Abstract
Summary. Carcinoma of the skin from Merkel cells is characterized by an aggressive course and a high incidence of relapses and metastases. Due to the rare occurrence, diagnostic errors and inadequate treatment are often tolerated. Among the priority tasks are early diagnosis of Merkel’s carcinoma, correct prognostic evaluation and adequate individual treatment of patients.

Summary
В пользу этого факта служит преимущественная локализация опухоли на коже наиболее интенсивно инсолируемых областей: у больных псориазом, имеющих в анамнезе многочисленные курсы терапии (Psoralen + UltraVioletA–УФА терапия + фотосенсибилизатор) вероятность развития рака из клеток Меркеля повышается в 100 раз. Вследствие длительного бессимптомного течения первичной опухоли первыми проявлениями рака из клеток Меркеля могут быть регионарная лимфаденопатия — у 27 % больных, или отдалённые метастазы — у 7 %. При проведении лучевой терапии как на ложе удалённой опухоли, так и на зоны регионарного метастазирования после радикального хирургического лечения, значительно снижается число местных рецидивов [17]. Анализ результатов комбинированной терапии (хирургическое лечение + ЛТ + ХТ) больных ККМ с высоким риском прогрессирования (рецидив после проведенного лечения, поражение лимфоузлов, невыполненная лимфаденэктомия) при краткосрочном наблюдении выявил снижение частоты рецидивов и повышение выживаемости. Лечение больных ККМ должно быть индивидуализированным и учитывать все прогностические факторы и включать все существующие методы
