Abstract

A 45-year-old man presented with 9 months of intermittent tremors, diaphoresis, oral paresthesia, and visual changes that were relieved upon eating. Over time, these symptoms became more frequent and refractory to dietary changes, and he developed onset of right upper quadrant abdominal pain. His serum glucose level was found to be in the 50s during a symptomatic episode. Physical examination and laboratory testing were remarkable for right upper quadrant tenderness, and an alkaline phosphatase level of 103 IU/L (normal 20-90 IU/L), aspartate aminotransferase of 43 IU/L (8-40 IU/L), and alanine aminotransferase of 62 IU/L (7-46 IU/L); prothombin time and serum albumin levels were normal.
