Abstract

In the otherwise excellent report by Fouran and Chicoine1 of fatal accidental imipramine (Tofranil) ingestion, an interesting earlier reference is not mentioned. In 1968 Doctor Robert Penny described a 3½-year-old who ingested 650 mg of imipramine and survived.2 The electrocardiographic changes in the two case reports are identical, but the abnormalities described by Penny responded dramatically to an infusion of potassium chloride, a treatment not mentioned by the authors, although they do briefly discuss potassium shifts.1
