Introduction: Abdominal aortic aneurysms (AAAs) lack pharmacological treatment. Recent large genetic analyses have shown several loci associated with the presence of AAA but have not explored associations to AAA growth. Objective: One in five cases of thoracic aortic aneurysm and dissection (TAAD) are familial. Although more than 60 genes (with varying degrees of evidence) have been implicated in causing TAAD, suspicious variants are found in only one-third of patients. Via exome sequencing of DNA samples from patients with thoracic aortic disease, we applied advanced gene searching techniques in an effort to identify additional causative genes for TAAD.

We describe the case of a 66-year-old gentleman with a previous replacement of the ascending aorta for an acute Type A aortic dissection who did not attend any scheduled follow-up visit. Seventeen years later, he presented to our institution with severe aortic regurgitation and with a giant aneurysmal dilation of the abdominal aorta.

Although open surgery is standard of care for ascending aortic pathology, endovascular approaches can be viable options. We report the case of a 77-year-old man with a 5.7-cm ascending aorta penetrating ulcer. Given his age and clinical profile, the patient underwent Zone 0 thoracic endovascular aortic repair.
