Abstract

Objective To investigate the current situation of community nursing for the elderly of Shaoxing city, and the evaluation of different groups of community nursing system, to provide references for construction of nursing mode of the health aging. Methods The methods of literature review and expert argumentation were used to produce a questionnaire of elders' health condition and needs. Stratified sampling method was used to do on-site survey for the elderly, community medical service center and nursing staffs. Results The age between 70~79 and 80~89 had a high prevalence of disease about the elderly health status, and the prevalence rate of chronic diseases from high to low were hypertension, hyperlipidemia, diabetes, chronic lung disease, coronary heart disease, cerebrovascular disease and tumor.The elderly accepted community nursing service's situation: for the measurement of blood pressure was 82.46%; the regular physical examination was 68.47%; dispensing was 58.58%; health lecture 46.82%; injection/infusion was 34.33%; in the investigation of the elderly's community nursing demands, the regular physical examination was 69.78%; nursing of chronic disease was 56.34%; lifestyle guide was 43.28%; home visit was 38.06%, home was 36.19%; in the investigation of the demand for the aged, in according to the mode of the elderly nursing, the most reasonable model was integration continuous management model of disease care, health management, Daily care . Conclusions The community resource's utilization degree is low, and the continuity of community nursing model could meet the demand of the health of the elders, and could accelerate the community nursing service in elderly individuals, groups and social healthy aging.
