Abstract
前床突起は蝶形骨縁の内側端に位置し，外側からは蝶形骨縁，尾側からは視束管の一部であるoptic strutに支えられている．内頚動脈瘤頚部クリッピング術において前床突起削除は，内頚動脈近位部の確保や広い術野を得るために有用な手技である．微小解剖学的知識に基づいた段階的前床突起削除を行うことで，視神経や内頚動脈への熱損傷および機械的損傷を回避することができる．本稿では，われわれが行っている傍鞍部動脈瘤の頚部クリッピングにおける硬膜外前床突起の段階的削除法について紹介する．段階的前床突起削除は傍鞍部内頚動脈瘤の頚部クリッピングにおいて，合併症を回避するだけでなく教育的観点からも有用である．
