Abstract

Hyperreactio luteinalis (HL) is characterized by bilaterally marked cystic enlargement of the ovaries containing multiple benign theca lutein cysts. HL is rarely observed during a spontaneously conceived normal singleton pregnancy. We present a case of a 29-year-old woman who was initially detected with markedly enlarged multicystic ovaries in the 9 weeks of a spontaneously conceived pregnancy. Ultrasonographic follow-up of the ovaries were done in order to avoid unnecessary exploratory laparotomy during pregnancy. At 40 weeks of gestation, a healthy female infant weighing 2,910 g was delivered vaginally. Since normal spontaneous remission occurs after delivery or during pregnancy in many cases, conservative management is appropriate rather than performing unnecessary intervention. We hereby report a rare case of HL managed non-surgically resulting in successful vaginal delivery.
