‘The voice of an oracle’: Little Nell and the art of ‘enacting the persona of the doctor’

Medical humanities have long been promoted as a means for trainees and doctors to gain deeper understanding of patients, illness and professionalism. In practice, teaching on medical humanities – while valuable –struggles to connect such learning to the realities of the clinical workplace and patient encounters. The solution to this shortcoming lies in recognising ‘soft’, but clinically relevant, concepts that operate within consultations. These concepts are, in fact, best revealed through literary and historic accounts of doctors, patients and illness. Such a vignette is Dickens’ parody of Victorian physicians in the shape of the doctor who visits Little Nell in his book the Old Curiosity Shop. By taking prescription and technology out of the encounter, Dicken’s parody lays bare ‘soft’ elements that constitute the art of ‘enacting the persona of the doctor’. Medical humanities in general (and literature in particular) constitute a valuable tool for teaching the place of performance and the power of human qualities in the consultation. This ‘old fashioned’ professionalism should be a foundation to every formation in clinical medicine.

Effect of advanced platelet-rich fibrin on accelerating alveolar bone formation in dogs: a histological and immunofluorescence evaluation.

Several methods have been developed to regenerate lost alveolar bone. Platelet-rich fibrin (PRF) is a useful adjunct for new bone formation in dentistry. To elucidate the effect of advanced PRF (A-PRF) on bone formation, we inserted A-PRF clots in sockets after tooth extraction. Premolars were extracted from beagle dogs, and A-PRF was applied to the socket. New bone formation was assessed using histological and immunofluorescence examinations, and the bone formation ratio was evaluated 14 and 30days postoperatively. Histological examination revealed newly formed bone filling the sockets up to the center in the A-PRF group at 14days postoperatively, while thick and regular bone trabeculae were arranged in porous bone after 30days. Higher expressions of osteocalcin and osteopontin were observed in newly formed bone in the A-PRF group, compared to the control group. The bone formation ratio was also higher in the A-PRF group than in the control group. Thus, A-PRF application may result in enhanced new bone formation and may aid in accelerating bone formation. A-PRF was more rapid than a self-limiting process during induction of bone formation by enhancing osteoblast activity and may be useful for bone formation in clinical medicine.

Abstract. Introduction. On the 190th jubilee of Sergei Petrovich Botkin, an appeal to his memory of his merits is evidence of the caring attitude of the current generation to the Russian medical heritage, allowing to preserve the continuity of professional generations. Aim. The purpose of the study is to show the role of the personality of S.P. Botkin in the development of scientific clinical medicine, in the formation of military medicine field and sanitary medicine in Russia. In the course of theoretical research, significant personal qualities of S.P. Botkin, his clinical experience, the influence of research results on the development of the physiological direction of clinical medicine, the formation of Russian military field medicine and the military sanitary industry in Russia were revealed. Material and methods. The research was carried out based on of theoretical analysis of literature on the history of Russian medicine and medical education, letters of S.P. Botkin. The historical observation covers the period from the beginning of the 30s up to the 90s years of the XIX century. Results and discussion. The study of articles and materials illustrating the life path of S.P. Botkin, his achievements in the field of the physiological direction of scientific clinical medicine, military field and military sanitary medicine, allowed us to identify a number of individual and personal qualities of this person, which as a whole determined his professional formation and influenced the entire course of development of a number of branches of scientific and medical knowledge. Purposefulness, originality, charisma of the scientist, his perseverance and persistence, courage and creative passion have become significant personality traits that allowed his inclinations as a researcher, doctor, teacher and public figure to be revealed. Conclusion. In the course of theoretical research, it was possible to identify a number of significant personal qualities of S.P. Botkin, analyze his clinical experience, research, pedagogical and social activities, which made it possible to highly appreciate his contribution to the world medical science and practice, to the development of domestic public health, to the improvement of the military sanitary industry in Russia.

“Pirogov's Legacy: Past, Present, Future” – Ten Years After the Anniversary International Conference (to the 210th Anniversary of the Birth of N.I. Pirogov)

The article is dedicated to the anniversary date, the 210th anniversary of the birth of the genius of Russian medicine, doctor and surgeon, anatomist and teacher, an outstanding citizen and ardent patriot of Russia – Nikolai Ivanovich Pirogov. On November 25, 2010, on the day of the 200th anniversary of the scientist, a memorial plaque was installed at the Department of Operative Surgery of the Academy, made in the Urals at the expense of cadets and being the only material monument in our country that reminds of this event. The article describes thelife path and stages of scientific activity of N.I. Pirogov, his contribution to science and the unconditional significance of his work for the development of clinical medicine and morphology. The international scientific conference “Pirogov's Legacy: Past, Present, Future”, held in November 2010, was a major event in the scientific life of many department teams of the Military Medical Academy and became a memorable part of the anniversary scientific events of St. Petersburg dedicated to the great scientist and organized by the Pirogov Surgical Society and the medical community of the city. This article presents the scientific works of N.I. Pirogov, which can rightfully be considered fundamental in the formation of modern morphology and medicine.

Em "Resposta a uma questão", artigo-réplica a perguntas encaminhadas a Michel Foucault pela revista Esprit, um parágrafo concernente às relações entre a constituição da medicina clínica e a Revolução Francesa nos parece, ainda hoje, paradigmático do modo de pensar foucaultiano. O presente ensaio dele parte, no intuito de distinguir a perspectiva foucaultiana daquelas ligadas à História das Mentalidades e à Sociologia do Conhecimento. Em seguida, usa-o como ferramenta analítica de uma recente polêmica, relativa a uma investigação que se propõe a mapear o cérebro de "adolescentes infratores". Avalia-se que o parágrafo mencionado faculta entender de modo singular o repúdio de uma parcela da intelectualidade e da militância ao projeto de pesquisa em pauta. Com isso, a "Resposta a uma questão" se amplia a uma resposta a muitas questões, especialmente ao que se pode entender por defesa dos Direitos Humanos no campo dos saberes e regimes de verdade.

The article is devoted to the study of the ethical traditions of domestic medicine and medical education in the development of the theory and practice of time resource management in the healthcare system since the formation of domestic clinical medicine, in the conditions of zemstvo medicine, the Soviet and post-Soviet periods. On the one hand, the actively developing processes of digitalization of the domestic healthcare system open up wide opportunities in the diagnosis, treatment and prevention of diseases, on the other hand, they give rise to a number of unprecedented problems, in particular, in the field of ethics. Their solution requires trained specialists with interdisciplinary humanitarian knowledge and is associated with the need to humanize modern domestic higher medical education, one of the most important problems of which is the reduction of humanitarian training and, as a result, the loss of educational function. The wide scale of the development of technical capabilities, the variety of pharmaceuticals, experiments in the organization of health care have created a situation where, behind an array of various innovations in the pursuit of rational use of time in the time management of the health care system, a person with his unique subjective characteristics has receded into the background.

The Clerkship Directors in Internal Medicine (CDIM) is devoted to teaching medical students and represents the allopathic medical schools in the United States and Canada. As CDIM has grown, its focus has broadened beyond the core 3rd-year clerkship in internal medicine to embrace internists who are responsible for their schools' introduction to clinical medicine, multidisciplinary clerkships, and 4th-year courses.CDIM held its 2004 National Meeting in Nashville, Tennessee, from October 14 to October 16, as part of Academic Internal Medicine Week. Themes addressed during the meeting included developing and rewarding excellent medical educators, revitalizing of the 4th year of medical school, and assessing the state of medical education research. The combined Alliance for Academic Internal Medicine (AAIM) format allowed participants to share the expertise of educators at multiple levels of the medical education continuum. Some areas of joint interest included projections on the future of medical education, the effect of residency work-hour restrictions on the entire medical education process, advising students on the residency application process, and mechanisms to revitalize interest in internal medicine. The discussions between clerkship directors, residency program directors, fellowship directors, departmental chairmen, division chiefs, and department administrators were generally felt to strengthen the meeting.s were selected for presentation by the CDIM Program Planning Committee, which worked in conjunction with the AAIM Program Planning Committee. Those submitted for the following publication were felt to demonstrate high quality and originality, to reflect the themes of the meeting, and to be of general interest to readers of Teaching and Learning in Medicine. The presentation by G. Denton, K. DeZee, and S. Durning, "Medical Student Resource Use and Knowledge Acquisition in the Medicine Clerkship," which suggests that use of a standard textbook benefited students with a weak fund of knowledge more than use of electronic sources, is published in this issue of TLM.
