Abstract

Objective To compare the clinical effect between 20 G and 23 G vitrectomy in the treatment of vitreous hemorrhage. Methods Thirty eyes of 30 cases with vitreous hemorrhage who underwent 23 G microincision vitrectomy were enrolled in this study as group A, other 40 eyes of 40 cases underwent ordinary 20 G vitrectomy were enrolled as group B. The records were retrospectively analyzed. The observed items included duration of building three channels and closing the incision, the best corrected visual acuity, intraocular pressure, intraoperative and postoperative complications and healing of scleral puncture. Results The mean duration of building three channels was (122.22±25.37)s in group A and(320.55±70.43)s in group B. The mean duration of closing the incision was (120.33±47.46)s in group A and (346.55±58.88)s in group B. The differences between two groups was statistically significant(t=15.480, P=0.007; t=16.520, P=0.009). Compared with the preoperative visual acuity , the postoperative ones of group A and B were improved. The differences were statistically significant(χ2=8.650, P=0.024; χ2=10.400, P=0.037). There was no significant difference in postoperative visual acuity between the two groups(χ2=0.580, P=0.160). The average intraocular pressure was (14.07±2.37) mmHg in group A and (16.00±2.39)mmHg in group B at 1 day after surgery. There was significantly different in that at 1 d after surgery (t=3.360, P=0.008), while there was no significant difference in that at 3 d or 7 d after surgery between the two groups(t=0.806, P=0.180; t=1.550, P=0.080). In the two groups, there was no case of iatrogenic hole or other serious complication occurred. The last follow-up results after three months showed no vitreous hemorrhage or retinal detachment occurred. Conclusion The clinical efficacy of 20 G and 23 G vitrectomy in the treatment of vitreous hemorrhage is similar, and 23 G shows a better advantage in less time and little trauma.
