Abstract

Development and origin of the new treatment proceduresAccording to the statistics of the“Deutsche Gesetzliche Unfallversicherung“ (DGUV, German statutory accident insurance) only few particularly severe injuries result in very high remuneration costs. The DGUV introduced the severe injury type procedure (SAV) for these cases. They should be concentrated in highly specialized accident clinics with the best therapy possibilities in order to keep the costs as low as possible.Severe injury type procedureFor accident insurance consultants (D-Arzt) involved in the SAV, qualifications in pediatric traumatology are also required in addition to the prerequisites for participation in the injury type procedure (VAV). The obligatory transfer regulations from VAV to SAV in accordance with the injury type catalog are new and are also valid if an application for participation in the SAV has already been made and also the possibilities for sanctions if these are ignored as well as the obligation for continuous further training.ConclusionThe expenditure on structural organization and manpower for standby duties without a planned improvement in profits is high, so that a future concentration in hospitals specialized in accident care in population dense areas is expected. The comprehensive high quality care is therefore endangered. Financial incentives could help to ensure short rescue and transport times nationwide and to optimize cooperation at the clinical and outpatient levels.
