Abstract

The impact of plaque morphology on the risk of stroke in patients with atherosclerosis of the carotid bifurcation has been a matter of interest for more than 40 years…. nearly as long as carotid endarterectomy (CEA) has been in existence!1 In the era of modern medical therapy for atherosclerosis, there are some who continue to believe that asymptomatic carotid disease should always be treated with optimal medical management as opposed to any carotid artery intervention. Although most vascular surgeons would agree that plaque morphology is exceedingly important, we continue to make our decisions regarding carotid artery intervention primarily based on the degree of stenosis.
