Abstract
背景．Remitting seronegative symmetrical synovitis with pitting edema（RS3PE）症候群は腫瘍随伴症候群の一つとして知られているが，肺癌に合併した報告は稀である．RS3PE症候群様症状で発症し,外科的切除を施行することで症状軽快を認めた肺癌の1例につき報告する．症例．79歳，男性．2009年12月頃から両下肢と足背の浮腫を認め，次第に手指と両手関節の腫脹，手関節と足関節の熱感，疼痛を自覚した．2010年4月に検診の胸部X線写真で左中肺野に腫瘤影を指摘され当院紹介となった．胸部CT画像では左S4に40×37 mmの腫瘤を認め，気管支鏡検査にて肺癌と診断された．原発性肺癌に伴うRS3PE症候群の圧痕浮腫を伴った関節炎と診断し，左上葉切除を施行した．術直後より関節症状の速やかな改善を認め，術後8か月経過し無再発生存中である．結論．高齢者に急速に進行する浮腫を伴った関節炎では悪性腫瘍の合併も念頭におき，全身精査を行う必要があると考えられた．
