Abstract

Background: Left atrial (LA) enlargement with mitral regurgitation (MR) is well documented. Our aim was to determine the phasic LA volumes with grades of valvular MR. Method: 48 patients with valvular MR, graded semiquantitatively as mild and moderate/severe MR were compared with 90 healthy controls. Maximum, minimum and pre-p LA biplane volumes were indexed to BSA and conduit, passive and active emptying LA volumes were calculated. Results: There was a significant increase in maximum, minimum and pre-p volumes with increasing severity of MR. Passive emptying increased with moderate/severe MR. There was a decrease in conduit volumes when compared with normals with no change in active emptying volumes, but a reduction in the active emptying fraction. The LA ejection fraction (LAEF) was lower than in normals. Conclusion: Despite an overall increase in LAV, there is a decrease in LAEF and atrial emptying fraction indicating LA contractile dysfunction with MR. Normal
