In vitro analysis of pulp volume in primary teeth using CBCT and periapical radiographs: diagnostic implications for osteogenesis imperfecta

Osteogenesis imperfecta (OI), often associated with dentinogenesis imperfecta type I (DI-I), leads to structural alterations such as early pulp obliteration in primary teeth. This study evaluated the diagnostic utility of cone-beam computed tomography (CBCT) and periapical radiographs (PXR) in assessing pulp chamber volume and morphology in primary molars of children with OI, with and without DI. An observational in vitro case–control study was conducted on primary molars from children with OI (DI and non-DI groups) and healthy controls. Dental dimensions and pulpal measurements were obtained from PXR and CBCT. Three-dimensional volumetric reconstructions were generated using 3D Slicer to quantify pulp chamber volume. Statistical analyses included ANOVA, Kruskal–Wallis tests, intraclass correlation coefficients (ICC), and generalized linear models adjusted for tooth type, sex, and age. Sixty-six molars were included: 20 with DI, 23 without DI, and 23 controls. Molars from OI patients with DI showed significantly reduced pulp volume compared with both OI without DI and controls (p < 0.001). Type III OI was associated with the lowest pulp volumes, independently of age, sex, and tooth type (p = 0.003). CBCT measurements demonstrated superior agreement with anatomical dimensions compared with PXR (ICC up to 0.99). Three-dimensional reconstructions revealed dysmorphic and partially obliterated pulp chambers in DI-affected molars. Primary molars of children with OI and DI present significant pulp volume reduction and morphological alterations, particularly in severe OI phenotypes. CBCT provides reliable quantitative assessment and may serve as a complementary diagnostic tool for identifying DI-related pulpal changes. • Patients with OI and DI often present structural alterations in the pulp chamber of primary teeth. • CBCT, as a minimally invasive technique, helps assess pulp chamber morphology in OI patients. • This study shows reduced pulp chamber volume in primary molars of OI types III–IV compared with type I and controls.

The accuracy of age estimation from pulp chamber/crown volume ratio of canines obtained by cone beam computed tomography images: an Egyptian study

BackgroundGlobally, the need for an accurate and valid method for age estimation in adults still exists. The aging process is associated with secondary dentine deposition that reduces the volume of teeth pulp. Therefore, dental age could be recognized from the volume of pulp cavity. The aim of this study was to assess the accuracy and validity of pulp chamber/crown volume ratio of maxillary and mandibular canines in estimating age using cone beam computed tomography (CBCT) images in a sample of the Egyptian population.ResultsThere were significant strong negative correlations between age and each of the maxillary pulp chamber volume (PCV), mandibular PCV, maxillary pulp chamber/crown volume (PCV/CV) ratio, and mandibular PCV/CV ratio (p < 0.001). Furthermore, no significant differences were detected between both sexes regarding the mean maxillary and mandibular PCV and PCV/CV ratios (p > 0.05). The best fit regression model for age prediction was as follows: age (years) = 70.21 − 784.0x maxillary PCV/CV ratio − 1.66x maxillary PCV. The proposed model showed good power of prediction (R2 adjusted = 0.951). Additionally, the model was validated on an independent sample of 100 CBCT images with a root mean squared error (RMSE) of 2.86 years.ConclusionThe obtained valid regression formula in this study can serve as a reliable tool for age estimation in Egyptians. This formula should be further validated on a larger sample size of the Egyptian population that considers more steady age distribution.

Cone Beam Computed Tomography Evaluation of Socket Healing After Third Molar Germ Extraction: A Case Series Study Including Adolescents with Osteogenesis Imperfecta Type I Treated with Bisphosphonates and Healthy Age-Matched Subjects

Bisphophonates (BPs) are widely used in Osteogenesis imperfecta (OI). Cone Beam Computed Tomography (CBCT) shows clinical usefulness in evaluating impacted teeth and adjacent structure relationships, extraction socket healing, bone mineral density (BMD) and BP-related jaw osteonecrosis (BRONJ). The aim of the study was to compare alveolar sockets and the adjacent bone area before and after third molar extraction in OI type I (OI-I) adolescents treated with BPs and age-matched healthy subjects (HSs) by CBCT. Methods: Five adolescents with genetically proven OI-I treated with BPs (three males and two females, mean age: 15.2 ± 1.78 years) and four age-matched healthy subjects (two males and two females, mean age: 15.5 ± 1.29 years) were included in this study. Eight Regions of Interest (ROIs) were evaluated: between 3.7 and 3.8 (ROI-1) and 4.7 and 4.8 (ROI-2); after 3.8 (ROI-3) and 4.8 (ROI-4); alveolar sockets 3.8 (ROI-5) and 4.8 (ROI-6); left (ROI-7) and right (ROI-8) cortical bone. Results: ROIs were evaluated on both sides of the mandible for all the subjects except one OI patient in which CBCTs were performed pre- and-post third molar extraction only on the right side. CBCT was performed 12.8 ± 4.60 and 11.5 ± 2.51 days before and 8.0 ± 1.41 and 7.7 ± 0.5 months after extraction in OI-I and HSs, respectively. BPs were discontinued 62.0 ± 36.5 months before extraction. None of the OI-I adolescents developed BRONJ. Statistically significant greater values were observed in OI-I for ROI-1 and -2 (p = 0.0464), ROI-3 and -4 (p = 0.0037) and ROI-7 and -8 (p = 0.0079) after extraction. Conclusions: This descriptive study confirms that, in OI-I adolescents treated with BPs, third molar extraction is safe, and socket healing occurs properly. In addition, it demonstrates that, if the same device and imaging conditions are used and comparisons to predetermined standard values are avoided, CBCT can be used to monitor BMD changes. The significant greater BMD observed for different ROIs in OI-I could reflect the increased secondary mineralization related to the BP-dependent reduction in bone turnover.

Dental age estimation using cone-beam computed tomography: A systematic review and meta-analysis.

This systematic review aimed to investigate the correlation between chronological age and dental pulp volume in cone-beam computed tomography (CBCT). The literature was searched in 4 databases (PubMed, Scopus, Web of Science, and Google Scholar). Within each study, the outcome of interest was the correlation (r) between chronological age and pulp volume. A random-effect meta-analysis was conducted. Subgroup analysis was carried out according to sex and tooth type. Of 5693 identified studies, 27 fulfilled the inclusion criteria and were selected for meta-analysis. These articles focused on single-rooted teeth (n=21), multi-rooted teeth (n=6), maxillary teeth (n=14), mandibular teeth (n=6), and maxillary and mandibular teeth (n=12). The relationship between chronological age and dental pulp volume was examined in the entire population (r=-0.67), men (r=-0.75), and women (r=-0.77) in single- and multi-rooted teeth. The results of the total population analysis showed a relatively strong negative relationship between age and pulp volume. This study suggested that CBCT is a reliable and repeatable tool for dental age estimation. A strong inverse relationship was observed between pulp chamber volume and age. Further studies on the correlation between chronological age and pulp volume of multi-rooted teeth may be beneficial.

Health-related quality of life in children with osteogenesis imperfecta: a large-sample study.

Osteogenesis imperfecta (OI) is a hereditary disease characterized by multiple fragility fractures and progressive skeletal deformities. No detailed investigations about the quality of life (QoL) have been carried out in a large sample of patients with OI. We evaluated the QoL and its influencing factors in a large and well-characterized OI cohort. We used a validated questionnaire of PedsQL 4.0 to evaluate the health-related quality of life (HRQoL) of children and adolescents with OI. We compared HRQoL among patients with OI types I, III, and IV. The relationship between HRQoL and pathogenic mutations in candidate OI genes was investigated. We also evaluated the influencing factors of HRQoL in OI patients. A total of 138 children with OI and 138 healthy controls were enrolled in this study. The HRQoL scores of OI patients were 64.4 ± 30.0, 71.9 ± 22.2, 75.7 ± 24.8, 63.7 ± 24.5, and 68.9 ± 22.0 in physical, emotional, social, school functioning, and total score, respectively, which were significantly lower than those of healthy children (86.5 ± 12.7, 83.3 ± 16.0, 92.1 ± 11.8, 87.5 ± 11.8, and 87.3 ± 10.7, all p < 0.01). Moderate and severe OI (type III/IV) patients had poorer HRQoL scores than patients with mild OI (type I). Gene mutations inducing qualitative defects in type I collagen led to worse HRQoL scores than those with quantitative defects in type I collagen, except in emotional functioning. The total HRQoL score was positively correlated with family income, lumbar, and femoral bone mineral density (BMD) Z-scores and negatively correlated with disease severity and fracture frequency. HRQoL was significantly impaired in OI patients, and patients with more severe OI had poorer HRQoL scores. For the first time, we found that children with qualitative defects in type I collagen had poorer HRQoL scores than those with quantitative defects in type I collagen.

A potential novel technique for measurement of pulp volume on periapical radiography: A pilot study

Pulp volume can be assessed during dental treatment. Three‐dimensional imaging techniques are not routinely used for this purpose because of high radiation doses. This study aimed to develop a novel method to measure pulp volume using periapical radiography. In this study, cone‐beam computed tomography (CBCT) was used as a reference method. Periapical radiography and CBCTs obtained from the same patients (n = 32) were recorded. Pulp volume was determined by observing the density differences between the pulp and peripheral structures using ImageJ. A method of graph and volume calculation was developed for each tooth. The Shapiro–Wilk test and Mann–Whitney U test were used to show normality and non‐normal distributions. The Bland–Altman plot was used to show the scattering of the mean versus difference values of the measurements of the two methods used to calculate the pulp volume. Normality was evaluated using the Shapiro–Wilk test. CBCT measurements are normally distributed (p = 0.307), while ImageJ is not normally distributed (p = 0.027). Therefore, the mean difference between the two groups was analyzed using the nonparametric Mann–Whitney U test. There was a statistically significant difference between the CBCT and ImageJ measurements (p = 0.01). According to Spearman's correlation analysis, the results obtained from the novel method were moderately correlated with those obtained from the reference method (r = 0.444). The results of this study indicated that a novel method‐based Java software can be used to calculate pulp volume using low‐dose radiation containing periapical radiography.

Chronological age estimation by measuring pulp chamber volume in teeth with open apices: A CBCT analysis.

In the domain of forensic medicine, the estimation of age is a critical aspect of human identification, including that of adolescents. The study aimed to examine the relationship between the pulp chamber volume of teeth with open apices and the chronological age of adolescents from the Turkish population. The study was conducted by examining cone beam computed tomography (CBCT) images of 51 pediatric patients who visited the Department of Pedodontics for routine dental examinations. All CBCT images were captured using a NewTom 5G unit (QR, Verona, Italy). The measurements were recorded in DICOM format using the SimPlant Pro 16 software (Materialise NV, Leuven, Belgium). A moderate and positive significant correlation was observed between the age of the premolar group patients and impacted pulp volume (IMPV). The increase in volume measurements of the patients in the premolar group indicates that their age is significantly higher (r = 0.561, p = 0.030). A moderate and negative significant relationship was identified between the age of the premolar group patients and erupted pulp volume (EPV). The increase in the EPV measurements of the patients in the premolar group indicates that their age is significantly lower (r = -0.491, p = 0.041). The pulp chamber volume of premolars, which narrows due to secondary dentin deposition, served as the physical marker for chronological age estimation of adolescents in the Turkish population. The measurement of the mature and immature premolar pulp volume could be used for age estimation, particularly in the context of CBCT analysis.

A rare case of osteogenesis imperfecta combined with complete tooth loss

Osteogenesis imperfecta (OI) is a heritable disorder of the connective tissue characterized by blue sclerae, osteoporosis and bone fragility. Dentinogenesis imperfecta type I is commonly seen in OI patients, but other dental impairments, such as tooth agenesis or complete tooth loss, are rarely reported for these patients. Here, we report the case of a 37-year-old female Chinese OI patient who experienced complete tooth loss before puberty. The patient has a family history of OI and her father has a history of tooth loss. She showed obvious OI phenotypes, including a dwarfed stature, blue sclerae, scoliosis, pigeon chest and a history of fractures. Tooth loss began at the age of 6 years and continued until complete tooth loss at 20 years; this occurred in the absence of dental decay, gum disease, accidents or drug usage. Radiological studies revealed osteoporosis of the lower limbs and an underdeveloped scapula. Type I collagen gene analysis identified a known c.2314G>A (p.Gly772Ser) substitution in the COL1A2 gene, which we suggest affects the interaction between type I collagen and extracellular matrix proteins, including cartilage oligomeric matrix protein, phosphophoryn and SPARC (secreted protein acidic and rich in cysteine). In silico prediction indicated a relatively mild effect of the mutation, so it is conceivable that the severity of the clinical phenotype may result from additional mutations in candidate genes responsible for abnormal dental phenotypes in this family. To our knowledge, this is the first report of an OI patient with a phenotype of complete tooth loss at a young age.

The vitamin D status in a Chinese osteogenesis imperfecta population and its correlation with bone metabolic markers and bone density.

Studies on the baseline vitamin D levels in osteogenesis imperfecta (OI) patients before medication are scarce. This study assessed the vitamin D status of a population with OI at both the overall level and within different age groups. It correlated baseline 25-hydroxyvitamin D (25(OH)D) levels with other bone-related factors, biochemical markers, and bone density. We collected 25(OH)D levels from 95 OI patients in East China (59 under 18 years old and 36 over 18 years old). Postmenopausal women and men over 50 years old are excluded. Measurements included body indicators, biochemical markers, and bone mineral density (BMD) assessed by Dual-energy X-ray absorptiometry (DXA). Data analysis was performed using SPSS 26.0. In the overall population, among those under 18 years old, and among those over 18 years old, 87.4, 83.1, and 94.4%, respectively, were vitamin D deficient (<30 ng/mL), while 47.4, 40.7, and 58.3% had vitamin D deficiency (<20 ng/mL), respectively. In the overall population and among those under 18 years old, serum 25(OH)D levels were negatively correlated with age and parathyroid hormone (PTH) levels, and 25(OH)D levels (<10 ng/mL, 10-20 ng/mL, 20-30 ng/mL, >30 ng/mL) showed a negative correlation with BMI. In OI patients under 18 years old, serum 25(OH)D was negatively correlated with serum β-CTX levels. In adult male OI population, 25(OH)D levels were negatively correlated with OI severity (Type I, IV, III). No statistically significant correlation was found between 25(OH)D levels and BMD Z-scores. This study on OI in East China reveals significant vitamin D insufficiency and deficiency in baseline levels among pediatric, adolescent and adult OI patients. It assesses the correlation of 25(OH)D levels with various influencing factors, providing crucial insights into understanding the impact of OI on vitamin D status across different age groups and aiding in better clinical management of OI patients.

Age estimation based on 3D pulp chamber segmentation of first molars from cone-beam-computed tomography by integrated deep learning and level set.

To develop an automatic segmentation method to segment the pulp chamber of first molars from 3D cone-beam-computed tomography (CBCT) images, and to estimate ages by calculated pulp volumes. Patients with CBCT scans were retrospectively identified. The age estimation was formulated as CBCT image segmentation using a coarse-to-fine strategy by integrated deep learning (DL) and level set (LS), followed by establishing a linear regression model. On the training data, DL model was trained for coarse segmentation. The validation set was to determine the optimal DL model, and a LS method established on it was to refine the coarse segmentation. On the testing data, the integrated DL and LS method was applied for pulp chamber segmentation, followed by volume calculation and age estimation. Statistical analysis was performed by Wilcoxon rank sum test to demonstrate gender difference in pulp chamber volume, and volume difference between maxillary and mandibular molars. Wilcoxon signed-rank test was adopted to compare true and estimated ages. A total of 180 CBCT studies were randomly divided into 37/10/133 patients for training, validation, and testing data, respectively. In the training and validation sets, the results showed high spatial overlaps between manual and automatic segmentation (dice = 87.8%). For the testing set, the estimated human ages were not significantly different with true human age (p= 0.57), with a correlation coefficient r= 0.74. An integrated DL and LS method was able to segment pulp chamber of first molars from 3D CBCT images, and the derived pulp chamber volumes could effectively estimate the human ages.

Comparison of the impact of orthodontic treatment on pulp volume in adolescents and adults

Objective: To compare the impact of orthodontic treatment on pulp volume in adolescents and adults. Methods: Cone-beam CT data of 62 patients undergoing orthodontic treatment at the Department of Orthodontics, Stomatological Hospital of Chongqing Medical University, from January 2019 to March 2022 were collected. Patients were divided into two age groups (31 patients in each group): adolescent group (aged 13-17, 17 males and 14 females) and adult group (aged 21-25, 12 males and 19 females). Pre-and post-treatment reconstructions of the pulp and dental tissues of upper first molars (UM1) and lower central incisors (L1) were performed. Measurements included pulp volume for UM1 (UM1 P) and L1 (L1 P), pulp chamber volume (UM1 PC) and root canal volume (UM1 RC) for UM1, root length for L1 (L1 RL), and mesiobuccal root length for UM1 (UM1 RL), as well as chamber heights at specific landmarks [the lengths from the central fossa fusion site to the roof of the pulp chamber (H1), the floor of the pulp chamber (H2), the nearest point of root divergence as well as crown-root bifurcation (H3), the farthest point of root divergence (H4), and the pulp chamber height (H5)] in UM1. Changes in these indices were calculated and analyzed using paired and independent sample t-tests for within-group and between-group differences, respectively. Pearson correlation was used to assess potential associations among H5, root length, and pulp volume changes. Results: Before and after orthodontic treatment, no significant difference was observed in the adult group for L1 P (t=-0.03, P=0.975), while significant differences were noted for UM1 P, UM1 PC, and UM1 RC (t=9.98, P<0.001; t=9.04, P<0.001; t=6.69, P<0.001). In the adolescent group, significant differences were found for both L1 P and UM1 P (t=2.25, P=0.029; t=6.30, P<0.001). After orthodontic treatment, the absolute value changes of UM1 P, UM1 PC, and L1 P in the adolescent group were (19.75±9.58), (15.07±7.65) and (1.89±6.29) mm3, respectively, and in the adult group were (13.33±9.41), (9.16±7.05) and (0.02±4.66) mm3, respectively (t=3.77, P<0.001; t=4.48, P<0.001; t=2.34, P=0.048). There was no significant absolute difference in the amount of UM1 RC between the two groups after orthodontic treatment (t=0.86, P=0.391). Before and after orthodontic treatment, the absolute value changes of L1 RL, H1 and H5 in the adolescent group were (0.54±0.41), (0.38±0.27) and (0.71±0.33) mm, respectively, and the absolute value changes in the adult group were (0.78±0.62), (0.26±0.20) and (0.57±0.28) mm, respectively (t=-2.43, P=0.017; t=2.96, P=0.004; t=2.57, P=0.011). Whereas no significant differences were observed for UM1 RL, H2, H3, and H4 (t=-0.85, P=0.400; t=0.43, P=0.669; t=-0.50, P=0.619; t=1.46, P=0.148). Additionally, significant correlations were found between changes in H5 and UM1 RL with UM1 P (r=0.35, P<0.001; r=0.19, P=0.030), but not between Changes in L1 RL and L1 P (r=0.11, P>0.05). Conclusions: The effect of orthodontic treatment on pulp volume in adolescents and adults were different.

Zoledronic acid in children with osteogenesis imperfecta and Bruck syndrome: a 2-year prospective observational study.

Treatment with zoledronic acid (ZA) over 2 years, among 33 children with osteogenesis imperfecta (OI) and five Bruck syndrome cases, showed reduction in fracture rates, pain, and improvement in bone mineral density (BMD) and motor milestones of development. This is the first study reporting the use of bisphosphonates in patients with Bruck syndrome (BS). OI and BS are genetic disorders that result in bone fragility and reduced BMD. There is little literature describing the efficacy and safety of ZA in this population. In this study, we assess the response to treatment with ZA at six monthly intervals in Egyptian children with OI and BS for a period of 2 years. Thirty-three patients with OI and five patients with BS were treated with 0.1 mg/kg ZA intravenously every 6 months for 2 years during which they were followed up using different parameters. A clinical severity score (CSS) was applied to the patients before and 2 years after the start of therapy. Comparison of disease severity and response to ZA treatment between autosomal-dominant (AD) and autosomal-recessive (AR) OI patients was also done. After 6 months of treatment, OI and BS patients showed a significant increase in BMD Z-scores (P < 0.003 in the spine and P < 0.004 in the hip), together with a significant drop in fracture rate (P < 0.001), relief of pain (P < 0.001), and improvement in ambulation (P < 0.001). CSS was significantly reduced after 2 years of treatment in both OI and BS patients. AR-OI patients were more severely affected than AD-OI patients and showed more significant improvement. Zoledronic acid proved to be safe and effective in the treatment of OI and BS. The biannual infusion protocol was convenient to patients. There was a positive correlation between disease severity and benefits of the treatment. The use of the CSS proved to be of value in the assessment of the degree of severity in OI, and with some modifications, it was a valuable tool for the assessment of response to treatment.

Assessment of dysplastic dentin in osteogenesis imperfecta and dentinogenesis imperfecta

Two semiquantitative scoring systems, Clinical Radiographic Score (CRS) and Dysplastic Dentin Score (DDS), were introduced for analyzing degree of dysplastic manifestations in dentin. The utility of both systems was demonstrated in a large material of teeth from patients with dentinogenesis imperfecta (DI) and osteogenesis imperfecta (OI). Twenty teeth from healthy controls, 81 teeth from 40 patients with OI, and 18 teeth with DI without OI (DI type II) were examined. The degree of dysplasia was correlated with type and form of OI and type of DI. The median DDS did not differ between DI associated with OI (DI type I) and DI type II. DDS in OI patients without clinical signs of DI was above that of control teeth. Both circumpulpal and mantle dentin showed increased DDS, although circumpulpal dentin was more severely affected. The median DDS was highest for the most severe type of non-lethal OI (type III). DDS increased significantly with form (severity) of OI. A significant association between DDS and CRS was found, although diagnosis of DI in less severe cases was not possible based on radiographic or clinical signs alone. Thus, the DDS system proved valuable when the CRS system based on radiographic/clinical manifestations failed, the most significant finding being subclinical histological manifestations of DI in patients with OI but without clinical or radiographic signs of DI. These subtle dysplastic changes are most likely an expression of genetic disturbances associated with OI and should not be diagnosed as DI, but rather be termed histologic manifestations of dysplastic dentin associated with OI.
