  • Congratulations to the authors of this work! Our remarks and suggestions concern the following points: It is stated in the article that Mozambique and Portugal have Universal Health Coverage (UHC): what is included in this UHC? long-term care? total coverage? It would be interesting to study direct patient payments, the OOP could be one of the important factors that would explain this cost difference between these countries

  • We all know that the OOP, especially in low- and middle-income countries, is often high and patients are often not able to afford them, which influences treatment decisions. It would be interesting for the authors to underline the limitations of their study: First, with regard to generalization, the reality of Mozambique might not be the reality of the rest of SubSaharan Africa, it might be better or worse elsewhere

  • It would be interesting to enrich the discussion by comparisons with the results of studies carried out in other countries, both developed and developing
