Abstract
Throughout 2020, the SARS-CoV-2 virus became widespread, resulting in a pandemic. To date, the USA has experienced 28+ million cases and 500,000 deaths. Data shows that 80% of cases are mild or moderate COVID-19, while 20% experience severe COVID-19. The COVID Front Line Care Team (MD/PA/NP/RN) was developed to provide telemedicine care for patients, in order to rapidly notify, risk stratify, contact trace, isolate, and provide support for patients. Approximately 70% of individuals with severe COVID-19 along with 10% of mild-moderate COVID-19 cases go on to develop Post COVID Syndrome (PCS).
