Abstract
ABSTRACT The journey towards high-reliability care is challenging for any organization, but particularly so for one whose ethos has historically straddled the healthcare and human services cultures without a strong data-driven quality management program. Such a transition, while bringing with it great strides in patient safety and improvement in quality of care, can also be associated with considerable culture change. We describe a transition journey undergone by a special hospital for those with developmental disabilities to a data-driven quality improvement and patient safety culture on a high reliability journey to deliver exceptionally safe, high quality care to every patient, every day.
