Abstract
This case study is written for Family Health Practitioners and Nurse Case managers. To add, reflective critical analysis, frameworks, exploration of advanced role of case manager, ethical and professional issues which contribute enhancing the health and care needs, reflection on roles and responsibilities in relation to interdisciplinary teams is discussed in the report. It is to revitalize health systems in under-developed countries based on the tenets of the Alma Ata Declaration of Health Care, presented by the World Health Organization remains essential for attaining the maternal and child health related Millennium Development Goals (MGDs). Ehiri [1] informed that MGDs have a target date of 2015 and synthesize many of the commitments made in achieving that target.
