Abstract

A 56-year-old man was struck by a falling metal beam and suffered severe blunt injuries to the head, chest, and lower extremities. He was confused, and his blood pressure was 111/59 mm Hg, pulse rate 103 beats/min, and respiratory rate 28 breaths/min. Ultrasonography showed a barcode sign over the left chest but a lack of an identifiable lung point (Figure 1). Following chest radiography, the respiratory rate increased to 36 breaths/minute (Figure 2). The emergency physician did a tube thoracostomy for his left pneumothorax, and the respiratory rate improved to 20 breaths/minute.
