Abstract
症例は28歳，男性．下痢，下腹部痛，発熱を主訴に当院受診．下腹部全体に圧痛，腹膜刺激徴候あり，腹部CTにて骨盤内に周囲脂肪織濃度の上昇を伴った嚢胞性腫瘤を認めた．その際，虫垂の同定は困難であり，穿孔性虫垂炎に伴う腹腔内膿瘍疑いにて緊急手術を施行した．術中，回腸末端より約50cm口側の小腸間膜内に約15cmの炎症を伴った嚢胞性腫瘤を認めた．同部を含めて約20cmにわたって小腸切除術を施行した．嚢胞部を穿刺したところ，白色混濁膿性排液を認め，培養検査にてPasteurella canisが検出された．病理結果では，小腸が腸間膜内に連続性に進展して嚢胞状に拡張しており，嚢状重複小腸と診断された．重複腸管症は消化管のいずれの部位にも発生する稀な先天的消化管奇形である．また，Pasteurella canisは犬猫の口腔内常在菌であり，これまでに腹腔内感染の報告はない．文献的考察を加えて報告する．
