Abstract

A 28-year-old nulligravida woman complained about pelvic pain and intense dysmenorrhea since two years. Ultrasound revealed an 8-centimeter right ovarian mass, which had the characteristics of an endometrioma. Microlaparoscopy was performed with 3-millimeter instruments and low pressure pneumoperitoneum (7 mmHg). Pelvic exploration confirmed the right ovarian endometrioma. First, total adhesiolysis of the pelvic area was performed. Ovarian cystectomy was achieved, without the need to increase the pressure. Major advantage of low pressure is decrease of shoulder postoperative pain. The patient had no complication, and was discharged the same day following surgery. She was totally asymptomatic three months after surgery.
