Abstract
In 2014, Children's Hospital of Michigan began an initiative to involve the Pediatric Palliative Care Team in to the standard initial work up of all patients undergoing evaluation of the heart transplant team. Since that time, over 30 patients have been evaluated with a complete initial consult by the palliative care team, as well as ongoing care throughout their course, regardless of outcome.
