Visitor Preferences to the Outeniqua Farmers’ Market in George, South Africa

This paper investigates visitors’ preferences to the Outeniqua Farmers’ Market (OFM) in George in the Western Cape Province of South Africa. The OFM is a typical farmers’ market that takes place every Saturday morning on the outskirts of the town, where stall owners and visitors congregate. Whilst it presents an opportunity for stall holders to sell their goods, it is also a platform where visitors to the event can have a meal and socialise. Using a quantitative research approach, 332 self-completion questions were analysed. The results indicate that about 40 percent of the visitors attended the OFM at least once a month, and the visitors were highly qualified – 41 percent had a post-graduate qualification. As was expected, 64 percent of the visitors were local and 71 percent from the Western Cape Province. As for the visitors’ preferences, 59 percent of the visitors were keen to purchase fresh products and 70 percent of the visitors were comfortable paying higher prices, if the products were of superior quality - this may be attributed to a more well-heeled clientele visiting the market. What was notable is that there was not a strong drive for organic products with 40 percent of the visitors being neutral about any preference towards organic products. Overall, there seem to be a preference for locally produced farm products. The variety of products on offer at the market, also seems to be very positive. Based on the outcome of the study, several recommendations are preferred.

First report of Neofusicoccum australe causing dieback of honeybush in the Western Cape, South Africa.

Honeybush (Cyclopia spp.) is an indigenous, leguminous member of the Cape fynbos biome growing in the coastal winter rainfall districts of the Western and Eastern Cape Provinces of South Africa (Joubert et al. 2011). Honeybush is used for the production of herbal teas and is harvested from wild-growing and cultivated plantations (du Toit et al. 1998). Very little is known regarding diseases caused by pathogens on this indigenous plant. Only one report of twig dieback on honeybush caused by several Diaporthe Nitschke species have been reported in South Africa (Smit et al. 2021). Several honeybush producers reported poor growth and dieback in their C. subternata plantations in the Western Cape Province, South Africa. Symptoms included twig dieback, branch dieback, death of branches as well as death of entire plants. In April 2008, branches from 8-year-old cultivated plants with dieback symptoms were collected in Stellenbosch. Fungal isolations were carried out from affected material as described by Van Niekerk et al. (2004) which consistently revealed the presence of a Botryosphaeriaceae species. Two isolates were grown on water agar with sterile pine needles and incubated at 25˚C using a 12-hour day/night cycle and near-ultraviolet light. Pycnidia formed after two weeks. Morphological characteristics similar to Neofusicoccum australe (Slippers, Crous & Wingfield) Crous, Slippers & Phillips were observed (Phillips et al. 2013). Conidia were hyaline, aseptate, fusiform with subtruncate bases (16.8-)18.8-22.1(-24.6) × (4.8-)5.3-6.1(-6.4) µm (n=50). Conidiogenous cells were holoblastic, hyaline and subcylindrical to flask-shaped tapering to the apex (11-15 × 2 µm) (n=10). Colonies on potato dextrose agar were light primrose turning olivaceous grey after 7 days with a light-yellow pigment diffusing into the medium. Mycelia was moderately dense with an appressed centre mat. The identity of the isolates was further confirmed by sequencing the ribosomal RNA Internal Transcribed Spacer (ITS) and the elongation factor 1-alpha (EF-1α) gene regions using primer pairs ITS4-ITS5 (White et al. 1990) and EF1-728F-EF1-986R (Alves et al. 2008), respectively. Sequences had a 100% similarity to N. australe ex-type CMW6837 isolate (accessions AY339262 and AY339270) (Slippers et al. 2004). Two isolates (STEU6554 and STEU6557) were deposited in the culture collection at the Department of Plant Pathology at Stellenbosch University and the sequences were submitted to GenBank with accession numbers ON745603, ON745604, ON746573 and ON746574. Pathogenicity tests using the two N. australe isolates were conducted by inoculating two shoots each of three field-grown C. subternata plants with a 4mm colonised potato dextrose agar (PDA) mycelium plug of each isolate on wounds made by a 4mm cork borer (Van Niekerk et al. 2004). A third shoot was inoculated with a uncolonized PDA plug as the negative control. After 12 weeks, brown-black lesions that were significantly longer (average 55.2 mm) than the uncolonized agar plug control (16.1 mm) were observed. Lesions were observed in all three plants. Neofusicoccum australe was re-isolated (van Niekerk et al. 2004) from all inoculated shoots confirming Koch's postulates. The economic impact and damages caused by N. australe as well as its incidence and severity on honeybush in South Africa is unknown. However, the pathogen caused dieback of entire branches and death of plants indicating that it could be an important pathogen of honeybush. Additionally, N. australe is one of the most important disease-causing Botryosphaeriaceae pathogens on a wide range of economical fruit and vine crops globally (Mojeremane et al. 2020). This is the first report of N. australe as a known pathogen causing decline and dieback of C. subternata in South Africa.

Gender-affirming care in South Africa: A cross-sectional survey of transgender and gender-diverse people in the Eastern and Western Cape provinces, South Africa.

Transgender and gender-diverse (TGD) people face significant discrimination in the South African (SA) health system, limiting their access to HIV services and gender-affirming care, which supports an individual's gender identity when it does not align with their sex assigned at birth. Despite the critical role of these services for TGD people, access to care remains understudied in SA. To describe TGD people and their access to and need for social, legal and medical transition, including psychosocial care, hormone therapy and surgery, as well as HIV services, in the Eastern Cape and Western Cape provinces, South Africa. A cross-sectional quantitative survey design was utilised, with 150 TGD individuals recruited via convenience sampling in the Western and Eastern Cape provinces. Interviews were conducted using structured questionnaires, with data captured on REDCap. Descriptive analysis was conducted using Stata 18. Of the 150 respondents, 74.0% were people assigned male at birth (AMAB) and 26.0% were people assigned female at birth (AFAB). Reported gender identities showed that 68.5% of AMAB respondents identified as transgender women/female, 56.4% of AFAB respondents identified as transgender men/male and 34.0% of all respondents identified as gender diverse or non-binary. Demographics showed a vulnerable population, with 18.7% with housing insecurity and 66.0% unemployed. While social transition was common (98.7%), access to legal transition (4.0%) was very low, as was access to all forms of medical gender-affirming care, with 44.7% of TGD people accessing psychosocial care, 32.0% accessing hormone therapy and 2.7% surgery. Of the respondents who had not legally transitioned, 71.4% wanted to. Most respondents who had not accessed medical gender-affirming care services expressed a need for psychosocial care (77.1%) and hormone therapy (68.6%). Gender-affirming surgery was more variable, with 33.3% of AFAB respondents wanting bottom surgery compared with top surgery (63.9%), and 49.5% of AMAB respondents wanting bottom surgery compared with top surgery (55.9%). Almost all (99.3%) respondents had had an HIV test in their lifetime, with reported HIV prevalence differing between AMAB (34.2%) and AFAB (7.9%) respondents. PrEP uptake among HIV-negative AMAB respondents was 30.4%, and 5.7% among AFAB respondents, while 78.0% of TGD people living with HIV were on antiretroviral treatment. Findings demonstrate a critical gap between needed and actual access to legal and medical gender-affirming care services. There is an urgent need for the provision of integrated and accessible gender-affirming care and HIV services as part of comprehensive care for TGD populations within inclusive health systems nationally.

Assessing differential gains that outstanding and average performing farmers attain from climate-smart cassava innovations in Nyando climate-smart villages, Kenya

In adoption of agricultural innovations, a few farmers attain outstanding outcomes above their peer majority. This reveals a positive deviance behavior in successful deployment of technologies and innovations. Assessing this behavior in climate-smart agriculture (CSA) can reveal the yield gap in triple wins of CSA (adaptation, farm productivity and mitigation). This study investigated differential gains in these CSA triple wins between outstanding (positive deviants) and average (typical) performing farmers who have adopted climate smart cassava innovations in Nyando Climate Smart Villages (CSV). In a household survey, a sample of 150 farmers were reached, which through snowballing approach, peers identified 30 to exhibit positive deviant behaviour. Presenting these in Focus Group Discussion (FGD) with stakeholders further isolated six farmers being those they consider positive deviants (PDs) in climate smart cassava innovations. Data were subjected to cross-tabulation to generate frequencies used to compute weighted index scores. This revealed increasing magnitude and was a preferred fair comparison of a sample of fewer positive deviants (n=6) with large number of typical (n=144) farmers. Results revealed substantial differences in the attained triple win gains from climate smart cassava innovations between typical and positive deviant farmers. The weighted index scores showed that positive deviant farmers had attained higher adaptability, production diversification, farm productivity and intensification, food security and were implementing more mitigation practices for climate change. This empirical evidence demonstrates potential gains from climate smart cassava innovations when deployed effectively. This is because innovative management practices distinguish positive deviant farmers from typical farmers. These typical farmers would benefit more by learning from positive deviant farmers about effective deployment of climate smart cassava innovations. The study recommends strengthening extension services linked to farmer platforms in order to grow capacity for more effective deployment of climate smart cassava innovations for realising the CSA triple wins. South Africa's coastal-breeding white-breasted cormorants: population trends, breeding season and movements, and diet

White-breasted cormorants Phalacrocorax [carbo] lucidus breed around South Africa's coast and at inland localities. Along the coasts of the Northern, Western and Eastern Cape provinces, numbers breeding were similar during the periods 1977–1981 (1 116 pairs at 41 localities) and 2008–2012 (1 280 pairs at 41 localities). Along the coast of KwaZulu-Natal (not counted in 1977–1981), 197 pairs bred at nine localities in 2008–2012, when the overall number breeding around South Africa's coastline was about 1 477 pairs. Between the two study periods, numbers decreased in the Northern and Western Cape provinces following the loss of several breeding localities, but they increased in the Eastern Cape. In the Western Cape, however, numbers were stable east of Cape Agulhas and at nine well-monitored West Coast localities that were surveyed from 1978 to 2012. White-breasted cormorants breed throughout the year, with breeding at some localities more seasonal than at others and the timing of peaks in breeding varying at and between localities. In the vicinity of Saldanha Bay/Langebaan Lagoon (Western Cape), in Algoa Bay (Eastern Cape) and in northern KwaZulu-Natal, it is likely that birds moved between breeding localities in different years, although breeding often occurred at the same locality over several years. Human disturbance, presence of predators, competition for breeding space and occurrence of breeding by other waterbirds may influence movements between colonies. Securing sufficient good habitat at which white-breasted cormorants may breed will be important for conservation of the species. The species may breed at an age of 4 years, possibly younger. The bulk of their diet around South Africa's coast consists of inshore marine and estuarine fish species that are not intensively exploited by humans.

Distribution and abundance of invasive Tamarix genotypes in South Africa

SummaryThe exotic Tamarix chinensis and T. ramosissima, believed to have been introduced into South Africa in the early 1900s to control erosion on mine dumps, are invading riparian zones and have been proven to hybridise with T. usneoides, which is native to southern Africa. In this study, we document the abundance of invasive Tamarix genotypes in South Africa. Eleven riparian zones from the Northern, Eastern and Western Cape Provinces were surveyed. Three quadrats of 600 m2 each were selected per site. Plant density, canopy cover and tree height were recorded to quantify invasiveness. Leaf samples were randomly collected from an average of eight individuals per site to record genotypes of the invaders. Tamarix density and canopy cover were significantly greater than those of co‐occurring trees and shrubs in Olifants River in De Rust (Western Cape Province). A linear correlation between percentage Tamarix spp. cover and other co‐occurring tree and shrub species showed a strong negative relationship (R2 = 0.78). Genetic analysis showed that the Western and Eastern Cape Provinces have the highest proportion of the exotic Tamarix species and their hybrids. This suggests that these two provinces require urgent management intervention to contain the spread of the weed. The distinctions made between the native and the exotic Tamarix species and their hybrids should also facilitate the testing and future release of potential biological control agents.

The use of emergency medical services for palliative situations in Western Cape Province, South Africa: A retrospective, descriptive analysis of patient records.

An estimated 56.8 million people require palliative care annually, while only 14% receive such care. This imbalance is particularly acute in low-to middle-income countries (LMICs), where up to 80% of patients requiring palliative care reside. To correct this imbalance, integration between palliative services and other disciplines has been recommended. While improved palliative care integration is a priority in the South African (SA) LMIC context, emergency medical services (EMS) and palliative care remain non-integrated. This has resulted in poor palliative situation management by EMS and a lack of research concerning their intersection. To examine EMS use for palliative situations in the Western Cape (WC) Province of SA by describing frequency of intersection, patient characteristics and outcomes. An observational, descriptive, retrospective patient record review was employed at two hospitals with palliative care services in the WC. All patient records of those who arrived at the hospitals between 1 January 2020 and 31 December 2020 via EMS conveyance leading to palliative care provision were included in the study. In total, 1 207 unique patients received palliative care services at both hospitals during the study period. Of these, 395 (33%) made use of EMS for hospital conveyance on 494 occasions. The median (range) patient age was 60 (20 - 93) years, and most transports involved male patients (54%, n=265). Family members were the primary caregivers in most instances (89%, n=440), dyspnoea was the chief complaint (36%, n=178) and cancer was the most frequent diagnosis (32%, n=159). The median length of hospital stay was 6 days, with most patients discharged home (60%, n=295). EMS in SA frequently encounter palliative situations for symptoms that may be managed within their scope of practice. Consequently, it appears that EMS have an important role to fulfil in the care of patients with palliative needs. Integrating EMS and palliative care may result in improved palliative care provision and, therefore, EMS and palliative care integration would be beneficial in SA.

Citrus black spot is absent in the Western Cape, Northern Cape and Free State Provinces

The South African citrus industry is strongly focused on exports and South Africa is a signatory member of both the World Trade Organisation Agreement on the application of Sanitary and Phytosanitary Measures and the International Plant Protection Convention. Citrus black spot, caused by Guignardia citricarpa, does not occur in all the South African citrus production areas and, therefore, South Africa has a responsibility to provide those trading partners that have identified G. citricarpa as a regulated pest with reliable information about the distribution of citrus black spot within South Africa. Detection surveys were conducted in citrus production areas in the Western Cape, Northern Cape and Free State Provinces and appropriate diagnostic protocols were used to ensure reliable detection of G. citricarpa. Trees in commercial orchards and home gardens on farms and in towns of 17, 9 and 5 magisterial districts in the Western Cape, Northern Cape and Free State Provinces, respectively, were sampled between 1995 and 2010. Fruit samples were taken during June and July, and leaf samples from November to January. None of the 3060 fruit and leaf samples collected during these surveys tested positive for G. citricarpa. Phyllosticta capitalensis, a non-pathogenic, ubiquitous, endophytic species was, however, detected during these surveys. In compliance with relevant International Standards for Phytosanitary Measures and based on the outcome of these official surveys, these three provinces in South Africa can be recognised as citrus black spot pest free areas.

Agroecological-based modeling of meteorological drought at 12-month time scale in the Western Cape Province of South Africa

To mitigate the effects of drought in water-stressed agro-ecological zones (AEZs), it is crucial to implement efficient monitoring of drought characteristics and establish proactive water-use strategies. The Western Cape (WC) province is being confronted with an escalating threat of diminished water availability due to unsustainable human activities. The objective of this study was to assess the spatio-temporal characteristics of meteorological drought within a 12-month timescale in the WC province of South Africa spanning from 1980 to 2020. The University of East Anglia's Climate Research Unit provided precipitation datasets from fifteen stations across the six AEZs of the WC province. These datasets were processed using the Standardized Precipitation Index (SPI). The Mann–Kendall (M–K) test was used to analyze the precipitation trend and SPI values at a significance level of p < 0.05. The results indicated substantial negative trends in annual precipitation across all AEZs. An examination of seasonal precipitation patterns revealed a rise in summer precipitation in subtropical cool-arid regions and subtropical warm semi-arid regions, with increases of 0.4 and 0.2 mm/year, respectively. Conversely, the overall annual precipitation trend fell by -1.0 mm/year in both AEZs. The SPI values consistently fell within the range of -2 to 0 in the subtropical warm-arid, subtropical cool-arid, and subtropical cool-arid zones. Conversely, the SPI values were consistently positive in the subtropical warm-arid, subtropical cool-humid, and subtropical warm-humid zones. This finding suggests that there will be persistent drought conditions that will affect agricultural production, surface flows in rivers, and groundwater levels across the WC province.

District hospital surgical capacity in Western Cape Province, South Africa: A cross-sectional survey.

The role of the district hospital (DH) in surgical care has been undervalued. However, decentralised surgical services at DHs have been identified as a key component of universal health coverage. Surgical capacity at DHs in Western Cape (WC) Province, South Africa, has not been described. To describe DH surgical capacity in WC and identify barriers to scaling up surgical capacity at these facilities. This was a cross-sectional survey of 33 DHs using the World Health Organization surgical situational analysis tool administered to hospital staff from June to December 2019. The survey addressed the following domains: general services and financing; service delivery and surgical volume; surgical workforce; hospital and operating theatre (OT) infrastructure, equipment and medication; and barriers to scaling up surgical care. Seven of 33 DHs (21%) did not have a functional OT. Of the 28 World Bank DH procedures, small WC DHs performed up to 22(79%) and medium/large DHs up to 26 (93%). Only medium/large DHs performed all three bellwether procedures. Five DHs (15%) had a full-time surgeon, anaesthetist or obstetrician (SAO). Of DHs without any SAO specialists, 14 (50%) had family physicians (FPs). These DHs performed more operative procedures than those without FPs (p=0.005). Lack of finances dedicated for surgical care and lack of surgical providers were the most reported barriers to providing and expanding surgical services. WC DH surgical capacity varied by hospital size. However, FPs could play an essential role in surgery at DHs with appropriate training, oversight and support from SAO specialists.Strategies to scale up surgical capacity include dedicated financial and human resources.

Application of the Standardised Streamflow Index for Hydrological Drought Monitoring in the Western Cape Province, South Africa: A Case Study in the Berg River Catchment

In many regions around the world, drought has been recurrent, more frequent, and more intense over time. Hence, scientific research on drought monitoring has become more urgent in recent years. The aim of this study was to test the applicability of the Standardised Streamflow Index (SSI) for hydrological drought monitoring in the Berg River catchment (BRC), Western Cape (WC) province, South Africa (SA). Using various methods described in this study, the sensitivity of the SSI to the commonly used Gamma, Log-normal, Log-logistic, Pearson Type III, and Weibull Probability Distribution Functions (PDFs) was tested. This study has found that all the tested PDFs produced comparable results for mild to severe drought conditions. The SSI calculated using the Gamma, Log-Normal, and Weibull PDFs is recommended for the BRC because it consistently identified extreme drought conditions during the 1990–2022 study period and identified the 2015–2018 droughts as the worst during the study period. Although more studies are required to test other PDFs not considered, this study has shown that the SSI can be applicable in the BRC. This study has provided a foundation for more research on the application of the SSI in the BRC and other catchments in SA.

Organic Food Certification in South Africa: A Private Sector Mechanism in Need of State Regulation?

Organic production targets the development of a sustainable cultivation system and a variety of high-quality products with emphasis on environmental protection, biodiversity and high standards of animal protection. In South Africa, the organic sector pioneered private practices and systems in small informal groups to guide the public and private sectors on environmental and sustainability issues. A private certification system for organic products is applicable in the country consisting of network certification and third-party certification in collaboration with foreign and locally-based certification organisations. Local producers also use self-declaratory vendor claims associated with organic labels. A State auditor mechanism is nonetheless applicable with respect to the use of the term “free range” on labels for meat products. South African National Standards (SANS 1369) on Organic Agricultural Production and Processing (OAPP) have been drafted by the South African Bureau of Standards (SABS) but the final version has not yet been made public. There is presently no specific legislation on organic products in the country but draft regulations (under the Agricultural Product Standards Act) on the control and sale of organic products which have not yet been promulgated. With mainly a private organic food certification system, to what extent do South African organic food products respect rules of organic production? This paper looks into the organic food regulation in South Africa and examines how far this private sector mechanism for organic food certification is in need of State regulation.

The impact of family physician supply on district health system performance, clinical processes and clinical outcomes in the Western Cape Province, South Africa (2011-2014).

BackgroundA revised family physician (FP) training programme was introduced in South Africa in 2007. A baseline assessment (2011) of the impact of FP supply on district health system performance was performed within the Western Cape Province, South Africa. The impact of an increased FP supply within this province required re-evaluation.AimTo assess the impact of FP supply on indicators of district health system performance, clinical processes and clinical outcomes in the Western Cape Province. The objectives were to determine the impact of FPs, nurses, medical officers (MOs) and other specialists.SettingThe study sample included all five rural districts and eight urban subdistricts of the Western Cape Province.MethodsA secondary analysis was performed on routinely collected data from the Western Cape Department of Health from 01 March 2011 until 30 April 2014.ResultsThe FP supply did not significantly impact the indicators analysed. The supply of nurses and MOs had an impact on some of the indicators analysed.ConclusionThis study did not replicate the positive associations between an increase in FP supply and improved health indicators, as described previously for high-income country settings. The impact of FP supply on clinical processes, health system performance and outcome indicators in the Western Cape Province was not statistically significant. A predominantly HIV type 1 subtype C-restricted epidemic in South African urban populations.

395 SOUTH AFRICA is now reaching the advanced stages of the HIV-1 epidemic, and in 1997 more than 16% of pregnant women attending government clinics were HIV seropositive.1 There is geographical variation in the timing and severity of the epidemic within South Africa, with the northeastern region of the country (KwaZulu-Natal Province) having a prevalence of 26.92% compared with the most southern province (Western Cape), having a prevalence of only 6.29%. In a study in Cape Town, located in Western Cape Province, subtype B was associated with homosexual transmission and subtype C with heterosexual transmission.2 On the basis of the age of the epidemic and the genetic distance between gag sequences, this study suggested multiple introductions of subtype C strains into this region of South Africa. HIV subtype C has also been identified in mine workers from the north central Gauteng Province of South Africa,3 as well as in women of Durban (KwaZulu-Natal Province).4 Owing to differences in the timing of the epidemic in different regions of the country, ethnic diversity within South Africa, as well as the presence of emigrants and refugees from other African countries, it is possible that other subtypes may have been independently introduced into different regions of the country. To assess the distribution of HIV-1 subtypes in South Africa, blood samples were obtained from 107 infected individuals who originated from five major cities situated in four geographically distinct regions of the country. HIV-1 provirus was subtyped by heteroduplex mobility assay (n 5 107); seven of these samples were sequenced in the V3±V5 region and the complete gp120 was sequenced for one subtype C isolate. In addition, to assess the relationship among HIV-1 subtype C sequences from the southern African subcontinent, these sequences were compared phylogenetically with 140 published V3 (env) sequences, 105 of which were subtype C sequences originating from the southern African region. EDTA blood was obtained from a total of 107 infected individuals attending urban clinics located in four provinces of South Africa: Johannesburg (n 5 34) and Pretoria (n 5 5) (Gauteng Province); Durban (n 5 20) (KwaZulu-Natal Province); Bloemfontein (n 5 28) (Free State Province); and Cape Town (n 5 20) (Western Cape Province) (see Table 1). Samples were collected between 1994 and 1996. DNA was extracted, the 700-bp V3±V5 region was amplified by nested polymerase chain reaction (PCR), and PCR fragments were subtyped by heteroduplex mobility assays (HMA) as described.2 The 700-bp V3±V5 fragment of seven samples, and the complete gp120 region of the env gene (1600 bp) from one of these samples (ZA347TS), were cloned and sequenced as described.2
