Reply from the Authors: We appreciate the interest of Drs. Lee and Brazis in our study results. All 26 of the patients with severe optic disc edema, retinal or disk hemorrhage, or macular exudates had no brain MRI lesions, and, as noted by Drs. Lee and Brazis, none of them developed CDMS within 5 years of optic neuritis. Excluding these patients from the CDMS risk assessment increases the life-table estimate of the 5-year risk of CDMS from 30% to 32% in the full cohort and from 16% to 19% among patients with no brain MRI lesions at the time of optic neuritis. We concur with Drs. Lee and Brazis's suggestion that when any of these ophthalmoscopic features are associated with optic neuritis in the presence of normal brain MRI, the etiology of the optic neuritis almost always, if not always, has an etiology …
