Abstract

Sub-ILM haemorrhages have been described in association with a wide spectrum of clinical conditions. The most frequent are Valsalva retinopathy and Terson’s syndrome. We present here a case of a 40-year-old man with sudden visual loss and wide macular haemorrhage. After excluding all other possible causes, we diagnosed Valsalva retinopathy, a rare form of haemorrhagic retinopathy due to a sudden increase in intrathoracic pressure with transmission to the intraocular venous pressure and rupture of retinal capillaries, in particular in the perifoveal region.
