A minimally invasive adrenalectomy for metastatic melanoma with hemorrhage is a procedure not commonly performed. However, adrenal metastasis is present in 50% of Stage IV melanoma. The robotics trained urologists can provide minimally invasive procedure for Stage IV melanoma. Our goal is to provide a methodical and reproducible template for performing a minimally invasive, adrenalectomy and metastatectomy for a suspected melanoma metastasis. METHODS We present a robot-assisted laparoscopic left adrenalectomy, highlighting the key steps to performing the procedure safely and efficiently. We provide narration and bulleted slides to present the case. RESULTS We perform the procedure without intraoperative or postoperative complication. The final pathology revealed necrotic melanoma involving the adrenal. The procedure was completed in three and a half hours. The patient was discharged on postoperative day two, and had resolution of preoperative nausea caused from gastric compression by the mass.
