Mapping the research landscape on chronic pain and electroencephalography (EEG): a bibliometric analysis

Chronic pain is a complex and debilitating condition that affects an estimated 1.5 billion individuals worldwide. The impact of chronic pain extends beyond the individual, with significant socioeconomic consequences, including health care costs and decreased performance. Today, electroencephalography (EEG) has become a valuable non-invasive tool in the study of chronic pain, allowing researchers to measure and analyse the brain’s electrical activity in response to pain stimuli. Thus, this bibliometric analysis evaluated the literature on chronic pain and EEG, identified main themes, authors, institutions, author keywords, and publications in the field and assessed the research impact and influence in this study area from 1972 to 2023. First, datasets were obtained from Web of Science (WoS) and Scopus, then analysed using ScientoPy and VOSviewer software. There has been a steady increase in the literature on chronic pain and EEG since 1972. In 2021, a significant number of publications (n = 69) were in WoS. Furthermore, “Neuroscience & Neurology” was the most popular subject matter, with 388 publications. Meanwhile, the top five author keywords associated with this subject were “chronic pain”, “EEG”, “fibromyalgia”, “spinal cord injury”, and “neurofeedback”. The term “machine learning” has garnered significant attention in recent years, particularly in 2022 and 2023. In summary, the trend in chronic pain and EEG research has consistently shown a rise in scholarly interest. These study findings can guide future research efforts, policy-making, and practical measures in diagnosing and managing chronic pain, which can improve patients' well-being and quality of life.

Many studies report on the incidence of chronic pain. However, deficiencies exist in prior research making it difficult to generalize results to trauma patients. This study evaluated the incidence of chronic pain in trauma patients at 4 months posttrauma and effect chronic pain has on life interference. The incidence of chronic pain was present in 79.2% of trauma patients 4 months posttrauma and a strong positive correlation (n = 80, r = 0.79, P < 0.001) existed between chronic pain severity and the effect on life interference. Chronic pain is prevalent and causes significant life interference in traumatically injured patients.

The quality of the evidence examining physical activity and exercise for chronic pain is low. This is largely due to small sample sizes and potentially underpowered studies. A number of studies had adequately long interventions, but planned follow-up was limited to less than one year in all but six reviews.There were some favourable effects in reduction in pain severity and improved physical function, though these were mostly of small-to-moderate effect, and were not consistent across the reviews. There were variable effects for psychological function and quality of life.The available evidence suggests physical activity and exercise is an intervention with few adverse events that may improve pain severity and physical function, and consequent quality of life. However, further research is required and should focus on increasing participant numbers, including participants with a broader spectrum of pain severity, and lengthening both the intervention itself, and the follow-up period.

We had high or moderate confidence in the evidence contributing to most review findings. Further research, especially into families' experiences of treatments and services, could strengthen the evidence for low or very low confidence findings. Future research should also explore families' experiences in low- to middle-income contexts; of pain treatments including opioid use in children, which remains controversial; and of social care services. We need development and testing of family-centred interventions and services acceptable to families. Future trials of children's chronic non-cancer pain interventions should include family-centred outcomes.

Spinal Cord Injury Pain: The Influence of Psychologic Factors and Impact on Quality of Life

To examine chronic pain prevalence in a spinal cord injury (SCI) population, and to determine the influence of psychologic factors on SCI pain and impact of SCI pain on quality of life. Five hundred seventy-five persons with SCI were asked to participate in the study. Demographic, SCI, and pain characteristics were obtained. The Chronic Pain Grade, anger items of the Profile of Mood States, Illness Cognition Questionnaire, Pain Coping and Cognition List, and Patient Health Questionnaire were used. General health and well-being were assessed with 0-10 scales. The influence of psychologic factors was assessed with regression analyses controlling for person and injury characteristics and pain intensity. Response rate was 49%. SCI pain prevalence was high (77.1%). More internal pain control and coping, less catastrophizing, higher level of lesion, and nontraumatic SCI cause were associated with less pain intensity. More pain was associated with higher pain-related disability. Lower catastrophizing was related to better health. Less SCI helplessness and catastrophizing, greater SCI acceptance and lower anger levels were related to higher well-being. Higher levels of SCI helplessness, catastrophizing, and anger were related to higher depression levels. Pain intensity showed no independent relationships with health, well-being, and depression in the regression analyses. Chronic SCI pain and quality of life were both largely associated with several psychologic factors of which pain catastrophizing and SCI helplessness were most important. Psychologic intervention programs may be useful for persons suffering from chronic SCI pain to improve their quality of life.

The effectiveness of peer support interventions for community-dwelling adults with chronic non-cancer pain: a systematic review

Background Chronic non-cancer pain is a common condition that by its nature requires ongoing self management. Peer support is a method of facilitating self-management of chronic conditions that has been shown to be effective. Peer support may be useful in overcoming some of the common barriers to chronic pain self-management, and if successful could reduce the health service burden of this common chronic condition. Objectives To identify the effectiveness of peer support interventions on physical and emotional function, pain, quality of life, self-efficacy and health service utilization in community dwelling adults with chronic non-cancer pain, compared to those receiving usual care or other self-management interventions. Inclusion criteria Types of participants Community-dwelling adults with chronic (12 weeks or longer) non-cancer pain. Types of intervention Any peer support intervention, or any intervention that included a defined element of peer support, delivered to groups or individuals in any format, including face-to-face, telephone and internet-based interventions. Types of studies Randomized controlled trials or, in their absence, other quantitative research designs such as non-randomized controlled trials and before and after studies. Types of outcomes Physical and emotional function, pain, quality of life, self-efficacy and health service utilization. Due to the lack of standardization of chronic pain outcome measures, any relevant measure of these five domains was considered. Search strategy This review aimed to find both published and unpublished studies using the following three-step search strategy: (i) An initial limited search of MEDLINE and CINAHL was undertaken using four initial keywords. (ii) All keywords identified from the initial search were utilized in a search of several databases. (iii) The reference lists of all identified reports and articles were searched for additional studies. Data collection and synthesis Two independent reviewers assessed 4078 studies for potential applicability to the review objectives based on the information provided in the title, abstract and subject headings. Thirty-six full-text articles were obtained and were likewise appraised for applicability to the review objectives. Twelve studies underwent critical appraisal by the two independent reviewers for methodological validity using standardized critical appraisal instruments from the Joanna Briggs Institute Meta Analysis of Statistics Assessment and Review Instrument (JBI-MAStARI). Five studies were included in the review. Data were extracted from the five studies using the standardized data extraction tool from JBI-MAStARI. There was a high degree of heterogeneity in terms of populations, interventions, outcome measures and comparators, rendering meta-analysis inappropriate; results were therefore presented in narrative form. Results There is some evidence that peer support may have a beneficial effect on emotional function, pain, quality of life and self-efficacy in specific subgroups of chronic pain patients; no evidence was found on the effect of peer support on health service utilization. Conclusions There is some evidence that peer support interventions may be more effective than usual care for some groups of patients with chronic non-cancer pain. However, it is not possible to make specific recommendations for practice until further high-quality research is conducted.

Machine Learning in Chronic Pain Research: A Scoping Review

Given the high prevalence and associated cost of chronic pain, it has a significant impact on individuals and society. Improvements in the treatment and management of chronic pain may increase patients’ quality of life and reduce societal costs. In this paper, we evaluate state-of-the-art machine learning approaches in chronic pain research. A literature search was conducted using the PubMed, IEEE Xplore, and the Association of Computing Machinery (ACM) Digital Library databases. Relevant studies were identified by screening titles and abstracts for keywords related to chronic pain and machine learning, followed by analysing full texts. Two hundred and eighty-seven publications were identified in the literature search. In total, fifty-three papers on chronic pain research and machine learning were reviewed. The review showed that while many studies have emphasised machine learning-based classification for the diagnosis of chronic pain, far less attention has been paid to the treatment and management of chronic pain. More research is needed on machine learning approaches to the treatment, rehabilitation, and self-management of chronic pain. As with other chronic conditions, patient involvement and self-management are crucial. In order to achieve this, patients with chronic pain need digital tools that can help them make decisions about their own treatment and care.

Employment Outcomes of Persons With a Mental Disorder and Comorbid Chronic Pain

OBJECTIVE: We sought to examine the independent and interactive effects of common mental health disorders and chronic pain conditions on employment and work outcomes among individuals younger than 65 years of age. METHODS: We analyzed cross-sectional data from the second wave of Healthcare for Communities (HCC2), a household telephone survey of U.S. civilian adults conducted in 2000 to 2001 (N=5328). Common mental disorders were assessed using the short-form versions of the World Health Organization’s Composite International Diagnostic Interview (CIDI-SF). Chronic pain conditions and labor market outcomes were identified by self report. Logistic and linear regression analysis was used to provide estimates for work impairment based on the presence of a mental health disorder and/or a chronic pain condition. RESULTS: The interaction between presence of a mental health disorder and presence of a chronic pain condition was significantly associated with no work for pay in the past 12 months (OR 2.3 [1.2–4.2]) and number of days of work missed in the past month due to health (regression coefficient 1.5 [0.6]). In stratified analyses, this effect persisted in women but not men. The presence of a mental health disorder or chronic pain condition were each independently associated with limitations in work and any work missed in the past 30 days due to health, although the interaction was not significant. CONCLUSION: Mental health disorders and chronic pain are each associated with work disability. Mental health disorders are more highly associated with some work disability outcomes when accompanied by chronic pain, especially in women.

Are Mind-Body Exercise Beneficial for Treating Pain, Function, and Quality of Life in Middle-Aged and Old People With Chronic Pain? A Systematic Review and Meta-Analysis.

BackgroundAging is a significant risk factor in chronic pain development with extensive disability and greater health care costs. Mind-body exercise (MBE) has been scientifically proven to affect the pain intensity and physical health.ObjectivesTo assess the effects of MBE modes (Tai Chi, yoga, and qigong) for treating chronic pain among middle-aged and old people, compared with nonactive and active treatment, as well as function, quality of life, and adverse events.MethodsWe searched PubMed, Embase, Web of Science, Cochrane Library, China National Knowledge Infrastructure (CNKI), Wanfang Database, and Chinese Scientific Journals Full-Text Database (VIP) till March 2022. No restrictions were chartered within the year and language of publication. We included randomized controlled trials of MBE treatment in middle-aged and elderly people with chronic pain. The overall certainty of evidence was evaluated by using the GRADE approach.ResultsA total of 17 studies (n = 1,332) were included in this review. There was low-certainty evidence indicating that MBE had a moderate effect on reducing pain compared with the nonactive and active control group (standard mean difference (SMD): −0.64, 95% confidence interval (CI): −0.86 to −0.42, P < 0.001). Very-low-certainty evidence showed that the pooled SMD for the functional improvement was −0.75 (95% CI: −1.13 to −0.37, P < 0.001). Low-certainty evidence presented that no influence was observed in physical component summary (SMD: 0.23, 95% CI: −0.16 to 0.62, P = 0.24) and mental component summary (SMD: −0.01, 95% CI −0.39 to 0.36, P = 0.95).ConclusionOur results indicated that MBE was an effective treatment for reducing symptoms of middle-aged and elderly people with chronic pain compared with nonactive and active control groups. TC and qigong had obvious benefits for knee osteoarthritis in self-reported function, but the efficacy of chronic low back pain was uncertain. No significant benefit of MBE on quality of life in older adults with chronic pain was found. More high-quality RCTs should be conducted to explore the efficacy and mechanism of MBE on chronic pain in middle-aged and elderly people from various dimensions, such as affective and cognitive dimensions.Systematic Review Registrationhttps://www.crd.york.ac.uk/PROSPERO/display_record.php?RecordID=316591, identifier CRD42022316591.
