Abstract

A patient evaluation and data retrieval system for peripheral vascular disease has been designed to insure accurate, consistent, quantitative recording of information at each patient visit and to permit ready retrieval and processing of such data. Duplicating NCR forms have been prepared in such manner that data can be transferred readily to IBM cards. The cards include information on past history, present history, physical findings, surgery and therapy, digital skin temperatures, digital plethysmographic studies, digital pulse contours, calculated data, arteriographic findings, and diagnosis. When information on parameters of a disease state, or effectiveness of a drug or surgical procedure is desired, the information can be retrieved readily from the cards with the use of a card sorter, collator, and rapid print-out or on a computer with the use of an appropriate program.
