Factors influencing adoption of clinical information system in private clinics in Chennai, India

Healthcare information technology (HIT) is being adopted to improve the quality of the care provided. However, the rate of adoption of HIT among independent practitioners appears to be significantly low. The scope of the study is to understand the factors that influence technology adoption and to evolve a model to examine the factors that influence adoption of clinical information systems by private medical practitioners in Chennai, India. Existing literature were reviewed. After studying different models, a new model was evolved through discussions with experts and validated. The research methodology will involve a descriptive study using a structured research instrument with 54 randomly chosen target population and validated with statistical tools. The Cronbach alpha score ranged between 0.75 and 0.9. The outcome should motivate the private medical practitioners to use clinical information system to manage their patient's health data and improve the quality of the care provided to patients.

Physician perspectives on a tailored multifaceted primary care practice facilitation intervention for improvement of cardiovascular care

Practice facilitation is an effective way to help physicians implement change in their clinics, but little is known about physicians' perspectives on this service. To examine physicians' responses to a practice facilitation program, focussing on their overall satisfaction, perceived most significant clinical changes, and interactions with the facilitator. The Improved Delivery of Cardiovascular Care program investigated the impact of practice facilitation on improving the quality of cardiovascular primary care in Eastern Ontario, Canada, from 2007 to 2011. We conducted a qualitative content analysis of post-intervention surveys completed by participating physicians, using a constant comparison approach framed around the Chronic Care Model. Ninety-five physicians completed the survey. Physicians overwhelmingly viewed the program positively, though descriptions of its benefits and impact varied widely. Facilitators filled three key roles for physicians, acting as a resource centre, motivator and outside perspective. Physicians adopted a number of changes in their practices. These changes include adoption of clinical information systems (diabetes registries), decision support tools (chart audits, guideline documents, flow sheets) and delivery system design (community resources). Most physicians appreciated having access to a practice facilitator and viewed the intervention positively. Insight into physicians' perspectives on practice facilitation provides a valuable counterpoint to outcomes-based evaluations of such services. Further research should investigate potential obstacles in the group of physicians who make fewer practice changes, as well as the sustainability of this type of facilitation intervention.

The tool developed and utilized was a change readiness survey to assess randomly selected team member and physician perceived readiness for the transition to an electronic documentation system. This article reviewed the rationale for using organizational change management techniques to facilitate adoption of a new clinical information system and discussed development of a change readiness survey tool. It explored the findings from the first 3 years of the survey.

Implementing a Computerized Charge Capture System to Improve Billing Work Flow and Reduce Errors in Data Entry

Background : Traditional paper-based billing operation has experienced a high illegibility error rate and the need for an extended processing time to cope with the billing tasks related to physician reimbursement. We hypothesized that information technology can be adopted to improve the effectiveness of the billing interface, assist in record keeping, and enhance financial outcomes of clinical practice. Methods : A computerized charge capture system was designed and deployed to test our hypothesis that information technology could reduce illegibility during data entry and improve patient record keeping and billing work flow. Results : Physicians found that electronic juxtaposition of the pilot system with the electronic medical record was helpful in patient data verification from the progress note. The pilot system was successfully designed to assist physicians in improving information access, and to reduce significant illegibility errors during data entry. Conclusion : Although overall technical improvements may provide access to desired information, major barriers to data entry and flawed health plan management systems still hamper the efficient implementation and financial outcomes of the adoption of clinical information systems in this time-pressured environment.

The Relationship Between Local Hospital IT Capabilities and Physician EMR Adoption

In light of new federal policies allowing hospitals to subsidize the cost of information systems for physicians, we examine the relationship between local hospital investments in information technology (IT) and physician EMR adoption. Data from two Florida surveys were combined with secondary data from the State of Florida and the Area Resource File (ARF). Hierarchal logistic regression was used to examine the effect of hospital adoption of clinical information systems on physician adoption of EMR systems after controlling for confounders. In multivariate analysis, each additional clinical IT application adopted by a local hospital was associated with an 8% increase in the odds of EMR adoption by physicians practicing in that county. Given this existing relationship between hospital IT capabilities and physician adoption patterns, federal policies designed to encourage this more directly will positively promote the proliferation of EMR systems.

Nursing self-efficacy of an integrated clinical and administrative information system.

Self-efficacy is a user's confidence that he or she has the ability to use an information system. A survey gathered demographics, self-assessed computer skills, attitude and self-efficacy before installation of an integrated clinical and administrative information system. Results showed that higher levels of nursing education, home computer use, and average levels of self-assessed e-mail, Internet search, word processing, and general computer expertise predicted self-efficacy of the system. In addition, previous use of home and office electronics equipment, such as an answering machine, predicted self-efficacy. Implications for training and future adoption of clinical information systems are presented.
