Accidental and Deliberate Self-Poisoning with Medications and Medication Errors among Children in Rural Sri Lanka.

Context. Pharmaceutical products are the leading cause accidental poisoning in middle- and high-income countries. Patterns of poisoning with medicinal drugs change across different geographic regions and over decades owing to variability in prescription practice, sociocultural factors, safe storage of medicines, and free availability of over the counter medications. Methods. This multicentre descriptive study was conducted over a seven-year period (February 2007 to January 2014) to assess patterns and trends of medicinal drug-related poisoning among children less than 12 years of age in thirty-six hospitals across rural Sri Lanka. Children with both accidental and deliberate medication poisonings and medication errors were recruited to the study. Data on poisoning events and medication errors were gathered via patient/parent interviews using multistructured questionnaires that assessed demographic factors, first aid measures, location and circumstances of poisoning, clinical management, and complications. In addition, focus group discussions were performed on all children and their families who had deliberate poisoning events and medication errors. Results and Conclusions. Among 1621 children presented with acute poisoning over seven years of age, 410 children had acute poisoning with medications. Male children (225, 54.9%) outnumbered female children. Paracetomol (137, 35.6%), salbutamol (55, 14.3%), and chlorpheniramine (35, 9.1%) were the most commonly poisoned medications. Prospective data at Anuradhapura teaching hospital (n = 112) revealed that unsafe first aid measures were practiced on 22 (19.6%) children. Although the majority of children remained asymptomatic (61, 54.5%), neurological symptoms (34, 67%) were predominantly seen in symptomatic children. The majority of poisonings took place within home premises (76, 67.9%). There were 16 reports of medication errors (14.2% of acute poisoning events) either due to erroneous administration by caregivers or erroneous issue of medicines by health workers. The current study did not observe mortality following medication poisonings. This study brings to light the burden of medicinal drug-related poisoning morbidity among children in rural Sri Lanka. Potentially, interventions such as community educational initiatives, written safety warnings, increased use of child resistant containers, and enforcement of laws to bring down accidental medication poisonings need to be implemented, and their effectiveness should be evaluated.

The integrity of important operating system files, programs or data controled of hash function. Hashing is used to construct associative arrays and duplicate in a series of datasets, build unique identifiers for a set of data, with a view to determining the Checksumming accidental or deliberate errors in you save or transfer, to save passwords on systems of protection, in the formulation of an electronic signature. More recently, industry related cryptographic hash faced a significant challenge-sustainability to multikolizijam that use attack Zhuks. In his work Zhuks has shown that sustainability, calculates the hash value by cascading these functions, not much more than the stability of one of them. In addition, the known hash algorithms do not allow to fully address the issues of ensuring the durability and performance of cryptographic algorithms. Therefore, developing new and improving existing hash functions with a view to enhancing the effectiveness of cryptographic protection will not lose its relevance. With this in mind, in the work of the proposed hash function newMD4, which was developed on the basis of the original hash function MD4. Designed hash function newMD4 has several innovations compared to MD4: if you are compressing instead of four 32-bit variables proposed using five 64-bit variable increased the length of the hash value to 256-bits; replaced by additional functions added additional operations at each stage. In the work of the experimental research on the evaluation of Expressway and the statistical characteristics of the proposed hash function. Under the same conditions, conducted pilot studies to assess mental speed characteristics, which showed that the hash function newMD4 faster than the original MD4 in 1,43 times. For research of statistical characteristics of NIST tests were used in the STS, at the same hash function used to generate sequences, statistical characteristics of teristiki which tested the specified tests. According to the research results, consistency, using the hash function sgenerirovanye newMD4 showed better statistical characteristics compared to the original.

The Elusive Nature of Truth in Scientific Studies and the Importance of Peer Review.

Constructive, critical peer review is an essential part of the information creation process that has evolved in our society. Journals constitute an essential component of the information dissemination infrastructure because, unlike meetings, the information contained therein is or needs to be subject to the rigors of peer review. Unfortunately, the peer-review process does not always operate as well as it should. For example, Bohannon1 submitted an intentionally flawed manuscript, concerning cancer, to 304 journals as part of an experiment to test the peer-review process. This paper was accepted by 157 of those journals. Godlee et al.2 submitted a paper with eight deliberate errors (in study design, analysis, and interpretation) to reviewers of the British Medical Journal. Among the 221 respondents, the average number of mistakes detected was two, and some reviewers found no mistakes in the manuscript. We are not perfect, and detection of errors in the peer-review process is not surprising. It does highlight, however, the need both for vigilance (among editors and readers) and an ongoing process for improvement. Because of the importance of peer review to the quality of Translational Vision Science & Technology (TVST), we are indebted to the TVST reviewers for the immensely important contributions they have made to the quality of the journal and for the service they have provided to the entire vision research community. Data quality and DRGs: an assessment of the reliability of federal beneficiary discharge data in selected Manhattan hospitals.

New York County Health Services Review Organization (NYCHSRO), the physicians' professional standards review organization of Manhattan, examined whether diagnostic coding errors identified in Manhattan hospitals would affect reimbursement under a diagnostic-related group (DRG) method of financing inpatient services. A sampling of 1,027 Medicare and Medicaid cases representing discharges from 18 Manhattan hospitals during 1982 and 1983 revealed incorrect DRG assignment for 17.5% of patient record abstracts, but these appear to have been unsystematic rather than deliberate errors. The difference between estimated reimbursement based on original and reabstracted records was not statistically significant either in the aggregate or for specific hospitals. It is emphasized that while New York State's Prospective Hospital Reimbursement Methodology (in effect during the study period) is not solely dependent upon DRG's case-mix is one of several factors used to make adjustments to existing per diem rates. A key recommendation is that hospitals conduct internal monitorings with all involved departments to improve the quality of the data abstracting process.

Run-Length-Limited Low-Density Parity Check Codes Based on Deliberate Error Insertion

We propose a novel approach to modulation and error control coding. The idea is to completely eliminate a constrained encoder and, instead, impose the constraint by the deliberate introduction of bit errors before transmission. The redundancy that would have been used for imposing the constraint is used in our scheme to strengthen the error control code (ECC), in such a way that the ECC becomes capable of correcting both deliberate errors as well as channel errors that occur during the detection. Our ECC-modulation scheme is based on iterative decoding of low-density parity check codes and a run-length constraint.

Alternating aperture phase shifting mask (AAPSM) technology is finding increased use in the patterning of critical layers due to the enhanced resolution and decreased linewidth variation characteristic of this technique. The potential advantages of AAPSM processes must be weighed against the increased complexity of reticle layout, higher reticle cost, and heightened sensitivity to parameters such as lens aberration. This work details the effect of shifter trench depth on patterning performance for the 100nm node. Data was collected at an exposure wavelength of 193nm using reticles built with deliberate errors in shifter trench depth. Differences in patterning performance observed as a result of these variations are compared with the impact predicted from modeling.

The utility of generative artificial intelligence Chatbot (ChatGPT) in generating teaching and learning material for anesthesiology residents

The popularization of large language chatbots such as ChatGPT has led to growing utility in various biomedical fields. It has been shown that chatbots can provide reasonably accurate responses to medical exam style questions. On the other hand, chatbots have known limitations which may hinder their utility in medical education. We conducted a pragmatically designed study to evaluate the accuracy and completeness of ChatGPT generated responses to various styles of prompts, based on entry-level anesthesiology topics. Ninety-five unique prompts were constructed using topics from the Anesthesia Knowledge Test 1 (AKT-1), a standardized exam undertaken by US anesthesiology residents after 1 month of specialty training. A combination of focused and open-ended prompts was used to evaluate its ability to present and organize information. We also included prompts for journal references, lecture outlines, as well as biased (medically inaccurate) prompts. The responses were independently scored using a 3-point Likert scale, by two board-certified anesthesiologists with extensive experience in medical education. Fifty-two (55%) responses were rated as completely accurate by both evaluators. For longer responses prompts, most of the responses were also deemed complete. Notably, the chatbot frequently generated inaccurate responses when asked for specific literature references and when the input prompt contained deliberate errors (biased prompts). Another recurring observation was the conflation of adjacent concepts (e.g., a specific characteristic was attributed to the wrong drug under the same pharmacological class). Some of the inaccuracies could potentially result in significant harm if applied to clinical situations. While chatbots such as ChatGPT can generate medically accurate responses in most cases, its reliability is not yet suited for medical and clinical education. Content generated by ChatGPT and other chatbots will require validation prior to use.

Computerized Orders with Standardized Concentrations Decrease Dispensing Errors of Continuous Infusion Medications for Pediatrics

The use of continuous infusion medications with individualized concentrations may increase the risk for errors in pediatric patients. The objective of this study was to evaluate the effect of computerized prescriber order entry (CPOE) for continuous infusions with standardized concentrations on frequency of pharmacy processing errors. In addition, time to process handwritten versus computerized infusion orders was evaluated and user satisfaction with CPOE as compared to handwritten orders was measured. Using a crossover design, 10 pharmacists in the pediatric satellite within a university teaching hospital were given test scenarios of handwritten and CPOE order sheets and asked to process infusion orders using the pharmacy system in order to generate infusion labels. Participants were given three groups of orders: five correct handwritten orders, four handwritten orders written with deliberate errors, and five correct CPOE orders. Label errors were analyzed and time to complete the task was recorded. Using CPOE orders, participants required less processing time per infusion order (2 min, 5 sec ± 58 sec) compared with time per infusion order in the first handwritten order sheet group (3 min, 7 sec ± 1 min, 20 sec) and the second handwritten order sheet group (3 min, 26 sec ± 1 min, 8 sec), (p<0.01). CPOE eliminated all error types except wrong concentration. With CPOE, 4% of infusions processed contained errors, compared with 26% of the first group of handwritten orders and 45% of the second group of handwritten orders (p<0.03). Pharmacists were more satisfied with CPOE orders when compared with the handwritten method (p=0.0001). CPOE orders saved pharmacists' time and greatly improved the safety of processing continuous infusions, although not all errors were eliminated. pharmacists were overwhelmingly satisfied with the CPOE orders.

This chapter explores the question of why, given all his philosophical interests, Proust chose to write a novel and not a treatise. Drawing in part on what Proust’s narrator says about fictional novelist Bergotte, fictional painter Elstir, and fictional composer Vinteuil, it suggests a number of effects the experience of reading In Search of Lost Time can have on us, precisely because it is a novel. We stand, first, to learn about ourselves; second, to gain a clearer feeling of what it’s like to mishear a sound, misperceive a face, remember involuntarily, and meander toward the truth; third, to feel our way into powerful formal models for the shape of our lives; and last but not least, to be trained in some valuable mental habits. As we make our way through this long, beautiful, and complicated novel, with its twists, turns, contradictions, deliberate errors, and intricate sentences, we get practice in storing material under advisement, reasoning probabilistically, revising earlier assessments, and thinking twice. Specific formal devices prompt particular forms of engagement, and those forms of engagement fine-tune our mental capacities. In Search of Lost Time could not have been an article, treatise, or textbook; it had to be a novel.

Efficacy and clinical use of novel high-resolution EPID-based 2D and 3D automated patient-specific quality assurance for SRS and SBRT patients

Stereotactic radiosurgery (SRS) and stereotactic body radiation therapy (SBRT) treatments should be delivered with caution because any delivery error can have a severe radiobiological impact on the patient due to the high dose per fraction used in such modalities. Therefore, a very high accuracy including a quality assurance program accompanying the entire treatment process is required. This study assessed the efficacy and clinical use of novel high-resolution 2-dimensional and 3-dimensional electronic portal imaging devices, an automated patient-specific quality assurance (PSQA) for SRS and SBRT patients. The dosimetric performance of these devices was evaluated by comparison with other detectors. Deliberate errors (errors of multi leave collimators (MLCs), gantry and collimator angle, and monitor unit (MU) changes) were introduced to assess the sensitivity of the devices. Twenty volumetric modulated arc therapy (VMAT) plans from various tumour sites, constrained to field sizes below 7.7 cm × 7.7 cm, were analyzed. Point dose measurements showed mean dose differences of 0.1% ± 1.5% for ionization chambers and 0.94% ± 1.8% for EPID compared to treatment planning system (TPS) calculations. With 2%/2 mm, average Gamma Passing Rates (GPRs) were 98.6% (film), 99.0% (SRS MapCHECK®), 98.7% (PDIP), 99.5% (PerFRACTION™,2D), 99.8% (PerFRACTION™,3D), and 93.2% for ArcCHECK®. At stringent 2%/1 mm criteria, high-resolution systems maintained > 94% pass rates while ArcCHECK® dropped to 73.5%. EPID showed comparable GPRs to films and SRS MapCHECK®. EPID detected most errors except small MLC shifts and gantry angle errors. It is concluded that EPID dosimetry can perform SRS and SBRT PSQA without additional devices. Thus, it offers a cost-effective, high-resolution alternative comparable to dedicated devices, enhancing clinical efficiency, without compromising accuracy.
