Abstract

Objectives: Many heart failure (HF) hospitalizations are due to patients (pts) inability to afford medications or poor understanding of self-care plans. The aim of this study was to evaluate whether intensive education with provision of medicines to under-insured or low-income HF pts would reduce hospitalizations and healthcare costs associated with decompensated HF. Methods: Thirty-eight pts contracted to meet with a PharmD monthly for medications, counseling, and assessment of adherence to prescribed medical regimen that was provided free of charge.
