Pott's puffy tumour with eyelid abscess

Abstract

A 33-year-old male with untreated sinusitis suffered right eyelid trauma and presented with an upper eyelid abscess (Fig. 1A). Computed tomography revealed a 2.7 × 5.2 cm rim-enhancing collection in bilateral frontal sinuses eroding into the right orbit (Fig. 1C, D, arrows) and complete opacification of bilateral maxillary sinuses (Fig. 1D, asterisks). A diagnosis of Pott's puffy tumour was made. The patient received bedside incision and drainage but declined intravenous antibiotics. On 3-day follow-up, there was improved swelling and associated ocular symptoms (Fig. 1B). Pott's puffy tumour is a rare, life-threatening complication of acute sinusitis that should remain on the differential diagnosis in patients with untreated sinusitis accompanied by periorbital swelling and pain.
