Abstract
Costa Rica has a long history of provision of psychiatric care. In recent years proponents of reform of mental health care have successfully moved decentralization of psychiatric services, development of community mental health services, prevention, and rehabilitation to the forefront. Nonetheless, obstacles to implementation of proposed reforms persist. Among these obstacles are a lack of communication between the communities served by the health system and that system, unequal geographic distribution of providers of mental health care, and primary-care providers' level of training in mental health care. In this paper we shall first trace the evolution of the system of care for the mentally ill in Costa Rica and then describe the current system and its limitations and strengths.
