In modern medical practice, there is an ever-increasing rise in litigation. A strong emphasis is placed on good record keeping as an integral part of patient care and the clinician legal defence. This commentary describes the potential flaws and vulnerabilities in the radiologist record keeping and the potential implications for patient care.
