Millet-based probiotic beverages: a comprehensive review of nutritional potential and applications in extended and challenging environments

Purpose The rising interest in functional foods has brought millet-based probiotic beverages to the forefront as sustainable nondairy alternatives for extended and challenging environments. This study aims to examine the nutritional benefits, probiotic compatibility and practical applications in long-term preservation settings, such as space missions, military deployments and remote expeditions, while also addressing their potential use for athletes, travelers and health-conscious consumers. Design/methodology/approach A comprehensive search using keywords such as “millet-based probiotics” and “functional beverages for long voyages” was conducted across Web of Science, Scopus, PubMed and Google Scholar. This search yielded a database of 163 papers. After applying inclusion and exclusion criteria, 19 publications met the review’s requirements. An additional set of supporting literature was used to contextualize the findings, resulting in a total of 117 references. Findings Millets are nutrient-rich and possess prebiotic properties that support beneficial gut microbiota. Their fermentation compatibility makes them ideal for functional beverage development, particularly for vegan and lactose-intolerant populations. Incorporating probiotics into millet-based beverages can alleviate gastrointestinal issues, nutrient loss and immune suppression caused by prolonged travel. The worldwide market for plant-based drinks is expected to reach US$26bn in the next five years, indicating an increasing demand for dairy substitutes. Innovations in formulation and bioactivity enhancement are addressing these gaps, positioning millet-based beverages as a viable option. Originality/value This review highlights the importance of millet-based probiotic beverages in promoting dietary health and sustainability, not only for long-duration missions but also for broader consumer markets seeking plant-based, shelf-stable dietary alternatives.

Patient and general public attitudes towards clinical artificial intelligence: a mixed methods systematic review

Artificial intelligence (AI) promises to change health care, with some studies showing proof of concept of a provider-level performance in various medical specialties. However, there are many barriers to implementing AI, including patient acceptance and understanding of AI. Patients' attitudes toward AI are not well understood. We systematically reviewed the literature on patient and general public attitudes toward clinical AI (either hypothetical or realised), including quantitative, qualitative, and mixed methods original research articles. We searched biomedical and computational databases from Jan 1, 2000, to Sept 28, 2020, and screened 2590 articles, 23 of which met our inclusion criteria. Studies were heterogeneous regarding the study population, study design, and the field and type of AI under study. Six (26%) studies assessed currently available or soon-to-be available AI tools, whereas 17 (74%) assessed hypothetical or broadly defined AI. The quality of the methods of these studies was mixed, with a frequent issue of selection bias. Overall, patients and the general public conveyed positive attitudes toward AI but had many reservations and preferred human supervision. We summarise our findings in six themes: AI concept, AI acceptability, AI relationship with humans, AI development and implementation, AI strengths and benefits, and AI weaknesses and risks. We suggest guidance for future studies, with the goal of supporting the safe, equitable, and patient-centred implementation of clinical AI.

To evaluate the accuracy of several methods aimed to detect Helicobacter pylori stool antigens in patients with upper gastrointestinal bleeding. Thirty-four patients with upper gastrointestinal bleeding because of peptic ulcer were included. The first stool sample during hospitalization was collected, and stool antigens were determined with: polyclonal enzyme-linked immunosorbent assay (Premier-Platinum-HpSA); monoclonal enzyme-linked immunosorbent assay (Amplified-IDEIA-HpStAR); and rapid monoclonal immunochromatographic test (ImmunoCard-STAT HpSA). A patient was considered infected when H. pylori was diagnosed with invasive tests (rapid urease test or histology) or with (13)C-urea breath test. When all tests were negative, a new breath test was repeated after stopping proton pump inhibitors. All patients were infected and, therefore, only sensitivity of the tests could be calculated: polyclonal enzyme-linked immunosorbent assay (74%), monoclonal enzyme-linked immunosorbent assay (94%), and rapid monoclonal immunochromatographic test (60%; concordance between the two observers was high, kappa = 0.9). Neither the presence of maelena nor the delay in obtaining stool samples explained false negatives. Neither the polyclonal enzyme-linked immunosorbent assay stool antigen test nor the rapid immunochromatographic stool antigen test can be recommended to diagnose H. pylori infection in patients with upper gastrointestinal bleeding. However, the monoclonal enzyme-linked immunosorbent assay stool antigen test is highly sensitive for detecting the infection in patients with this complication, although more studies are necessary to evaluate the specificity of the method.
