Abstract

With the aim to promote a seamless journey of patient care from diagnosis through to aftercare, coupled with improving quality and possibly reducing the length of hospital stay, an integrated care pathway for patients undergoing colorectal surgery incorporating the enhanced recovery program (ERP) was developed by the colorectal team in Jersey, Channel Islands. Applying the principles of the ERP, the pathway document encompasses six key stages: referral; assessment; surgery; postoperative management; aftercare; and follow up. Developed by a clinical team responsible for its application in practice, the pathway′s formation followed a systematic process that included: establishment of aim, objectives and scope; application of evidence-based practice; care sequence mapping; staff training; piloting; ratification and rollout.
