Abstract

The case of a child with cystinosis in metabolic imbalance is presented in detail. This patient manifested multiple ectopic beats and serious atrioventricular and intraventricular conduction disturbances during intravenous administration of 5% glucose in 0.45% sodium chloride solution. The authors attribute these alarming manifestations to hypopotassemia. The conduction disturbances observed in this case have not been described in patients with hypopotassemia, but they have been frequently seen in experimental animals. The authors suggest that the sudden death of children with cystinosis during intravenous administration of glucose is caused by cardiac effects of potassium deficiency, possibly of the type presented in this case.
