Background: Assessing health technology is a vital tool for determining whether interventions should be continued or stopped. Delivering efficient medical services in remote tribal, hilly regions demands careful planning to maximize limited resources. To identify the effectiveness of telemedicine centres, we conducted an economic evaluation of telemedicine services in term of avoidance of patient transportation costs. Methods: A qualitative telemedicine evaluation was conducted through in-depth interviews with patients and hospital authorities, as well as focused group discussions with medical officers and paramedical staff. By analyzing direct and indirect costs, including setup, operations, and patient-related expenses, as well as patient satisfaction levels, the study provides insights into the sustainability of telemedicine in resource-constrained settings. Main Outcome Measures: Cost-effectiveness of telemedicine services, Reduction in patient transportation expenses, financial sustainability of telemedicine centres, and Patient satisfaction and healthcare accessibility. Results: The analysis reveals that telemedicine can significantly reduce patient transportation costs, making healthcare more accessible and affordable, particularly for underserved populations in tribal and hilly regions. The study also highlights substantial operating costs driven by staff salaries and emphasizes the importance of efficient budget planning and resource allocation. Conclusions: The findings underscore the potential of telemedicine to promote health equity by reducing disparities and improving health outcomes. To fully understand the impact of telemedicine services, a comprehensive economic evaluation considering patient outcomes, quality of care, and long-term cost savings is recommended.

Quality of antenatal and childbirth care in rural health facilities in Burkina Faso, Ghana and Tanzania: an intervention study

To assess the impact of an intervention consisting of a computer-assisted clinical decision support system and performance-based incentives, aiming at improving quality of antenatal and childbirth care. Intervention study in rural primary healthcare (PHC) facilities in Burkina Faso, Ghana and Tanzania. In each country, six intervention and six non-intervention PHC facilities, located in one intervention and one non-intervention rural districts, were selected. Quality was assessed in each facility by health facility surveys, direct observation of antenatal and childbirth care, exit interviews, and reviews of patient records and maternal and child health registers. Findings of pre- and post-intervention and of intervention and non-intervention health facility quality assessments were analysed and assessed for significant (P < 0.05) quality of care differences. Post-intervention quality scores do not show a clear difference to pre-intervention scores and scores at non-intervention facilities. Only a few variables had a statistically significant better post-intervention quality score and when this is the case this is mostly observed in only one study-arm, being pre-/post-intervention or intervention/non-intervention. Post-intervention care shows similar deficiencies in quality of antenatal and childbirth care and in detection, prevention, and management of obstetric complications as at baseline and non-intervention study facilities. Our intervention study did not show a significant improvement in quality of care during the study period. However, the use of new technology seems acceptable and feasible in rural PHC facilities in resource-constrained settings, creating the opportunity to use this technology to improve quality of care.

Hospital Services for Ill Patients in the Middle-Belt Geopolitical Zone, Nigeria: Patient’s Waiting Time and Level of Satisfaction

An important parameter in the assessment of quality health care lies on patient satisfaction. Despite concerted efforts to improve health-care services, patient satisfaction coupled with the quality of hospital care at disposal remain a significant challenge in Nigeria. The purpose of the study was to determine the perception on factors associated with prolonged waiting time and patient satisfaction at the outpatient department of Ibrahim Badamasi Babangida Specialist Hospital in Nigeria. A mixed-method research was utilized. Questionnaire was administered on 95 outpatients, and a focus group discussion (FGD) was held with eight participants. Statistical analysis was utilized to determine the association between dependent and independent variables. Data from FGD were analyzed with NVivo 10. The overall hospital satisfaction was found to be 75.8% among the study population. There was a significant inverse relationship between the level of satisfaction with the doctor and employment status and educational level and a significant direct relationship between the level of satisfaction with the doctor and appointment status and type of visits. The results of FGD show that patients were satisfied with the neatness of the hospital, doctor’s professionalism, and patient–doctor relationship. Dissatisfaction was with extended patient waiting time and the small size infrastructure of the hospital, inefficient handling of patient files by nurse aids, and thoroughness of the physicians. The results showed that majority of the patients were dissatisfied with the waiting time for consultation in the hospital. In other words, consultation time positively correlated with the level of patient satisfaction. To improve the overall patient satisfaction, the waiting time for consultation should be reduced significantly.

Patient Satisfaction Levels During Teleophthalmology Consultation in Rural South India

The application of new technologies for analysis of retinal images, neural networks for automated retinopathy grading, and teleophthalmology have been shown to have great value in ophthalmology. India has only one ophthalmic surgeon for every 107,000 population. Given this situation, teleophthalmology can play a vital role in addressing to the eye care needs of the country. The objective of this study was to assess patient satisfaction levels and factors influencing it during teleophthalmology consultation in India. A patient satisfaction questionnaire was prepared to assess the prominent aspects of patient satisfaction, including teleophthalmology screening, confidence in the technology used to transfer the fundus images, comprehensiveness of information available to the doctor in the central hub, and graded the opinion regarding the teleophthalmology. A cross-sectional survey among patients who attended a teleophthalmology screening conducted across eight villages in rural Tamilnadu. Responses were then subjected to qualitative analysis and conclusions made. The number of respondents was 348. Of this, 56.4% were males. The mean age of was 50 +/- 17 years. Age ranged from 2 years to 83 years. 44.4% of the respondents were satisfied with teleophthalmology screening. (95% confidence interval [CI]: 38.58%-49.42%) No association was found between age, gender, education, and occupation, respectively, with satisfaction levels. We found that patients who asked questions during the screening were 2.18 times more likely to be satisfied with teleophthalmology than those who did not (odds ratio [OR] = 2.19, 95% CI 1.37-3.5). This study highlights sentiments of the rural subjects when they underwent teleophthalmology consultations. This study provides valuable insights about patient's preferences and satisfaction levels with this newer technology.

Background: Health services in India is mainly shared by the Public health sector, Private health sector and by some voluntary organizations, Public health services mainly serve the middle class and lower middle class population in India. Lack of funds and human resource is a major factor contributing to compromised services rendered in this sector. Objectives: The present study was planned to assess the quality of care provided and the patient's satisfaction level in the obstetrics and Gynaecology department of the Government Medical College hospital of Garhwal, Uttarakhand. Material & Method: The study was conducted as a hospital based cross sectional study. A predesigned structured questionnaire was devised and 5 newly admitted patients were interviewed daily between 5-7 pm for a period of two months. The infrastructure and other facilities were assessed by direct physical observation. Result: In the present study it was observed that the department is mainly suffering with lack of manpower and supply of essential drugs. Patients expect better services from the hospital as well as they conclude that the behaviour of the doctors and the paramedical staffs needs further improvement. Conclusion: The department needs better infrastructure, human resource facilities and there is a need to improve the quality of care as well as patient satisfaction.

Background, Caring for nurses is very important for patient satisfaction, this is a measure of the quality of hospital care. The nurses showed me that they provide comfort, attention, care, and maintain health. Meanwhile, the level of patient satisfaction at the Sheikh Yusuf beIum stabiI and beIum hospitals meets the Community Satisfaction Index, because of patient satisfaction, the quality of care is increasing. The purpose of this study is to determine the influence of nurses' caring behavior on patient satisfaction in the quality of health services at Syekh Yusuf Hospital. Methods The type of research used in this study is a quantitative method, which uses a cros sectional study approach. This research was conducted in the Inpatient Room of Sheikh Yusuf Hospital as many as 294 respondents which were determined through the purpose sampling technique. The data in this study were obtained through questionnaires and secondary data from hospitals. The results and conclusions of the study showed that the direct influence between caring behavior variables on service quality was 0.624, while the direct influence between service quality variables on patient satisfaction was 0.115, so the indirect influence between patient centered care variables on patient satisfaction through service quality was 0.072. It is recommended that the Syekh Yusuf Hospital continue to maintain and improve the caring behavior of nurses towards patients, patient satisfaction and the quality of health services.

Satisfaction of patients with directly observed treatment strategy in Addis Ababa, Ethiopia: A mixed-methods study

BackgroundDirectly observed treatment, short course (DOTS) strategy has been a cornerstone for Tuberculosis (TB) control programs in developing countries. However, in Ethiopia satisfaction level of patients’ with TB with the this strategy is not well understood. Therefore, the study aimed to assess the satisfaction level of patients with TB with the DOTS.MethodExplanatory sequential mixed method design was carried out in Addis Ababa, Ethiopia. Interviewer-administered questionnaire with 601 patients with TB who were on follow-up was employed in the quantitative approach. In the qualitative approach telephonic-interview with 25 persons lost to follow-up and focus group discussions with 23 TB experts were conducted.ResultSixty seven percent of respondent was satisfied with the DOTS. Rural residency (AOR = 3.4, 95% CI 1.6, 7.6), having TB symptoms (AOR = 0.6, 95% CI 0.4, 0.94) and treatment supporter (AOR = 4.3, 95%CI 2.7, 6.8) were associated with satisfaction with DOTS.In qualitative finding, all persons lost to follow-up were dissatisfied while TB experts enlightened lack of evidence to affirm the satisfaction level of patients with DOTS. Explored factors contributing to satisfaction include: on time availability of health care providers, DOTS service delivery process, general condition of health care facilities, nutritional support and transportation.ConclusionDOTS is limited to satisfy patients with TB and lacks a consistent system that determines the satisfaction level of patients with TB. Therefore, DOTS strategy needs to have a system to captures patients’ satisfaction level to respond on areas that need progress to improve DOTS service quality.

Moderate relationship between function and satisfaction of total hip arthroplasty patients: a cross sectional study.

Patient satisfaction regarding their hip replacement is often assumed to be directly linked to functional outcomes. We led this study to answer 2 questions: (1) what is the level of patient satisfaction, function, and quality of life after primary total hip replacement (THR); and (2) what is the relationship between patient satisfaction and functional and quality of life PROMs after THR? We led a retrospective study using our institutional registry of prospectively and consecutively collected data on patients after primary THR undertaken between 2004 and 2017. We included 6710 patients with a complete 2-year set of follow-up data for Oxford Hip Score (OHS) (for assessing patient's function), EQ-5D (for assessing patient's quality of life) and satisfaction PROM scores. There was a significant improvement in all OHS and EQ-5D scores from preoperative level, and the mean postoperative satisfaction score was 89/100. We found moderate positive correlations between the patients' outcome satisfaction VAS score and OHS (r = 0.665) and EQ-5D (r = 0.554). THR is a successful procedure generating high levels of patient function and satisfaction. Because patients' OHS and EQ-5D scores may be influenced by comorbidities, those scores are of poor predictive value in estimating patient satisfaction, and therefore should not be used as a surrogate to determine the success of the THR procedure.

Patient satisfaction with referral service and associated factors among public hospitals in and around Addis Ababa, Central Ethiopia

Objective:Studies in sub-Saharan Africa have shown that patient satisfaction with referral procedures and systems is low due to multiple factors. Despite the challenges on patient satisfaction with referral service in Ethiopia, there are only limited studies on overall satisfaction and associated factors with referral services of the hospitals. The purpose of this study was to determine patient satisfaction with referral services and related characteristics among patients referred to referral hospitals in Central Ethiopia, in the year 2021.Methods:A facility-based multi-center cross-sectional study was conducted at public hospitals in and around Addis Ababa using both quantitative and qualitative data collection methods. The hospitals were selected using a simple random sampling method from the list. Data were collected from 317 referral forms and chart reviews, 310 patient interviews, 27 focused group discussions, and 16 insider observations. The collected data were coded and entered into EPI INFO version 7 and transferred to SPSS version 25 for further analysis. Both binary and multivariable logistic regression models were computed to see the association between independent versus outcome variables. The findings were taken as significant with a p value of <0.05 at 95% confidence interval and results were presented with text, graphs, figures, and tables.Result:Overall level of patients’ satisfaction of referral services in Central Ethiopia was 73.5%, confidence interval (67.9%–81.1 %,) and near 85% of the participants had got an examination by the respective health care worker on arrival. This study identified that factors that increase odds of patient satisfaction toward referral services were status of communication (adjusted odd ratio 44.09 (95% confidence interval, 5.47, 355.68), perceived level of experience of health providers who handle the referral process (adjusted odd ratio 6.42 (95% confidence interval, 1.20, 34.42) and coordination status of the hospitals during referral acceptance (adjusted odd ratio 35.50 (95% confidence interval, 4.87, 258.60) and these were significantly associated with patient’s satisfaction toward referral services. Referral without communication among hospitals was stated as the leading challenge by focused group discussants and very limited practice on referral feedback was reported by insider observers.Conclusion:This study determined that the overall level of patient satisfaction toward referral services in Central Ethiopia was moderate with identified factors that increase odds of patient satisfaction toward referral system being the way of communication, health provider’s experience, and coordination status of the hospitals on referral acceptance. Hence, policymakers and the ministry of health would better consider the aforementioned factors during referral program development and training for improvement of patients’ satisfaction with referral services.

Overall patient satisfaction at the Pandega Hospital Inpatient Installation, Pangandaran Regency may be considered less than optimal in terms of suitability and consistency. Concerns regarding appropriateness arise from the lack of services provided by physicians and nurses in a timely and consistent manner. In particular, nurses were not adept at adequately explaining the necessary steps to patients. The level of patient satisfaction is influenced by the standard of service provided, and it is proven that Pandega Pangandaran Regional Hospital is still lagging behind in terms of reliability and responsiveness. The reliability of nurses is still lacking in providing comprehensive, thorough and fast services in accordance with the timeframe and responsiveness that has been determined, with nurses showing reluctance to handle patient complaints. This research was conducted using an explanatory survey method with quantitative methodology. The data analysis methodology used includes instrument testing to assess validity and reliability, research model testing using the coefficient of determination, and hypothesis testing using the t-test. The research results show that the level of service provided at the Pandega Pangandaran Hospital Inpatient Installation is of a high standard. Has the highest score on the empathy indicator and the lowest score on the responsiveness indicator. The level of patient satisfaction at the Pandega Pangandaran Hospital Inpatient Installation is quite high. Has the highest score on the aesthetic indicator and the lowest score on the suitability indicator. The research results found that service quality has a strong and beneficial influence on patient satisfaction at the Inpatient Installation of Pandega Pangandaran Regional Hospital. Thus, there is a direct relationship between the level of excellent service and patient satisfaction at the Pandega Pangandaran Hospital Inpatient Installation.

The low quality of nursing care of patient expectations not only due to the limited number of nurses , but due to the limited professional skills possessed by most nurses . This research is quantitative research with cross sectional approach to the relationship between the dependent variable and independent variables by means of observation or data collection approach as well . Analysis is conducted multivariate logistic regression analysis were used to examine the relationship of independent variables simultaneously with the dependent variable . Also can be known where the dominant variables associated with the dependent variable . The study sample as many as 87 people , with a random sampling technique sampling Of the 87 respondents there is a relationship between empathy officer with the level of patient satisfaction p value = 0.000 , there is a relationship between physical appearance officer with the level of patient satisfaction p value = 0.005 , there is a security relationship with the level of patient satisfaction p value = 0.019 , there is a relationship between speed officers with patient satisfaction level p value = 0.005 . However, after multivariate analysis test to look for the most dominant factor between the independent variables and the dependent variable showed that the most dominant related to patient satisfaction is the speed of the clerk where p value = 0.000 . Improving Cancer Patients Satisfaction with Treatment Process: A Pilot Interventional Approach in a Tertiary Care Hospital in Karachi, Pakistan

Background and Objective: Chemotherapy is one of the treatments of choice worldwide to combat cancer. The complex nature of chemotherapeutic agents results in multiple physiological and psychological issues and if the patient and their caregivers (family) are unaware of these effects, poses additional stress which may result in patient dissatisfaction. This pilot project aimed to provide patients'/families' awareness regarding the process of treatment and subsequently to improve their satisfaction level. Methods: This academic project is a mandatory assignment of clinical practicum course (for the degree of Masters in Nursing Sciences (Aga Khan University School of Nursing &amp; Midwifery). An interventional design was employed for this study. A total of 30 cancer patients, that is 15 in the intervention group and 15 as controls were recruited. The project was initiated as an academic requirement (started in Jan 2015) and last for four months (Apr 2015). Patients planned for their first chemotherapy cycle were selected in both groups. A self-developed questionnaire was used to assess the patient satisfaction level regarding the chemotherapy process before and after the standardized and video-based educational intervention. An independent T-test was used to analyze the data. Results: The overall mean score of patient satisfaction level after the intervention was 18.80; whereas, it was 11.93 for another group. The patient's mean satisfaction level was higher for the intervention group. Conclusion: This study indicates video-based intervention for pre-chemotherapy teaching as an effective educational mode to assist patients throughout the treatment process. It can also facilitate in reducing treatment-related stressors and enhance patients' satisfaction levels.
