Abstract
A 61-year-old male presented with 20/20 vision and a lesion on his left eye, unchanging since childhood (Fig. 1). A giant pigmented mass with cystic spaces in bulbar conjunctiva with pseudopterygium-type extension was noted on the cornea. Pathology result was compatible with giant benign conjunctival nevus. Conjunctival nevi are the most common melanocytic conjunctival tumours. Giant nevi, defined as >10 mm, represent 5% of cases. Nevi usually exhibit intralesional cystic spaces (also visible on ultrasound), a characteristic finding that is helpful in differentiating them from other worrisome lesions. The overall risk of malignant transformation is 1%–3%, so continued vigilance is warranted.
