Purpose:The purpose of this study was to evaluate the treatment efficacy of vitrectomy combined with subretinal recombinant tissue plasminogen activator (r-tPA) and factors affecting visual improvement in patients with submacular hemorrhage (SMH) due to neovascular age-related macular degeneration (nAMD).Materials and Methods:Medical records of 17 consecutive patients diagnosed with SMH secondary to nAMD were retrospectively reviewed. The initial surgical procedure involved a 23-gauge transconjunctival vitrectomy, subretinal r-tPA application through a self-sealing inferior retinotomy, and sulfur hexafluoride gas for tamponade in all patients. The duration, size, and thickness of the hemorrhage and the pre- and post-operative visual acuity (VA) using a Snellen chart were recorded. VA was converted to logMAR for statistical analysis. Results:The average duration and size of the SMH were 12.8 ± 18.2 days and 8.6 ± 5.3 disc areas, respectively. The mean follow-up time was 16.9 ± 4.7 months. A statistically significant visual improvement was found when comparing initial VA with postoperative best-corrected VA (BCVA) and final BCVA (Wilcoxon rank test, P ≤ 0.01). There was no significant correlation between the size of the hemorrhage and postoperative BCVA and final BCVA (Spearman's rho test). There was no statistically significant correlation between the initial VA and postoperative BCVA and final BCVA (Spearman's rho test). There was no significant correlation between the duration of hemorrhage and postoperative BCVA and final BCVA (Spearman's rho test). The preoperative thickness of hemorrhage (747.5 ± 30 μm) was not correlated with postoperative BCVA or final BCVA (Pearson's test). Conclusions:Vitrectomy combined with subretinal r-tPA injection and gas tamponade is an effective surgical intervention to preserve VA in selected patients with apparent SMH.

Background: The high birth rate is the main reason for the need for family planning services. Data at the Semen Health Center shows low interest in IUD compared to other contraceptives, this can be caused by several existing factors. This study aims to determine the relationship between the level of education and knowledge of acceptors with the use of IUD (Intrauterine Device) contraceptives at the UPTD Semen Health Center, Kediri Regency East Java Indonesia. Method: This study is a quantitative study with a Cross-Sectional research plan. The sampling technique used in this study was Purposive Sampling with a sample size of 100 KB acceptor respondents in Semen District, Kediri Regency. This study was conducted in April 2024. Data collection used a questionnaire instrument. Data were analyzed using the Spearman's rho test. Result: Secondary education level (SLTA/SMK) (51%) and basic education level (SD-SMP) (43%). High knowledge, namely 83 respondents (83%). Half of the birth control acceptors are non-IUD users (74%). The results of the analysis of the Spearman's Rho test at the education level with the use of IUD contraceptives are known that the results of sig. (2-tailed) of .140, and knowledge with the use of IUD contraceptives is known as the results of sig. (2-tailed) of .038. Conclusion : There was no relationship between education level and IUD contraceptive use and there was a significant relationship between knowledge and use of birth control. Acceptors need to be proactive in seeking information about the use of IUD contraceptives through various sources such as print media, electronic media, the internet, and consultation with health professionals. This can help in choosing the right method or contraceptive for them to use.

Autism spectrum disorder (ASD) is a common neurodevelopmental disorder. Increasing evidence suggests that it is potentially related to gut microbiota, but no prior bibliometric analysis has been performed to explore the most influential works in the relationships between ASD and gut microbiota. In this study, we conducted an in-depth analysis of the most-cited articles in this field, aiming to provide insights to the existing body of research and guide future directions. A search strategy was constructed and conducted in the Web of Science database to identify the 100 most-cited papers in ASD and gut microbiota. The Biblioshiny package in R was used to analyze and visualize the relevant information, including citation counts, country distributions, authors, journals, and thematic analysis. Correlation and comparison analyses were performed using SPSS software. The top 100 influential manuscripts were published between 2000 and 2021, with a total citation of 40,662. The average number of citations annually increased over the years and was significantly correlated to the year of publication (r = 0.481, p < 0.01, Spearman's rho test). The United States was involved in the highest number of publications (n = 42). The number of publications in the journal was not significantly related to the journal's latest impact factor (r = 0.016, p > 0.05, Spearman's rho test). Co-occurrence network and thematic analysis identified several important areas, such as microbial metabolites of short-chain fatty acids and overlaps with irritable bowel syndrome. This bibliometric analysis provides the key information of the most influential studies in the area of ASD and gut microbiota, and suggests the hot topics and future directions. The findings of this study can serve as a valuable reference for researchers and policymakers, guiding the development and implementation of the scientific research strategies in this area.

Apnea-hypopnea index (AHI) and nadir oxygen saturation (SpO2) are the indexes used to measure the severity of obstructive sleep apnea (OSA). Obesity, measured by body mass index (BMI), is one of the main contributing factors to the onset and severity of OSA in patients. This study was conducted to find the association between BMI and OSA severity indexes, mainly AHI and nadir SpO2 levels. Polysomnography reports of patients with diagnosed OSA in a teaching hospital were retrospectively reviewed. BMI, AHI, and nadir SpO2 levels were recorded from the sleep study reports of the patients. Spearman's Rho test was applied to find the correlation between BMI and AHI/nadir Spo2 levels. A total of 167 patients were included in the study, comprising 83 males and 84 females. The Mann-Whitney U test was utilized to investigate the association between BMI and gender and age groups. The analysis revealed a significant difference in BMI between males and females, with females having a higher BMI. However, there was no significant difference in BMI among individuals in the early middle and late middle age groups. Spearman's Rho test was employed to explore the correlation between BMI and AHI/nadir SpO2 levels. The results indicated no significant correlation between BMI and AHI (p= .122) or nadir SpO2 levels (p= .239). Contrary to common belief, BMI was not linked to the severity of OSA. It implies that several other factors, independent of BMI, play a role in the disease progression and severity.

BackgroundSurveillance programs are a key element of interventions aiming to reduce rates of surgical site infections (SSIs). The aim of this study was to evaluate rates and trends of SSIs following hip arthroplasty and colon surgery procedures in Piedmont, a region in North-western Italy, from 2010 to 2019. Further, we aimed to assess the burden of SSIs in terms of Disability-Adjusted Life-Years (DALYs).MethodsA prospective cohort study was conducted among 42 hospitals participating in the surveillance system. Procedure-specific SSI rates were calculated and the 2010 - 2019 trend was evaluated using Spearman's Rho test. Patients were stratified according to age, sex and infection risk index according to life expectancy in order to calculate DALYs, using a modified version of the ECDC’s BCoDE toolkit: disease models for both procedure types were adapted to incorporate long-term disability associated with SSIs.ResultsOverall, 20,356 hip arthroplasty and 11,011 colon surgery procedures were monitored over 10 years and were included in our analyses. Hip arthroplasty and colon surgery cumulative SSIs rates were 1.5% and 8% respectively. Using the Spearman’s Rho test, we evaluated a significant downward trend from 2010 to 2019 for colon surgery interventions (Rs − 0.7, p < 0.05), while there was no difference for hip arthroplasty. (Rs − 0.04, p > 0.05).Regarding disease burden, a total of 955.3 (95%CI 837.7–1102.98) and 208.65 (95%CI 180.87–240.90) DALYs were calculated for SSIs following hip arthroplasty, whilst 76.58 (95%CI 67.15–90.71) and 38.62 (95% CI 33.09–45.36) DALYs for SSIs in colon surgery, in 2010 and 2019, respectively.ConclusionsThe significant decrease both in terms of incidence and DALYs found in this study for colon surgery and the stability for hip arthroplasty support the role of surveillance networks in improving quality of care. Despite the smaller SSI rate, the burden associated with SSIs following hip arthroplasty was higher, which is important to consider in light of the aging population in Italy.

The bacteria Mycobacterium leprae is the source of the chronic infectious illness leprosy (M. leprae). In society, leprosy still carries a shame. The erroneous impression of leprosy gives birth to stigma. Leprosy is thought to be brought on by curses, witchcraft, divine retribution, sin, or genetics. A person's perception of leprosy and lack of understanding about it might have an impact on how they feel about those who have it. Even among health students, information alone will not be sufficient to end the stigma against those who have leprosy; also, students need to learn how to develop greater empathy for those who have the disease. This study sought to ascertain the association between leprosy knowledge and stigma and attitudes among nursing students at the professional level. In this study, a cross-sectional methodology is used with a descriptive correlational design. A total 320 professional nursing students participated in the survey. Total sampling was used to select respondents based on inclusion and exclusion criteria. Utilizing the SPSS version 21, data were gathered by questionnaire and analyzed using the Spearman's rho test at a significance level of 0.05. The Spearman's rho test results revealed a positive link between attitude and knowledge (p=0.001), but a negative relationship between attitude and stigma (p=0.000). It was determined that attitudes toward people with leprosy were significantly influenced by information, stigma, and those attitudes. The better the mindset, the more one knows about leprosy. Leprosy patients are treated better when there is less stigma associated with their condition.
