Abstract
Surgical treatment of peripheral nerve injuries is indicated in patients with open injuries of peripheral nerves and in patients who suffered closed injury and nerve laesion where even after three months patients didn't achive functional recovery. Primary neuroraphy is performed in the first 48th hours, after that till 18th days delayed primary neuroraphy is performed, and after three to six weeks early secundary neuroraphy is carried out. In sharp lacerations primary neuroraphy is indicated, or delayed primary neuroraphy. In blunt lacerations optimal timing for surgery is three to six weeks after injury. In gunshot wounds and in closed peripheral nerve injuries after three months, delayed secundary nerve reparation is indicated. In traction injuries of brachial plexus we advise delaye surgery for at least two months, after necessery diagnostic procedures, that will enable establishing the right level of brachial plexus injury. Nerve transplantatio using free nerve graft is the method of choice in peripheral nerve injuries where nerve defect is no longer than 2 cm. Nerve transplantation is secundar procedure, after treating tetive, bone and skin injuries.
