is faced with multiple challenges, especially financing and local skilled manpower for which solutions have to be identified in order to ensure sustainability and future growth. This study is aimed at highlighting these obstacles to growth of cardiothoracic surgery based on our own institutional experience at Lagos State University Teaching Hospital (LASUTH) and the current status of OHS activity in other cardiothoracic centers in Nigeria. Prospectively acquired data from our center from March 2004 to December 2015 was reviewed. A telephone survey was also conducted with all other institutions in Nigeria performing cardiac surgery. During the study period 1,520 patients underwent various procedures with a mean age of 37±22.4 years and 813 (53.5%) were males. There were 450 major procedures (29.6%), 889 minor procedures (58.5%) and 181 endoscopic procedures (11.9%). The top ten clinical diagnoses were empyema thoracis (17.5%), malignant pleural effusion (14.7%), chest trauma (12%), hemodialysis access (6.1%), bradyarrhythmia (5.3%), aerodigestive foreign bodies (4.1%), vascular injury (3.9%), pericardial disease (3.8%), lung cancer (3.6%) and congenital heart disease (3.4%). The range of procedures was chest tube insertion (41.6%), endoscopy (11.9%), lung procedures (7%), arterio-venous fistula (6.1%), pacemaker implantation (5.3%), vascular repair (4.4%), OHS (3.4%), esophageal procedures (2.6%), chest wall surgery (2%), video assisted thoracic surgery (2%), closed heart surgery (1.6%), diaphragmatic procedures (1.6%) and thymectomy (1%). Survey of 15 centers in Nigeria with cardiac surgery activity showed a total of 496 OHS cases between 1974 and 2016, with 330 cases (66.5%) done between 2012 and 2016. Infections, malignancy and trauma currently account for the bulk of cardiothoracic surgery practice in Nigeria, with surgical activity showing a predominance of minor procedures and comparatively minimal OHS activities. Identified challenges to increasing cardiothoracic surgical activity were limitations in manpower development, infrastructure, laboratory support, local availability of consumables, cost of surgery, funding mechanisms for surgery, multiple models for development of cardiac surgery, decentralization of efforts and lack of outcome data. Data collection and reporting of results must be started to enable development of more evidence-based practice.

A case control study of the factors associated with occurrence of aerodigestive foreign bodies in children in a regional referral hospital in South Western Uganda.

BackgroundAerodigestive foreign bodies (ADFB) in children are a common emergency in ENT clinics globally. The aim of this study was to determine the prevalence and common types of ADFB’s presenting to a referral hospital in South Western Uganda, and to review clinical presentation and factors that influence their occurrence among children under 12 years of age.MethodsWe conducted a case control study comprising 40 cases and 80 unmatched controls. Consecutive and random sampling were used for the cases and controls respectively. A questionnaire was used to collect data. Clinic records were reviewed to calculate prevalence.ResultsPrevalence was 6.6 % of all paediatric cases seen in the ENT department that year. The most common symptoms included: history of choking [45 %], sudden cough [72.5 %], stridor [60 %] and failure to swallow [35 %]. The most common location for an airway foreign body was the right main bronchus [40 %] and the upper one third of the oesophagus [32.5 %] for digestive tract foreign bodies. Seeds and coins were most frequently removed. Children from upper level SES had a significantly lower risk of foreign body occurrence [OR = 0.29, p = 0.02] compared to those from a low SES. Also significantly, most cases were referrals from other government health centres [p = <0.01]. The male to female ratio among cases was 2:1. Children of older mothers were less likely to have an ADFB.ConclusionPrevalence of ADFB’s is relatively high. The most common symptoms are a history of choking, cough and failure to swallow. Age under 5 years, male sex, younger maternal age and low socioeconomic status increased odds of ADFBs.

Aerodigestive tract foreign bodies: an experience at a teritiary-care hospital

Background: Foreign bodies (FBs) in aerodigestive tract are a common concern for all ENT surgeons and chest physicians. While FBs in air passage are commonly seen in younger children, FB in food passage is encountered in children and adults alike. Foreign body is ingested accidentally but occasionally homicidal or suicidal. This study is about the experience of aerodigestive FBs at a tertiary-care hospital. Objective: To reveal our experience of aerodigestive FBs in terms of (i) age/sex distribution of aerodigestive FBs; (ii) site of impaction/nature of aerodigestive foreign bodies; and (iii) different procedures done for aerodigestive foreign bodies. Materials and Methods: This is a retrospective study done in the Department of ENT and Head and Neck surgery and Department of Chest Medicine In this study, ENT operation theatre registers and registers of bronchoscopy laboratory of chest medicine were analyzed for all the data about removal of aerodigestive FBs from April 2007 to March 2014. Result: This study includes a total of 1125 foreign bodies, of which 878 were in digestive tract and 247 in respiratory tract. Bone chip was the commonest foreign body ingested, whereas whistle and seed were the commonest FBs inhaled. Conclusion: FBs in an airway is an acute emergency. The general physician should know when to suspect an aerodigestive FB and should refer such cases as soon as possible to tertiary centers for removal. Rigid bronchoscopy, especially, and flexible bronchoscopy in few selected cases are the treatments of choice for tracheobronchial FBs. Cricopharynx is the most common site of FB lodgment in the digestive tract. Right main bronchus is the commonest site of lodgment of inhaled FB. Seed of dry fruits is the commonest FB inhaled.

Aerodigestive foreign bodies are common in children and can result in significant morbidity if not recognized and managed appropriately. This requires a familiarity with this topic because the evaluation and management varies based on the patient's symptoms, object ingested or aspirated, and location of the foreign body. We saw a need to develop this module for our residents and fellows that work in our pediatric emergency department to standardize the approach to these patients. Methods: The module consists of a pre- and posttest, a PowerPoint presentation with embedded review questions, and a small-group exercise (think-pair-share). This module is designed to be used in a group classroom and requires approximately 55 minutes. Prior to the session, the instructor should review the lecture notes that accompany the PowerPoint presentation to become familiar with the concepts and the slides. Results: The mean score on the pretest was 53% compared to a mean score of 85% correct on the posttest. In addition, 93% of learners successfully created an algorithm for the management and evaluation of a patient with a suspected aspirated foreign body and 96% of learners were able to complete the suspected gastrointestinal foreign body algorithm. Discussion: This module references recent research providing evidence based guidance to the approach and management of aerodigestive foreign bodies. We believe this is an effective module that will be useful for anyone involved in the care of children. Upper aerodigestive magnetic foreign bodies in children

Small, powerful magnets are increasingly available in toys and other products, and are responsible for increasing numbers of foreign body injuries in children. Small, spherical, neodymium magnets available since 2008 are of particular concern. We aimed to identify all cases of upper aerodigestive foreign bodies at our institution over 15.5 years of study. Case series including all patients treated at an urban, tertiary care children's hospital who had upper aerodigestive magnetic foreign bodies, from January 1, 1998 through April 30, 2013. We manually reviewed 7,049 patient records abstracted from billing data to identify all patients 0 to 20 years of age who had upper aerodigestive magnetic foreign bodies. We identified four cases of upper aerodigestive magnetic foreign bodies, one involving the hypopharynx, and three involving the upper esophagus. Three occurred in 2010 or later. Two cases involve the ingestion of multiple, spherical, neodymium magnets recently marketed as desktop toys. In both of these cases, there was a rapid development of mucosal injury at the site of attraction between two magnets. As small, powerful magnets become more ubiquitous, pediatric magnet foreign body injuries are increasing. Although most are gastrointestinal, we identified four recent cases involving the upper aerodigestive tract. Multiple magnets lodged in the hypopharynx or esophagus can rapidly cause pressure necrosis of mucosal tissues, and merit prompt management. Education regarding magnet safety and improved magnet safety standards are needed to reduce the risk of these injuries. 4.

Clinical profile and management of aerodigestive foreign bodies in North - western Nigeria

Background: Aeordigestive tract emergences in Nigeria occur in all age groups. They sometimes have dreaded outcome if lodged in air passages. Objective: The aim is to present the results of this problem in a 5-year retrospective survey of cases in the Emergency Unit of the National Ear Care Centre, Kaduna, Nigeria, from January 2009 to December 2013. Methods: Retrieved cases were studied noting the clinical history, examination and intervention. The study included all patients diagnosed with aerodigestive foreign bodies within the period stated. Results: Fifty-seven thousand nine hundred and four patients were studied. Fifteen thousand eight hundred and fifty were emergencies. The hospital prevalence was 0.61% and the average annual incidence was 0.13%, male preponderance with male to female ratio of 2.24-1.00. All patients were symptomatic. Mortality was recorded in one patient who had a co-morbidity of esophageal perforation. Conclusion: Aerodigestive foreign bodies pose a serious challenge in a setting of poor resources and scarcity in diagnostic and interventional facilities

Aerodigestive foreign bodies in adult ethiopian patients: a prospective study at tikur anbessa hospital, ethiopia.

Introduction. Foreign bodies (FBs) in the aerodigestive tract are important causes of morbidity and mortality and pose diagnostic and therapeutic challenges. The best method of removal of an esophageal and tracheobronchial FB is endoscopic guided extraction. Objective. To present our experience of the removal of aerodigestive FBs in adult Ethiopian patients using rigid endoscopes. Methods. A hospital-based prospective study, at Tikur Anbessa Referral and Teaching Hospital, from January 2011 to December 2012 (over two years). Results. A total of 32 patients (18 males and 14 females) with a mean age of 28.0 ± 12.74 years were treated for FB ingestion and aspiration at Tikur Anbessa Hospital. The FBs were impacted at the esophagus in 18 (56.2%) patients, at the pharynx in 7 (21.8%), and at the air way in 7 (21.8%) patients. Pieces of bones were the commonest objects found in the esophagus (17/18 of the cases) and the pharynx (4/7), while fractured tracheostomy tubes and needles were frequently seen in the air way (3/7 cases each). The foreign bodies were visible in plain radiographs of 26 (81.2%) patients. Successful extraction of FBs was achieved by using Mc gill forceps in 11 cases, rigid esophagoscopes in 9 patients, and bronchoscopes in 4 cases. Four cases required open surgery to remove the foreign bodies. Two complications (one pneumothorax and one esophageal perforation) occurred. All patients were discharged cured. Discussion and Recommendations. Aerodigestive FBs are not so rare in the hospital and timely diagnosis and removal of accidentally ingested and aspirated foreign body should be performed so as to avoid the potentially lethal complications associated. Rigid esophagoscopy requires general anesthesia and is associated with its own complications, but our experience and outcome of its use are encouraging.

&#x0D; Introduction: Aerodigestive and Ear foreign bodies are common Ear Nose Throat (ENT) emergencies. They arecommonly encountered by otolaryngologists, paediatricians, and primary care physicians. Objective: The aim ofthis study is to analyze different types of foreign bodies and sociodemographic correlates of self-inflicted foreign body insertion in ear, nose and throat.&#x0D; &#x0D; Method: Seventy-four patients with aerodigestive foreign body coming to our Hospital over a period of one year, starting from January 1, 2012, were enrolled in the study. Hospital based cross sectional descriptive study was done. Socio-demographic data was collected by history and clinical examination of all those patients. The data collected from 74 patients were entered and analyzed using descriptive and analytical statisticalmethods using SPSS version 17.0.&#x0D; &#x0D; Result: There were total of 74 cases of ENT foreign body. Male predominance was noted (61%). Foreign body of ear was found to be most frequent (43%) followed by throat (37%) and nose (20%). Foreign body of nose was almost limited in paediatrics population (14 out of 15) whereas in the elderly group foreign body of throat was only found. Seeds, meat bolus or bone or both, insects and cotton bud were the most common foreign bodies. There was significant relation between the type of foreign body and age (p &lt;0.05) and the living foreign body and ear.&#x0D; &#x0D; Conclusion: Foreign bodies of nose, ear and throat are common in ENT department. They should be timely managed to prevent potential complications.

Paediatric aerodigestive foreign bodies: remember the nasopharynx

Nasopharyngeal foreign bodies are relatively uncommon, compared with other aerodigestive sites. They may mimic a lower airway location, or may be coughed up into the nasopharynx prior to endoscopic evaluation. To describe our experience with nasopharyngeal foreign bodies mimicking a more distal airway location. Case series from a tertiary referral children's hospital. We present a series of four children who were initially thought to have a lower aerodigestive tract foreign body (three respiratory, one oesophageal), but who were finally diagnosed with nasopharyngeal foreign body. The concept of panendoscopy at all foreign body removals is suggested as a means to avoid a missed nasopharyngeal foreign body. Although nasopharyngeal foreign bodies are uncommon, their rare ability to mimic distal sites mandates a thorough endoscopic approach to ensure that all sites (i.e. larynx, lungs, oesophagus and nasopharynx) are evaluated endoscopically.

3-Year Impact of a Provincial Choking Prevention Program

To determine the impact of a provincial choking prevention program (CPP) on the incidence of aerodigestive foreign body cases among children. The CPP, including posters, pamphlets, an informative video, and annual participation in the Parents & Kids Fair, was launched throughout Quebec in October 1999. The incidence rates of aerodigestive foreign body cases prior to implementation (during 1997-1998) and subsequently (2000-2002) within the province and our tertiary care centre (Sainte-Justine Hospital) were compared by estimating incidence rate ratios (IRRs) and associated 95% confidence intervals (95% CIs). No significant changes in the incidence of aerodigestive foreign body cases after program implementation were observed in our hospital (age-adjusted IRR 0.92, 95% CI 0.79-1.07). The provincial rates were higher after program implementation (age-adjusted IRR 1.15, 95% CI 1.05-1.25). To influence choking prevention habits, modifications to the campaign are required. Strategies are discussed.

Clinical experience of removing aerodigestive tract foreign bodies with rigid endoscopy in children.

This study was undertaken to document the aerodigestive tract foreign body accidents among children, and to investigate the circumstances surrounding these events. A review of the charts of pediatric patients admitted with the definitive or suspicious diagnosis of aerodigestive tract foreign bodies was carried out in the period between January 1, 1998 to December 31, 2002. There were 53 eligible children; 39 boys and 14 girls, with an age range of 7 months to 14 years. Food items were the most common airway foreign bodies and coins were the most common esophageal foreign bodies. Among the 32 patients who underwent bronchoscopy, no foreign body was identified in 9 patients. Among the 21 patients who underwent esophagoscopy, foreign body was removed in 19 patients. In 2 cases, large foreign bodies which we could not extract with forceps were pushed into the stomach. Foreign bodies in the airway and esophagus constitute a constant hazard in all age groups, which demands immediate approach and management. Although the rigid endoscopic removal of aerodigestive foreign bodies was successful in this series, the most effective treatment of foreign body accidents is their prevention.
